    Thank you so much for the very precise explanation. I just realised that I have few conditions at the same. I wish my doctor will give me referral for MRI. I got results from ultrasound regarding bursitis, but I also have horrific pain in my knee, & front of my front of my tight, could be sciatica, piriformis muscle, lower back stenosis. I have the horrific pains from the beginning of This April. Did some walk today & came back and had to take 2 Panadols. I am very concerned about the steroid injection as after the Astra Zeneca I thought I will not survive. The worst pains come night pain, , hip, knee, and upper right like really agonising pain. Probably I will have to go for the injection. And pain in groin. The X-ray did not show arthritis. I was always very active & walking a lot. I live in Australia, hope the doctor will help

    Any experience or suggestions if ossification within hip labrum is found on pelvis mri..?

    • @JosephGonduskyMD
      @JosephGonduskyMD 8 днів тому

      Ossification of the labrum can be common and is not necessarily a problem if pain is not associated with it. For pain, especially in the groin, it can signify a symptomatic labral tear or early arthritis. Other sources of groin pain can be from hip flexor strains or inflammation, and many other etiologies. I would overall see an orthopedic surgeon who specializes in hips if you have pain associated with the pathology on MRI.

    I had my knee replacement July 11th anout a month ago. I went to a rehab facility July 15th. On Monday Aug 12th I had severe pain swelling and my range of motion was decreasing instead if increaseing. On Monday my knee was popping all day and some was bulging biut . All the pt and nurses said oh that's just weak quads or aoart of the healing process the pain was severe. Even using the cpm machine wasn't working my knee would bend. I messaged my irtho team. My 2nd oost otlpt was that Thursday Dr dud X-rays he said you're right something bis wrong there is something loose on xray. So on Thursday he said he has to see what happened and fux it.

      He said it shouldn't take more than 30,min but I literally thought I was going nuts because a few days before Monday I kept saying my knee was extremely tight and swollen. I cried every day. I'm glad I'm nuts because now everything in my knee keeps moving and it wasn't doing that on the first pre opt the X-rays were fine. I just want relief this pain is unbearable he stopped to all PT and I wear a brace until surgery this week.

      That does not sound like a good scenario. Big concerns would be infection and/or component loosening. You definitely need an orthopedic team to work that up and make a plan for you.

    Does hip osteoarthritis cause lower back muscles to tighten up (or knot up) especially while walking? Thank you.

    • @JosephGonduskyMD
      @JosephGonduskyMD 8 днів тому

      The back and hip are interconnected. Hip arthritis can cause back pain to be worse. But, isolated back pain is common as well. Overall, back and hip issues are so common that it is very frequent with patients to present with both. It’s our job to differentiate these things. I often see patients who think they have a hip problem that is certainly related to the low back, and vice versa. And often both of these things are occurring. But yes, if you do have hip arthritis, the gait abnormality that can occur due to this can cause some low back discomfort fairly frequently.

      My recent xray shows calcification in the hip joint that is causing friction that is really painful. My orthopedic appt is not until November 2024. Is it common to have the calcification removed for relief? I'm afraid the orthopedic will start with treatment that is too conservative, such as cortisone shots and NSAIDs which haven't worked on me ever over the past year.

      Great to hear and thank you. Total knee replacement is the most common major surgery in America. It is excellent, but not perfect. And, it has gotten better and better over the years with improved technology and less invasive surgical techniques. Patients are often scared to have knee replacement, which is entirely reasonable. I would be too! But, it really helps everyone to hear that the vast majority of knee replacement patients do well and are glad they sought a fix for their knee pain. Thank you for sharing.

    scolioses can be corrected ….

    • @JosephGonduskyMD
      @JosephGonduskyMD 8 днів тому

      True! As a hip replacement surgeon I generally think of scoliosis as either younger, adolescent idiopathic scoliosis or older, degenerative scoliosis. Most patients I see have degenerative scoliosis. Almost all people over 60 have some form of back degenerative disease, whether symptomatic or not. And, degenerative scoliosis is very common. It is the most common reason why older people lose height. The back goes from a straight line to a curved “S” and you may go from 5’6” to 5’4”. True scoliosis deformities can only be corrected with surgery. Which, as you can imagine, is a fairly massive surgical procedure which does not make sense for most people.

