    I had a finger PIP joint injury on my middle finger around 6 years ago.. since then my PIP joint has been thickened and swollen and the swelling actually had never gone down completely.. there is also some low grade instability in my PIP joint.. but i do not have any movement restrictions and pain.. is it actually normal, that the swelling around my joint is not going down anymore?

    • @danimoffit9825
      @danimoffit9825 8 днів тому

      Normal is not the right word. Typical? Yes. We use our hands so much that the joint may never fully heal, especially if it's your dominant hand. If it has been this long, there has probably been a thickening in the capsule, which is what you actually are seeing, rather than swelling. As long as you don't have disability, I would say learn to love the finger as it is!

      @@danimoffit9825 thanks for your comment!.. as youve mentioned earlier that i should "learn to love the finger as it is" is quite a complicated statement for me.. i might not have any restrictions on using my hand, but i just cant accept the fact that my finger is thickened and has a different form then my any other fingers.. my body might have accepted and adapted to my fingers situation, but my brain didnt.. its really hard to accept the fact that i have to live like this.. i think i have a lack of acception for difficult situations.. since i dont accept my fingers form, it is something that like my brain is always telling me sonsense stuff that i should fix my finger, so that it comes to the old state and is always manipulating me.. although, lots of doctors had told me that its normal that my finger is thicker then normal and will stay like this forever.. there is nothing they can do.. but my brain always tries to look for ways to fix my finger again.. i think my brain is playing with me, since i cant accept the situation of my finger.. i think i have some mental health problems..

    • @danimoffit9825

      Because both bones have fractures, you do not want to hurry the healing process. You can start with wrist flexion and extension exercises, but pronation and supination (rotating palm up/palm down) may put too much stress on the bones. You can start some flexion and extension of the elbow. It may feel "stuck" when you're trying to straighten your elbow, so you can work on that using gravity to help you (lay on your stomach on your bed or couch and allow the arm to straighten as much as possible). Again, stay away from pronation/supination until you see your orthopedic provider. Good luck!

    My mother just suffered a comminuted spiral fracture of her left humerus and is in tremendous pain. The ER only gave her xray…left the sling on her arm the EMS placed(losely) and gave her 1 Norco then sent her home with an orthopedic referral. Her appointment is in 3 days. She’s barely gotten through 24 hours and she can’t bear for me to try and put a new sling on and can barely handle a stabilizing strap. Any suggestions? She’s older and has heart problems so I’m not sure what to do. Can’t move without pain. Not even sitting down…trying to walk…even laying down she’s crying. Of course no one in the ER explained what the fracture actually was…si thank you for this video

      @danimoffit9825 2 місяці тому

      I am so sorry to hear about your mother! It sounds like patient-centered care was NOT included in the visit! The humerus is a major bone; I'm quite frankly shocked there wasn't an orthopedic consult in the ER. However, that is past so right now, my suggestions are these: 1. Heat or ice may help with the pain. Heat will probably be more comfortable, so if you have a heating pad, use it. 2. Alternate taking over-the-counter painkillers. You can use both Tylenol and Ibuprofen to help with pain. Space them out so every 2-4 hours she's taking one or the other. 3. Try placing a pillow or cushion under the arm to help support it. If she's on her back, having that will help prevent extra stress that would occur if there's not something in-line with her body. It may help when being seated, as well. 4. The sling is to protect the arm, but if she's not moving, it's not required. Often, the sling is to show OTHER people there's something wrong and to avoid it. I hope one of these suggestions will work for her. Again, I'm sorry she is going through this. I really hope the orthopedic appointment is positive for her.

    I'm a healthy slim 58 yr old male, fell hard and fractured greater trochanter, fixed with dynamic hip screw 12 hours later. All was going well, discharged from hospital 72 hours later, but 48 hours later my whole leg swelled up, lots of bruising, fluid, knee and foot throbbing, feeling very tight, so went back to A and E wondering if clots, it wasn't but they x rayed hip and found a new greater trochanter avulsion fracture. I was advised to minimize weight bearing on right foot for 5 weeks, that physiotherapy would be delayed and return for more x rays in 5 weeks. Those showed original fracture doing well, no change to avulsion fracture. I'm told that will probably heal itself in time, but there's a chance it may not so more x rays again in 5 weeks. I've now been given an appointment for first physio session and told I can weight bear as much as comfortable. But my knee, esp behind it and calf is still incredibly stiff, tight, I can only bend about 40-50% compared to my left leg, (they said I would normally be able to bend more at this stage) and the foot still sometimes throbs/burns a bit, especially evenings. Could this be a result of the avulsion fracture, and from your experience are you able to give any likelihood of the avulsion healing itself, or needing surgery? Obviously difficult to say without knowing specifics I realize.

