from the same example of water spill, you calculate your risk (likelihood and severity) what are the odds that person will step on the water (likelihood/probablity) what can happen if he/she steps on water and slips. Can they get severe injuries or just bruises (consequence/ severity) then you compare it against your risk matrix to get a risk rating for that particular hazardous scenario

One more example Worker can get electric shock by touching exposed contacts in electrical panel containing high voltage. Source: Exposed contacts in an electrical panel containing high voltage Mechanism: Worker inadvertently touching high voltage contacts Outcome: Worker got electric shock Example Description: There is a chance that a worker may inadvertently touch exposed high voltage electrical contacts within the electrical panel which could result in the worker getting electric shock.
