When I gave birth to my son 18 years ago, I sustained pudendal nerve damage. I developed pelvic organ prolapse, incontinence, loss of sexual functioning, and had chronic SI joint pain.

    It’s interesting. My potassium level is usually border line towards the high. I’m wondering if that’s why I get up to urinate during the night.

      Interesting question. It has been suggested that people with interstitial cystitis/ painful bladder syndrome may experience bladder pain because of breakdown in the mucous barrier on the inside of the bladder. It is thought that for these people high concentrations of potassium in the urine irritates the bladder lining. Having said that, the most likely cause for frequent night time urination is overactive bladder not interstitial cystitis. Furthermore, the potassium concentration has more to do with hydration status then blood potassium levels.

    Hello doctor I tried kegal it was not for me but reverse Kegel is for me! So I will do reverse Kegel for month or so until I see big improvements and my detrusor muscle get relaxed and then I will do bladder retraining wish me luck

    Thank you Dr. Im 63 & had surgery in my 30s. Nothing was explained. My vagina hung out so low but I never realized they reinserted it...I assumed they cut it off. You are right, it did prolapse again soon & I was pressured to have another surgery but I declined . I worried that the risks may out weigh the problem ( really only asthetics).

    Thank you so much Dr Bruce, but, I’m way past this already. I am going to have a vaginal hysterectomy in February 2025. I’ve not had any issues with my prolapse, only when I’ve had to have pessaries fitted has it been uncomfortable for me. So I’m going to be so glad to have this surgery with a surgeon in whom I have great confidence too

    OB drugs prices are criminal. I have to stop merbeyriq as it costs me half of my SS. Is there an affordable alternative without horrible side effects?

      Myrbetriq (myrbegron) is generic now so should be less expensive. Drugs like oxybutynin or tolterodine should be inexpensive but have more side effects (dry mouth and constipation). Pelvic floor conditioning with the PfilAtes method works for 80% of women that try it. Check out pfilates.com

    I didn’t need an operation . .l have had a pessary fitted over the last 4 yrs and have it changed every . 6 month and had no problem with it you can still have sex

    I had this condition. Had an extensive surgery to re attach it to I guess what was my pelvic wall. It was a horribly painful recovery. Of course I was given pain killers to help ,but the caused constipation . Surgeon explains not to push or exert pressure in the area. Having had five various surgeries this was the worst EVER.

    You are such a good Dr/teacher. This is such important advice! I’m 73 now. I had a Hysterectomy at age 31. At age 65 I started going to a Urologist for my bladder issues. Mostly I felt my bladder had dropped and was looking for the bladder mesh, which was a no go as they quit doing that surgery. I had severe pulling when my bladder was full, not tolerable after a while. I was sent to GYN next and tried a pessary for a while and then ended up with the pelvic lift surgery Roboticly. The Gynecologist and Urologist did the surgery back to back and I did get a small urethral mesh put in too. I did pelvic floor PT “after” all this, not before. Next up, my bladder fell down into the vaginal canal. So I’m back to wearing a pessary to hold up my fallen bladder. I do pelvic floor exercises everyday now. I also walk daily, swim three times a week and also change my pessary myself and go every 6 months to see GYN for a pessary check. Had I known about pelvic floor exercises way earlier when I had my hysterectomy maybe a pelvic lift could’ve been avoided. I’m ok, and I do fine with the circumstances. P.S. your pelvic model is perfect. 👌🏻 ❤

    Will getting into knees chest, position, with buttocks in the air, for a few minutes each day, along with exercises, help prolapse. When on the toilet or standing, walking, etc, it feels like my womb is falling out? I have problems with anesthesia, so NO surgery. I'm almost 69 years old.

      Hi. An “inverted position” can relieve symptoms briefly. Pelvic floor exercise can , over time, produce a more lasting effect. (www.pfilates.com). Perhaps talk to your doctor about a pessary device.

      Symptoms can be improved with regular pelvic floor exercise. A pessary device is helpful for some patients. Surgery is usually successful.