    • @danimoffit9825
      @danimoffit9825 2 місяці тому

      Sir, I'm sorry you're going through this! You would hope that the screw would have been enough! The majority of avulsion fractures I have seen have been on adolescents, so I can only comment on that . . . They are young and have incredible healing capacities; the older we are, the more time it takes. To answer your question, though, I have never had a person need surgery for an avulsion fracture. All have healed on their own. Depending where the avulsion is, non-weight-bearing was important to the healing process at the beginning, with increases in weight-bearing throughout the process. An avulsion is a fracture; you are still at a minimum of 6 weeks, and longer depending on the severity. Throbbing/burning indicates that nerves are working! They may have been damaged (which is normal) during the initial surgery and they're trying to heal. The nerves in your leg come from your spinal cord and travel down the leg, branching as they go, which is why you may be having the symptoms lower in your leg. Trust the process. Your age (sorry, I'm there to) will slow things down a bit, but with time, you should make a recovery. Good luck with everything!

    In 2018 when I was 10 years old, I was trying to get up a concrete plateform. I hit my knee aginst the platform edge very hard. In my right knee, above the patella, I got hit. Since then, it has happened 6 times that while running, my patella displaces slightly, I lose balance and fall down. It is very painful. I can hear its sound while falling. I think I damaged my medial retinaculum or vastus medialis.

    • @danimoffit9825
      @danimoffit9825 5 місяців тому

      It's more likely to be the retinaculum than the muscle. This many years later, your muscle would have healed. Unfortunately, the retinaculum can get damaged and it doesn't repair itself. If your patella is continuing to sublux, I would suggest you fine a rehabilitation specialist (e.g., PT, athletic trainer) to teach you exercises to help prevent it from happening. You can also get a brace with patellar stability, but make sure someone can help you find the correct one. Find a good specialist to help you!

      Depending on where it hurts, it can be more than a sprained wrist. I suggest (if possible) that you see a physician, preferable one that has a background in orthopedics. I can't imagine you would have fractured anything, just from the mechanism of injury. An emergency room trip is going to lead to x-rays and a referral unless you are lucky enough to have an orthopedic doc on call. If you can see someone who knows bones and/or the wrist will be your best bet. IF you know an athletic trainer, you might want to ask them to take a look at it too. A wrist brace that can be purchased at a drug store may help with the pain while you're doing your work. You can have a soft-sided one, or one that has a hard cast-like stay. That is up to your comfort. Good luck . . . I hope you can get it looked at!

      Plica is something you are born with, it can't be developed. Did you have an MRI to confirm? I ask only because I have had patients who the MDs thought were meniscal tears and in surgery realized it was plica. Lateral patella compression is a tilt of the patella, which can be relieved with taping techniques and rehabilitation/strengthening. Were you referred for either? It may be worth your time to look into seeing an athletic trainer or physical therapist to help with that. It may or may not be plica, but try to get some help first, before jumping to that diagnosis.

      @@danimoffit9825 thank you for your response. I really like your videos you’re very informative. yes I have had this pain for 2 years.. it has gotten better through strength exercises. I have made a lot of progress. But in the last 6 months I have developed a weird sensation.. When I stand up or extend my knee to 0 degrees after sitting for a long period of time I get this “squishing” type of pain it subsides shortly after.. My knees used to never feel hot, now the more I exercise or walk I get this burning/hot sensation. I have one knee that’s worse than the other. when I bend my knees and contract my quads my VMO on the bad knee doesn’t want to contract but the rest of the quad does. weirdly enough if my other VMO fires perfectly and makes an abrupt upward motion as a good response. I have no pain with stairs, have no pain running for short periods. My pain is worse in the first 40-60 degrees knee flexion, the deeper I go the better I feel. Around that 40-60 degree mark my kneecap or something under this cracks or slips.. the more I make this slipping popping sensation my knee starts to burn up. I’m thinking I have some sort of tissue impingement that is causing this to occur, maybe it’s stuck under my kneecap. The symptoms just don’t perfectly match up to PFPS or lateral patella pain. I had an xray that saw my kneecap has became tilted and subluxes on the sunrise view in I think what is taken at 30 degree knee flexion, got an mri 1 week ago and I have got the results from a radiologist it says everything looks perfect except “mild joint fluid buildup” the mri was taken with knee fully extended and it said the dynamic stabilizers lateral/medial are in perfect shape, the kneecap is in perfect position. I think what has happened recently is there is something on the medial side underneath my kneecap or the fat pad has impinged and lodged itself under my kneecap subsequently causing inhibition of the vmo, creating this excessive pressure but only during knee flexion.

