      Fantastic question! For test purposes, it is unlikely that they would both match up, especially because other medical professionals will help handle highly abnormal or severe changes in arterial blood gases. We, however, need to have a basic understanding in order to work alongside those professionals and spot life-threatening issues, therefore the test can ask about arterial blood gas problems.

    could you please describe how the knee and ankle reflexes help to identify the level of disc herniation

      Yes! Since the knee and ankle reflexes represent different nerve root levels, you can use them to identify where the problem is occurring. For example, the Achilles reflex (ankle) is associated with the S1 nerve root (which should make sense because the S1 myotome is ankle plantarflexion). Therefore, if the Achilles/ankle reflex is abnormal, there could be something wrong with the S1 nerve root, and you can trace that root up to the spine. However, much more testing would be needed to confirm if that hypothesis is correct. That's where the entire PT examination comes into play in the clinic.

    From IC to LR it is said that the hip extension is because of concentric hip flexors but it is written concentric hip extensors. I'm confused! Is it a typo or I missed sth? (7:15)

      @ladanrz5272 Thanks for pointing this out! Yes, it is a mistake, because it should be the concentric hip extensors that are working to move the hip into extension. Please let us know if that doesn't clarify the question.

    Are there multiple UE synergy patterns because I have seen the exact same type of question but the synergy is described as scap retracion, shoulder ADduction, and IR, forearm pronation and wrist flexion.

      Great question. There are more than one synergy. For the test, we recommend knowing the UE flexion synergy, the UE extension synergy, the LE flexion synergy, and the LE extension synergy. Be careful to note that abnormal synergy occurs during active motion, which might look different than how a patient presents in a passive position.

    The question saying "progressive" without giving a time frame was a little confusing, as it means something different with GB than with things like MS or ALS. helpful, though, so thanks for the vid.

      Yes - that is a good thing to point out. That is KEY on the test. They use wording that can apply to about three different diagnoses. So you can't use "progressive muscle weakness" or it isn't the best piece of information to use. Instead, look through the rest of the question to figure out what can definitively say "it is THIS answer, not THAT answer".

    Is taking the safest answer every time always the best option? Kyle rice reported a student who repeatedly failed and was missing ~17 questions simply because she took the safest answer choice each time but was not the best answer for that question. So I think it’s something to keep it mind, but one should never forget that it depends and sometimes the answer may be safe the the question has nothing to do with safety

      It's true that most of the readings will say that both are shin splints. And in fact, they are each a "type" of shin splint. Most of the time when someone talks about "shin splints", they are referring to the anterior periostitis version. MTSS is quite similar, but the biggest difference (especially for something like the test) would be the anatomical location of pain and pathology.

      "Shin splints" is a poor term, because it is a lay term. So medically, it isn't the best for us to use that term.

    For IC to LR isnt the moment a Hip Flexion moment using ECCentric Hip Extensors and then LR to MSt it's going from a Hip Flexion moment to a Hip Extension moment and using ECCentric Hip Extensors with the help of the Passive Y-Ligament? @8:43, just wanted to clarify!

      Thanks for the question! We leave out "moments" due to the confusing nature, though do recognize that gait is a MUCH more in-depth topic than we state here. This video is mainly for purposes of simplifying and studying for the examination, therefore the basics to get out of the video are the muscles that are working (yes, hip eccentric extensors to slow down hip extension from TS to IC, probably transitioning from hip eccentric extensors to hip concentric flexors around IC to LS through MS, though the exact moment this changes over is beyond the scope of the video). You also bring up a great point with the ligaments, which absolutely play a role, however is also beyond the scope of this video.

    Great presentation! The one thing I would say is vital to know about foams is that they cant be used with necrotic tissue. This wasn’t mentioned before the question and you need to know that for the correct answer!

    Ok so, transparent film, hydrogels and hydrocolloids cannot be used if it’s infected? Also what is impregnated silver? that wasn’t even in the discussion. Honestly, that’s basically what the NPTE is! you study the stuff, you graduate, they give you everything it’s gonna be tested on, supposedly, and then you get a bunch of questions with words you’ve never even seen before in your life. 🤦‍♂️ sorry I’m venting…lol 😂😭
