The final mission opens with a tense congressional hearing where Congressman Huston questions General Herschel Shepherd about his involvement with Farah Karim and the Urzikstan Liberation Force (ULF). Shepherd defends the ULF, stating they have always been American allies. However, Huston presses him further about illegal missile shipments to the ULF. Shockingly, Shepherd admits to personally commissioning the illegal missile transfers, claiming it was to save lives, which stuns the room. The focus then shifts to the events of October 2022 in Las Almas, where Shepherd is accused of ordering Phillip Graves and his Shadow Company to fire on Task Force 141. Shepherd denies the order, leaving Kate Laswell bewildered, as they had agreed Shepherd would tell Congress everything. Graves is then questioned and states that he did receive such an order from Shepherd but chose not to act on it, further escalating the tension.
While the hearing unfolds, Task Force 141 and Farah prepare for their next mission: to stop Makarov. Laswell receives intelligence from MI6 that Makarov and the Konni group are in London, attempting to meet with a hacker. Ghost monitors the situation via CCTV while Gaz and Soap track down the hacker, ultimately discovering a Trojan horse virus aimed at taking control of London’s train network. As Makarov is spotted boarding a train to the Channel Tunnel, Task Force 141 and the Special Forces Operation (SFO) rush to intercept him before he can destroy the tunnel.
Splitting into teams, Gaz, Ghost, and the SFO leader take one side, while Price and Soap take the other. After intense firefights and attempts to stop Makarov, Price and Soap find themselves at a bomb site guarded by Konni forces. As Soap works to disarm the bomb, they are ambushed by Makarov. He shoots both Price and Soap, incapacitating them. In a brutal fight, Makarov kills Soap with a shot to the head, leaving Price devastated.
Gaz and Ghost arrive with reinforcements, forcing Makarov to retreat. They manage to disarm the bomb, but the loss of Soap weighs heavily on the team. Laswell files a report noting Makarov’s injury and unknown whereabouts. She also confirms John “Soap” MacTavish as K.I.A.
Later, Price, Gaz, and Ghost travel to the Scottish Highlands to scatter Soap’s ashes. In a somber moment, they reflect on their fallen comrade and friend. Ghost releases the ashes into the wind as they mourn their loss.
In the epilogue, General Shepherd returns to his office late at night. When he turns on the lights, he is confronted by Price, who reveals that Kate Laswell helped him gain access. As Shepherd reaches for his gun, Price pulls his own and, after a tense standoff, shoots Shepherd in the head, ending their long-standing feud.