      You shouldn't be able to move your patella when the leg is extended. It may not be a full subluxation, but the laxity is not good. If it continues, you have a much greater chance of dislocating it. It may be worth having it seen by an orthopedic physician. It could be due to muscle weakness, but you may have damaged the other tissues that should keep it in place. You may be able to strengthen it with rehabilitation.

As long as it isn't painful for you, I wouldn't worry about it. If it does start getting painful, then you may need to consider a second opinion.

      @danimoffit9825 9 місяців тому

      I'm sorry to hear you're not completely healed. If the avulsion was the rectus attachment, there will be a delay in the healing. In reality, the bone could take up to 2 years to heal . . . Which it sounds like you have now reached. My suggestion is to do exercises not only for strength, but also for endurance. When you are doing exercises, do both high weights with low reps (e.g., 3 x 5-8, slow) and low weights with high reps (e.g., 3 x 15, fast but controlled). Additionally, you need to be doing both knee and hip exercises. The hip is often neglected in favor of knee extension exercises. Make sure you're including things like dead lifts, which focus on hip flexion. Take advantage of the internet. Look for hip flexion and knee extension exercises and make sure you're doing both. Good luck!

I did all activities with this knee hyper extension for 5 months before i diagnosed and fixed my posture to stop knee hyperextension. The hyper extension and hinging on the ligaments left me with loose knees. Highly likely that the LCL is stretched via multiple microtares in the last 5 months. as i have lateral knee instability in that direction, when i have my knee bent (lcl is more loose with more knee flexion). Hi, I am 16 and throughout my life I have had occurrences where I have bent my right knee and it feels like it suddenly seizes up and hurts, I then have to straighten it out to hear it pop back in place and the pain goes away. I would say I’ve had it happen about 4-6 times throughout my life since I was about 11 maybe. It happens very infrequently. However, the instant pain of it has always made me extremely apprehensive when bending my right knee or kneeling and stuff like that. I have never worried too much about it and it hadn’t happened for awhile but it did today. Since it hadn’t happened in awhile I haven’t been apprehensive much of bending my right knee, but now it’s going to be in the back of my mind for quite awhile. It’s always so random and the fear of it happening/ the fear of the pain is quite strong. I went to crouch to get something from a low shelf while I was at work when I felt the sudden seize in my knee as I bent it, and being familiar with this I quickly stood up even if it hurt so it could pop back in place, since I was at work I had to react quickly because I didn’t want any issues and just had a bit of a limp for a couple minutes but it wasn’t too bad. Just felt a little pressure/ pain at the knee occasionally when bending/ moving it but barely. I’ve never had much issues with the aftermath, usually just letting my leg rest and it feels fine within a day or two. However it concerns me that this is still happening to my knee after so many years and I was thinking I should probably get it looked at, after some research I was thinking maybe I had patella subluxation, or something similar. I also noted that literally yesterday I had been using my knee a lot, doing a lot of crouching and kneeling when I was cleaning for hours yesterday which I don’t normally do, which, with the timing, makes me suspect it may be why my knee did that when it hadn’t for at least a year or two. I am just looking for another opinion, but I probably will go see a doctor about it soon anyway. Thanks :)

      Hello. It sounds like you have really thought about this. This nice thing is if this is what is truly happening, it hasn't progressed to something more serious. However, if you don't take care of what is happening, you can end up with life-long issues. Your patella is not supposed to move like that and the abnormal tracking will destroy the articular cartilage, leading to osteoarthritis. My point is, please get it looked at. If you have an athletic trainer at your school, they may be able to show you some exercises that will help decrease your problem. If that's not possible, please go to an orthopedic physician who can help you in various ways, including referring you to rehabilitation. It may not seem like a big deal, but 40 years down the road, you don't want to have to have a knee replacement because it didn't get taken care of now. Good luck with this!
