Is this available on the nhs in the uk? I'm in agony with fibroids and HMB. The nhs is in such a state the chance of a hysterectomy seems so far away. This could be life changing for women.

This comment is regarding the question asked by another Dr. and not about Dr. Krentel's presentation or answer but I still think it is important to point this out. I think it is very worrysome that a Dr. thinks in term of a patient as single or not to make a judgement about the best treatment. The age is of course important as well as the question if the person wants to conceive or to keep the uterus but to emphasize the aspect of the patient being single hints to a very infantalizing view. It is not the Dr.'s place to judge if the patient will change her opinion on wanting to have kids because of her relationships status. If the person is only looking for relief from the pain and is not interessted in remaining fertile then the treatment should focus on that.

The EEL webinars are so interesting - thank you very much! I am keen on the #ENZIAN terminology and will start using it soon (I am a UK sonologist). But there is an issue with terminology and I have 3 questions. ONE In the IDEA classification, the 'rectovaginal septum' is the area between the rectum and the vagina, below the level of the external cervical os. In my experience, there is almost never any Deep Endometriosis here. In IDEA, the section of vagina above the level of the external cervical os is called the Posterior Vaginal Fornix - an area often involved with DE. In the #ENZIAN classification, compartment A contains the posterior vaginal fornix and the RVS (which stands for RectoVaginal Space - but this is not defined very clearly - what exactly do you mean by this?). TWO - Compartment A includes the retrocervical area ie the torus, where DE is commonly seen, at the insertion of the uterosacral ligaments, as well as more laterally in the USLs proper. So on Ultrasound, retrocervical fibrosis with DE in the ligaments can be difficult to distinguish between compartment A and B...How should we distinguish them? THREE Many gynaecologists still call involvement of the posterior vaginal fornix with DE in the torus with rectal muscularis involvement a 'rectovaginal nodule' - what should they call it now (?ABC)..... best wishes
