At 1:39, 10:07 I’m sharing one of the reasons why I’m relocating to Germany to begin medical school there and subsequently complete my degree. The notion that the doctor must file the claim is also repulsive to me. The fact that I have to communicate with a third party to explain my needs to them in order to receive payment is simply unacceptable. In my opinion, this is an evil practice. I know individuals residing in Third World countries who have mastered this system for decades. For instance, my father underwent eye surgery in Cairo in 2006, and everything went smoothly. He recovered quickly, and the procedure was relatively new at the time. My mother had LASIK surgery in 2007. Recently, I heard about someone who had to undergo the same surgery as my father in 2024 in a supposedly developed country. This individual suffered from degeneration and poor eyesight in one of her eyes, effectively rendering her disabled. To me, it’s incomprehensible that Egypt offers superior healthcare compared to America. However, I suppose things may have changed since then.
It's great that you share this here. I believe it's possible to deliver good medical care in nearly any healthcare system as long as we can accept the limitations of that system. For example, somewhere in the middle of Asia or Africa I may have no resources so the clinical aspect will be minimal but the patient care will dominate. In a highly privatized healthcare system like the US the only way the physician can make it to another day to practice medicine is by satisfying each and every burgeoning guideline and rule and only then can they take care of the patient, defensively. Both of these systems are limited in their own way but one can do their best in each. Having volunteered at an orphanage in Tijuana, Mexico and having practiced in the US, like yourself, I can't imagine how this modern healthcare system is good for the patient.
If healthcare is a business driven by profit and protection against liability, does this make quality of care and clinical outcomes better or worse?
Clinical outcomes is an obfuscated term with metrics determined by commercial interest. An actuary might measure my "clinical outcome" based on how good my A1C is which is a terrible metric pertaining to the actual patient; I can deliver fabulous A1Cs if you let me inject you with insulin, the problem is that you'll become insulin resistant rather quickly. If you let me do a lot of colonoscopies on people I'll find a lot of colon cancer and that, too, can be a great clinical outcome metric but catching cancers that won't kill you are terrible cancers to catch. As for healthcare, it's not just a business that's driven by profits, it's also the physical therapist next door, the home health nurse, the adult daycare coordinator, the medical assistant, the physician with the private practice, and the pharmacy tech. It's too big to be lumped into one thing. Which means that for the healthcare consumer it's more important than ever to know all the moving parts and how to pick and choose the right key players according to their health standards.

The sanitary conditions and processes are quite rigorous. However, in certain health settings such as the US, we put on more of a show that is not based on clinical science. So I mention my observations because many Americans might be accustomed to a certain standard which they believe is necessary for good healthcare delivery but in fact it just creates a lot of waste.

Hey! Yes, still have my base in Europe out of Santiago. It's listed on home exchange so if you find it, you're welcome to stay there. It's a lovely part of town and though the summers get a little busy inside the city center, you can always find great spots nearby where the locals hang out.
This post gave me some helpful insight to delineate the practice I am trying to build but at the same time I find it very hard not to overlap health coaching with being a physician. I understand your point of view medical legally but as you perform preventive care medicine aren't you coaching? I dont see how they are completely exclusive from one another?
Fortunately, any physician can also do health coaching with their own patients. It's just that if you are only doing health coaching, you cannot also practice medicine. That's the tough thing about it.

Makes sense , the board investigation traumatized you.
The medical board investigation was definitely traumatic though not every trauma leads to PTSD. It's important to be cautious with offering up any kind of online diagnosis of a condition such as PTSD, both to yourself and anyone else because it's a pretty involved diagnosis with serious consequences. If any doctor watching these videos is experiencing PTSD, that's as much of a mental health emergency as anything and I would recommend reaching out for help right away.

Hllo sir i am from india i want to come to spain for my study but i have some health issues i have mctd problem i taking treatment in india ....i want to know that if i come to spain can i continue my treatment there from my reports and prescription from india???
It would be best to contact a local physician or hospital group to find out. I don't think there is a treatment in India you can't get here in Spain but I'm not an expert on the topic.

Definitely. For issues that aren't life threatening the wait time is rather long. There is the private system where you can get much faster access if waiting isn't ideal.