    • @JosephGonduskyMD
      @JosephGonduskyMD 8 днів тому

      Needing a new part is generally never really the answer. Pain after knee replacement can occur for many, many reasons. It’s like a car that is not running properly, you need to bring it to a good diagnostician (mechanic) to work it up. Knee need a good evaluation and x-rays to determine if they are OK or need something done. It’s a complex issue to determine, and I would encourage you to get a few opinions from fellowship trained knee replacement surgeons.

    Your last video was eight years ago. Glad you are back. I am hoping to have total hip replacement next year. I just returned to pouring over surgery info a few days ago. I was born with dislocated hip. A surgeon corrected it when I was 15 months old. My mother was told the reason my femur ended up one inch shorter was because healing slowed the growing of that femur. I have always walked with a limp due to the shorter femur. When I delivered my child in my late 30's, my hip was damaged. Instead of a 1/2 inch lift I needed a 1-1/8 inch lift inside my shoe to even out my legs. I have just have a difficult and exhausting gait. Could you do a video that discusses leg lengthening options that can be included during total hip replacement? I understand you can only go so far before the nerves can no longer stretch. At this point getting back to 1/2 inch leg length discrepancy would greatly improve the quality of my life.

    • @JosephGonduskyMD
      @JosephGonduskyMD 8 днів тому

      Leg lengthening can issue through hip replacement to some extent. Lengthening over a very long distance would generally be considered by an orthopedic trauma surgeon or oncologist, I would seek care from these specialists to consider this.

    Thank you for the information. Over time, the original components of a hip replacement procedure can wear and loosen from bone surfaces. A revision hip replacement involves an operation to replace some or all of the original prosthetic components with new ones. The complexity of your revision surgery depends on the amount of loosening or damage that has occurred to bone surfaces, and the specialized components and bone graft material that may be required to rebuild the hip joint. No one has mentioned my problem: my new hip makes that leg much longer! I had hip surgery, and after 12 years I needed a revision. The plastic between the ball and joint became dislodged, so my surgeon put a larger ball in. I am one month short of 69 years old. I went home the same day of surgery. Hardly any pain. No PT because I am familiar with it since I have had both hips as well as both knees and both shoulders replaced over the years due to wear and tear. I feel fine, only used the walker 3 days. No cane. I just want to know when I will be able to get back to my active life: skiing, cycling, tennis, walking, exercise classes.

      Must have been a typo: I had the revision after 7.5 years. I think I needed the revision because my other hip has no restrictions since I had anterior surgery, but this newer hip has restrictions since I had posterior surgery, and I am an active person who has a difficult time following restrictions.

Unit the next day I could not walk or could not put pressure back on that new hip and the pain was just like before, I went to the doctor yesterday he took an x-ray and said everything was fine that I am overworked the area that I need to let it rest and recover and that the pt person was wrong for pushing me on the PT. Today is day two of icing and not moving at all other than needed and I’ve have no relief it feels like it’s dislocating at the top of the hip it’s when I roll off the tip of my toes I feel pressure at the top of the hip in the front and down my leg and a lot of pain in my right knee. Has anyone else ever experienced this and do you think it’s just swelling or a pinched nerve my surgeon is a little aloof he has a great reputation and has been doing anterior hip replacements for 12 years averaging 4 to 8 a week he also does knee replacement. I’m fearful that I’ve done something that he does not want to disclose to me ? Thanks for your help or advice Howard.

The quicker they get you back to doing better the better it makes them look. It seems to me that your doctor gave correct assessment at the time. But if rest and ice doesn't work it could have loosened a bit which will cause the symptoms you described and the right knee could be from over compensating to ease the pain from the hip. Dr. Gondusky did my knees which was IMHO, the absolute Top of the Line! Knowledgeable, Courteous, with great bedside manner! Technique and Surgery were perfect with lowest pain of all patients in the therapy sessions by far as I observed IMHO! I got my 90 degree range of motion the first therapy session! I can walk now!! Very satisfied!! :)

    Thank you. Getting a revision in 3 days. Hospital stay is overnight and expected to be a straight-forward procedure. Hopefully it will be. Original hip replacement done almost 20 years ago and worn out.

      Hi Marjorie, I was wondering if you had your revision? You are the only person I have see to have the same situation as myself. I am going in for revision on Sept. 3.
