Not just Italy but its common in many European countries! Usually at some busy market areas there are no public toilets and people have no other option but to use toilets in such fast food joints. So in order to avoid over crowding, they use such a practice! I had some trouble while travelling with my baby but the good was that they let me use it whereever we visited! :)