@@GhassanJneinatyIf you are not a Spanish citizen or if you have not been in the country legally working and paying taxes for several years, you will have to choose a compulsory private insurance because the public health system is for Spanish citizens paid with our taxes, not for foreigners to abuse it.

Any demand for English teachers or would I have to bus an hour to another town if I lived here?
Most people will bus out - SDC is rather saturated with English teachers.
Do you limit your patients within the 15-20 states that allow out of state prescriptions?
In my practice I don't prescribe a lot but when I do my patients have to be in the 2 states I'm licensed in. I don't send prescription outside of these 2 states.
But does it really work if we don't give patients personalized advise?
It depends what your patients are looking for. If they want a quasi doctor I think it's too risky. If they want a real doctor then that's where you should direct them. If they want to ask you your expertise and use your knowledge to improve their health then they aren't going to look for personalized advice. "What would you do if you were in my shoes" is very different from "So what's your diagnosis of my condition and what should I do?"

Seems like everyone is raising their prices in Q1, a cascade started for need or for greed?
I suspect the need vs greed is in the eyes of the beholder. Every physician should set a price that's in line with the value they offer, their income need, and what the market bears. If you can offer more value to your client then the extra money they spend should be of value.
Are there any full-time jobs for internal medicine telehealth?
Quite a few now since there are lots of telehealth companies now.
I suspect that….. if insurance companies would stop ping-ponging everyone around, maybe more harmonious doctor/patient relationships could be established. I appreciate the clinic I work in now, for the structure of transparency around the insurance coverage. There is so much work that goes into insurance & prior authorizations. So. Much. Work. Also please don’t hate us front desk staff. PLEASE TAKE CARE OF YOURSELVES & don’t waste your appointments on us! The nature of our work is to play “monkey in the middle” all day long & we are the monkeys! 🙈 🙉 🙊
Thank you for sharing your experience. The amount of admin work in the middle is staggering. There are 100 micro decisions behind every decision you and your physician make and that's why the overhead of a medical practice is about 80%. Cutting that overhead is the only way to decrease the admin burden on healthcare.

Did this anxiety about going to work start after your run-in with the California board of medicine or before?
I never had it when I first started working. When I started feeling burnt out I got some of it and after the medical board investigation it was quite a lot worse.
I am a physician, interested in doing some health coaching. A few questions.... 1) Would keeping track of a patient's measurements (eg. waist circumference) be considered part of the "objective" and thus considered to be practicing medicine? It's such a blurry, fine line! 2) How about not disclosing to the client that I am also a practicing physician to avoid the confusion? 3) What are the circumstances that would lead to the State Board coming after a physician who does health coaching?
1. Depends on how you are keeping track. My rebuttal would be why should you keep track of the coaching clients information? 2. You have the right to keep your identify hidden in any way shape or form you like. 3. Too many to list here but if you get on the board's radar they may choose to investigate. If they don't have anything to hang on you then you're golden. Remember that the state medical board is not on your side, their job is to discipline physicians. And they would be generally quite perturbed of a physician doesn't fall under their jurisdiction.

Oh that's so cool. I really like that. Starting is the funnest part because you get to confront your fears and you recognize that you only have to please the person in the mirror. And that's often your toughest critic, seemingly. But, really, they are your best ally and love you the most. I have to say that I have zero aspirations to be great but I do want to be of value to at least one other person in my community. You, for example, are my community. And if something I said was of value to you then you're likely to spread it and help someone else. The R0 of being of value to others. But to be great, to "succeed", thinking back of all the greats of the world, I consider few to have had as big of an impact as the ex addict who went sober and just started supporting people around him.

It's a good question. I don't have the answer to that. I suspect it would depend on what the definition of practicing medicine is in that european country. As a physician my family and friends ask me medical questions all of the time so if I give out medical advice to friends and family on the phone am I practicing medicine?

