That's your beta mentality rearing its ugly head again. It's called paying your dues. You betas want to work 10h/wk and get six figures. That's the reason why we'll always be on top of you with our boot on your neck. I have 107 employees, and the only ones who fail at life are the ones whining like you. The ones who whine about seeing my IG of taking PJs, about all my cars, and about all the other stuff that comes with my lifestyle. The ones who ask me "how can do better? I also want to drive a rolls royce and have a dozen other cars that are all paid off," are the ones who thrive. You just have to come to terms with being a loser at life, for life.

from EMCrit: contrast-induced nephropathy (CIN) isn't a significant concern is discussed in detail in two parts on the EMCrit website by Dr. Scott Weingart. The key takeaways from these discussions are: 1. **Reevaluation of CIN**: Recent evidence suggests that the risk of contrast-induced nephropathy has been overestimated. In many cases, the supposed link between intravenous contrast and acute kidney injury does not hold up under scrutiny. The episodes argue that clinicians should be less worried about administering contrast, particularly when the diagnostic benefits outweigh potential risks. 2. **Clinical Decision Making**: Dr. Weingart emphasizes the importance of not letting the fear of CIN prevent necessary diagnostic procedures. He suggests that the risks associated with missing a critical diagnosis (such as pulmonary embolism or aortic dissection) are often far greater than the risks of potential contrast-induced nephropathy. 3. **Evidence Against CIN**: The episodes cite multiple studies that failed to show a significant increase in renal failure in patients who received IV contrast compared to those who did not. The argument is that pre-existing kidney injury and other factors often attributed to CIN may actually be due to the patient's underlying condition rather than the contrast itself. For more detailed information and the complete discussion, you can read the full articles and listen to the podcasts on EMCrit's website [here](emcrit.org/emcrit/nephropathy-ct-contrast/) and [here](emcrit.org/emcrit/contrast-reactions/).

If I could give one piece of advice, it would be to listen to your patients when they come into the ER, and follow protocol, even if you personally don't believe something could be happening. My son was diagnosed with kidney disease at age 12. He would relapse often and his particular case was a strange one actually presented in medical publications. When he came to me one afternoon, I knew immediately he was having a stroke-we knew all of the symptoms. He was just 21, but we had dealt with this terrible disease for 9 years already. The nearest ER was just 2 minutes away and it was much quicker for me to take him than to call an ambulance. He walked inside the ER, I screamed he was having a stroke. I begged and pled for assistance. My son was screaming in pain, at one point saying "Mom, find something sharp and stab me-if im bleeding maybe they'll help faster!" This was not a busy ER, the waiting room was not overfilled with waiting patients-we were the only ones. They treated me like I was a hysterical mom who didn't know anything. Long story short, when they finally put a blood pressure cuff on him (which I kept begging them to do, at one point just asking for one myself and was denied) his blood pressure was 281/196. I held my son in my arms and watched my child stroke out. The fear and terror in his eyes as he held my hands like an anchor, will never leave my mind. The hospital ignored their own stroke protocol, because they didn't think a 21 year old would have a stroke. They ignored his medical records which they had full access to on their system. They ignored me as his caregiver, because I was being loud and insistent. You're going to run into all kinds of patients. But overdoing tests in the place of deciding you know what's best, could kill someone's child, or mother or father, etc. Ego can destroy families, and it's SO easy to become complacent, so easy to forget that you are being entrusted with actual human beings, not just numbers! As you start this journey of patient care, work hard every single day not to lose your sense of humanity. Not every case is life and death, but it absolutely could be. You may see a hundred patients in a day, but you are likely the only doctor that person is seeing. You hold the power of human life in your hands, you make decisions decisions to heed or ignore the pleas of a patient, or their Mother, you can save or destroy a family. Don't lose that knowledge as you become desensitized in your job ❤️‍🩹

You remind me of the Nurse who was on the Neuro ICU floor and my son's one on one. My son Jordan suffered a catastrophic stroke due to his kidney disease 2 months after he turned 21 in October of 2022. The stroke took out his entire brain stem, he was gone. We kept him on life support for 3 days so that he could be an organ donor, which from the time of 12 he wanted to be. He somehow knew that he wouldn't live a normal long life, he told me he wouldn't live to see 25, and he was right. But because of this, we had the really uncomfortable conversations about what he wanted. So we were able to know what he wanted. It's so important to have the uncomfortable and sad conversation with your family members so that you CAN be their mouthpiece in the worst of times..it has almost been 20 months, and my heart is still broken. But his caregivers that we had in the hospital he was Life Flighted to for those final 3 days, will hold a special place in my heart for as long as I live.

OMG, my husband and I literally just had this conversation yesterday! Dementia and Alzheimer's run in my family. Every single person has gotten it, currently I have an Aunt, Uncle and my Mom starting their journey all at the same time. I myself at 47 have already noticed my memory failing. My husband and I have been married for 30 years. He watched my Grandma's demise. He was warned and we joke all the time about what he's going to deal with. Yesterday I got sad and I said "I am realizing there's going to be a point where I don't know you or the kids anymore and I am so sad about that!!" And he said "Sweetie, YOU won't know that you don't remember." But it is hurting me now, knowing that that time will come. I am a writer and for the last few years he has been asking me to write our story, so that just like in The Notebook, he can read to me all about our life and our story. He and my kids say they will never put me in a care center (2 of my daughters work in memory care, and one in an ICU with brain injury), so I know that I'll be cared for and loved. But it hit me yesterday, that a time will come when I don't remember my loved ones 😭

In 5 years ive been known in the psykiatry system (I live in Denmark 🇩🇰 so I believe it is a bit different) and Ive got medicine all theese years and social workers knocking on by door all the time, which has made me feel worse because of overwatching. By keeping me in the belief I am mentally sick makes it difficult to change it, because it gets to feel like a unavoidable part of me. Too much 'help' is making everything worse and its a waste of money from the government anyway.
