I Have 90% ineffective Swallows and diagnosed me with IEM and I couldn’t get Nissen fundoplication because I wouldn’t be able to get food down. What other surgery can I get? I have dysphaiga symptoms with this and doc said I might have a variant of achalasia?

I realised this connection with my chronic pain and histamine etc however doing low histamine foods, along with fodmap etc was impossible. I am very underweight and struggle to gain or maintain weight hence it is very difficult to have food restrictions. Would taking anti histamines help?? Also if very sensitive to medication and not a fanof meds or big pharama over issues due to past medications trialled i am not keen on re trying anti represents again. Seem to make matters worse and side effects are not good. If someone was to try anti depressents again in desperation which type would you suggest? TCAs are a no go as make my heart issues worse so would SSRI or another type. Thank-you

Mine is caused by chest & abdomen area injury. Injury to my organs. Causing spasm and contrastion and severe aerophagia. Etc. Etc. Caused by a chiropractor misdiagnosing me. She must have thought i had hatial hernia ( which i didn't) she used aggressive technique/ applied heavy force to move my organs down ( more details) causing major injury/ major complications/ edema etc. Etc.

This a such a great video and theres alot of detail there. I would just like to see more research being done on patients thats past this stage and actually living with. Possible treatments and the role Magnesium can play. Theres not alot of data out there that can assist patients living with this disorder

The first presentation is based on outdated and simply wrongly done research. Look at MAST reseach group’s research at Cedars Sinai where they found that more than half of the overgrowth is in the mucosal barrier and required mucolytics to properly culture. It simply wasn’t done right in other studies, where they didn’t use proper agar, mucolytics, and often contaminated the catheder with oral microbiota. The cause is very often anti-vinculin antibodies following food poisoning, leading to vinculin immune attack in the interstitial cells of Cajal, leading to migrating motor complex dysfunction, which leads to SIBO. Shotgun sequencing revealed commensal, colonic E. coli strains, and Klebsiella pneumoniae and Klebsiella aerogenes as the major causative microbiome disruptors. They were also the first to directly correlate the presence of these microbiota with breath testing results, as well as symptoms, and came to the conclusion that SIBO is a cause for about 70% of IBS-D, and that 90min cutoff for lactulose breath testing is about 85% specific with a 20ppm from baseline increase. SIBO is a MAJOR cause of IBS!!!
But i used these vonoprazan 20mg for gastritis didn't feel good.....????

Since centuries the optimal manner to overcome not only occasional constipation but chronic constipation too will be taking castor oil. Remember that castor oil is the only laxative that will not work in the intestines by irritating them, but by activating and synthesizing certain substances in the human body. So any habituation of the human body or diminishing laxative effect is absolutely impossible and castor oil will be suitable in a special manner for Pelvic Floor Dysfunction. Take 1 tablespoon (= 15 ml) castor oil early in the morning on an empty stomach and 1 to 2 hours later you will remark its relieving effects.

I was diagnosed with Ooesophagea Aachalasia a few weeks ago. Since I am at the early stage, I have been looking for alternative treatment other than the traditional symptomatice treatment. I did not want to have Botox or POEM. I am very thankful to Dr. Jain for this video. It helps me to understand much more about this disease. Perhaps if I can be seen by a neurogastroenterologist, I may try cognitive behavioral therapy and diaphragmatice breathing instead of surgery.

What is the best pediatric motility and oralangyal Dysphasia for infant who suffered 50 dayd in NICU full term due to birth injury c section +hie, destats braydcardias, in utero small brain hemorrhage, anemic and level 5/5 high risk for aspiration needing thicker formula - bwh said it was all because gerd,? Dysphasia? Silent aspirations and intolerance still not managed 2 month old almost three months. Please advise

what kind of medication is it? they have me taking nitroglycerin 3 times a day and each time gives me a 5-10 minutes window to eat and drink water. it scares me though and am interested if another drug works for you. i am not sure that i am a surgical candidate. was just dx and haven’t even met with a doctor yet. ☹️
Can refflux cause iem issues my test showed 50% in trouble swallowing and reflux symptoms. The les relaxation is normal i really think my spitchner is not working properly so i really want to get the fundoplication surgery

How can an high fat diet be accused of causing neuronal damage and inflammation in human? That doesn't make sense, the study are done on rats, they have a really different diet than human, they are not designed to be high fat like human are. And how can high fat be accused of causing all these diseases if the majority of the people in the western world aren't on high fats diet but on high carb diet?.

I'm on the same path, I have several food allergies and problems with my esophagus. Immobility of my digestive system.
Patients do not understand FD; in addition, there is a perception among many patients (developed due to physician attitude in many cases) that FD is not taken quite as seriously as GP (and GP is already not taken seriously enough in the view of many patients). So, when the diagnosis is changed, the patient often sees that as an indication that the GI believes there has been some sort of improvement. This is problematic when symptom severity has not changed. A change in diagnosis (especially when back and forth several times) is confusing and upsetting to patients, as they already feel they are not on solid footing. It can also cause issues with insurance coverage for certain medications and procedures when the diagnosis is changed from GP to FD. Just asking that you consider this perspective.
This was an excellent presentation -- one of the best summaries of clinical trial issues I have seen. You clearly understand this population.