We don't realize that by using health insurance for day to day ailments and not just emergency cases we drive up healthcare costs. We could do away with expensive nonsensical health insurance which charges different patients different copays and deductibles for the same drug/trx if there was a universal health care model in place. We could offer more streamlined affordable healthcare if providers weren't constantly pushed to bill health insurances to maintain a profit. If only Congress was made up of physicians and other healthcare providers...Instead it's lawyers who dictate how and when an OBGYN should do their job. Like your background btw.
Great points. I don't know which direction healthcare will go but whether we have universal healthcare or pure fee for service I don't see access to clinical outcomes data happening anytime soon. I also don't see any chance in public health policy in the near future. All I can do is run a system in which I feel a little more comfortable.

Virtual office and home visits
This is such a great video along with the content on your website. Last summer, I returned from a 7 week solo trip in Europe (4 of those weeks in Spain) determined to get a job in telemedicine so I can live in between CA and Spain. It is definitely not as easy as I thought it would be, especially as a PA but I am determined and know it will be worth it. I don't have any such list. It would depend on the company if they will let you reside abroad. A little more likely with peds but I am not even familiar with any peds telehealth companies.

How do you handle prescription services for the expatriate community outside the USA
Usually not an issue outside of the USA since medications are so cheap. I either work with a local physician locally to get someone their prescription or I've been able to have many foreign pharmacies honor my prescription though the patient has to take it there and they have to make an exception. If someone is living abroad long-term it's simple - they just find their own doctor there to prescribe things while I focus on all the other health related issues.

Us healthcare providers who left corporate medicine are the ones who are truly needed to revolutionize healthcare. Our intention is pure! We shouldn’t need a covert operation to deliver ethical, valuable and personal healthcare. That’s why we became healthcare professionals. We like helping. More people are looking for us than we think. We need better business models, profes support and professional advise. We shouldn’t struggle to do a better job than the puppy mill insurance industry business model. I appreciate that your content and information helps responsibly pave the way to great medicine! We also need to acknowledge that patients are looking for a more functional and holistic approach to their health. People actually want to be well! Truly well! They are tired of being told everything is WNL when both the patient and the provider know that there clearly is some sort of imbalance going on. Pharmacuticals, surgery and life style modifications are not always just the only answer. We need to listen and be willing to expand our medical mindset into a safe and responsible explanation for symptoms when labs and meds cannot explain. So we need to expand laboratory panels, our optimal ranges and think of the same answers we want for our own health is what patients are looking for theirs. I am always floored that us providers don’t want more for our own health and are then offended when our patients want more out of their health. Read about optimal levels. Look at what true health looks like and don’t be afraid to prescribe supplements and probiotics. Our patients are teaching us how to look at health so we can all age better. Optimal health is at our fingertips. We need to just open the doors to treating true health. Thank you Mo for your channel. Yes we need a voice to be more than health coaches. We need freedom to be medical healthcare professionals without being a slave to the system. Apologies if this is too much. But I believe in the cause and so do the people who are looking for us.
Not only is this incredibly well-written but it sounds like it comes from such a genuine place. I try not to demonize corporate medicine too much in my articles and videos because it leaves a little bit of negativity in me that's hard to shake later but I fully agree with your sentiment. We have lab levels which are created in order to set a minimum standard of billing and reimbursement but imagine if each American only set enough aside according to IRA and SS limits - we'd be a country of very poor retirees. I would love to see more voices like this represented on YT channels and online articles. Thank you for sharing.
@@DigitalNomadPhysicians I’m a terrible UA-cam content maker lol. I have spent many months, weeks and hours recording and deleting lol. Nothing has been published 🙄 I truly appreciate your channel and movement. I did leave corporate medicine and found a medicine that makes sense to me mixing primary care, functional medicine and regenerative medicine. Unfortunately, I haven’t figured out the sustainability, profitability lol. But I’ve learned too much to go back now! Terrible when your trusted healthcare provider also qualifies for Medicare insurance but doesn’t take it because I have great purpose to succeed. Independents comes with a cost but the view is spectacular! We can get there. Medical professionals need to support each other and all the different ways we have acknowledged in healing ourselves and patients. Medicine is always evolving. We got this! We are not alone and we all have valuable insight. Apologies. I’ll get off the soap box. But thanks for supporting ideas so many providers have but are uncertain about how to say it out loud.