    Best description I’ve seen. I wish woman were taught about this very early. I’m tired of it. I’m 67 had 2 kids. Took care of me disabled husband till I pulled my guts out. Not complain id be there for him again. Il Be very careful after surgery now that I know it can happen again. I do love to push mow and garden. Guess those days are gone. 3 more months Il be in recovery mode hopefully my sister has it 4 years ago and is a beautician on her feet all the time. She still doing ok. Hoping I’m the same. I wish I had taken better care of my pelvic floor. Thanks doctor, for great info 👍

    The demonstration with the sock was really helpful and made it very clear what a prolapse is. Is diet really beneficial to prolapse as well as exercise?

    I would love to have some practices I can do for strengthening those muscles and ligaments. I have some control over those internal ring muscles but vagina wall pops out after longer periods of walking or standing ( I’m a massage therapist and learning Bowen ) thank you

    How terrifying. Im really glad that I won't have to deal with this. I dont have anyone to wipe my butt (or kick me into my open grave) when I get old. 😂 Im personally not concerned as people don't seem to die of old age anymore. This really sucks for people who have kids and some dreams about getting old or life plans beyond the age of 48. One of the good things about the interwebs is that people (I mean women specifically) are wise-ing up about what they are walking into instead of being kept in the dark and are as a result making different decisions. It's a bit like having children now comes with cigarette warning labels that read "may cause your teeth to fall out", or "may cause your genital tract to (develop sentience) and attempt escape from your body cavity". I don't like people but it's icky to let them suffer, cause it, herd them into it, or let them walk into it.

    I had a prolapse of the vagina, bladder and rectum after my second natural childbirth. And I had been doing kegel exercises before, during and after childbirth. I was told by my gynecologist ( not the same person who assisted me with childbirth) that corrective surgery would solve the problem. After the surgery I thought the problem was solved only to experience 6 months later that my uterus exerted so much pressure on my bladder that it caused incontinence to develop. I went back to the doc who did the surgery and he said the next step was a hysterectomy. As I suffered from endometriosis and painful menstruation, I agreed to have the. hysterectomy. I was 34 years old. 10 years later, I developed incontinence again in spite of continuing kegel exercises. Then I was prescribed hormone replacement therapy alleviating most of the symptoms. Eventually I had to go for surgery again to insertion of a sling under my bladder. If I knew all of this was going to happen, I would have selected caesarean childbirths.

      I am always surprised these risks are not explained to women before pregnancy. Elective cesarian section is now an option in many places.

      My aunt got the same problem of incontinence while she's much older than you..she's in early sixties..we then got an Indian dr from India who came and did some physio therapy plus electric shots on her pelvis..etc..she's now happy again and controlled urine..Alhamdulillah

      @@thepelvicfloorsolution I am going to venture a guess that women who have this issue gain more than 25-30 lbs. during their pregnancy and some do not eat that well . There is the option of an abdomen belt to keep the weight of the baby off the pelvic floor somewhat. Having a c-section is not the answer unless it is necessary for some other reason. Babies get many benefits from going down the birth canal that helps their immune systems. Also if you plan on nursing you are going to have the issue of drugs in your milk. Having a c-section scar is not something many womwn want unless it is totally necessary.

    I am 72 and have a bulging prolapse. I really don't want surgery but now I have a better idea of what is going on with me. The first thing to do is get myself on some HRT, go to a specialist , and do some pelvic floor exercises before even considering something radical like surgery.     I have two lower back collapsed vertabrae due to osteo.peroses and cannot do the excercises I used to do. I received a ,,ring,, from our day hospital and it works, well for me? I do not hear anyone else mentioning this? They do not even consider me for an.operation probably because I am too old and to them it is a waste of money. I am 77. Have heard similar comments from.older people.

      The pessary “ring” can be an excellent option. Candidacy for surgery usually has more to do with health than age.

      Restoring the normal attachments requires surgery. The good news is symptoms of mild and moderate prolapse can be relieved with pelvic floor exercise such as PfilAtes

      It is true that loss of estrogen changes the urethra, bladder, and vaginal bacteria that seems to predispose some women to UTI
