In the novel Itachi was one who killed her using Tsukoyomi. In that tsukoyomi Itachi showed Izumi that they married,have family/children & grandchildren the grew old together till they die. Izumi thanked him for that and he also thanked Izumi for loving him

He took care of her as a friend, but he did not show any love for her, not in anime, not in manga, they did not even really talk, yes, they sat on the spot and talked, but after that their communication could not be because Itachi is a busy Shinobi he has no time for love it is not for him, love which was shown, she was described by Izumi, but Itachi is not obliged to answer them, he gave her in this genjutsu what she wanted, he knew that she loved him but he could not answer these feelings. I do not take the novel as canon because that is where their relationship is revealed, of course not completely, but at least a little novella I have to rely on anime and manga, and that wasn't there, yes Itachi deserves his happiness, but Izumi is not that person, Itachi is not obliged to respond to these feelings

@@H.T.2.0 Do you know Japanese language or culture? In Japan, novel and manga are canon while Anime is not always canon especially if they cut important details or change something that was originally in the novel or manga version if that happens then that is just considered as a filler episode. In the earlier part of the Naruto shippuden there was a mention of Itachi's lover, Izumi might not be his formal girlfriend or fiancee but still was mentioned as the only girl he acknowledge or befriend (no other girl in the clan was shown/mentioned). In the novel that was created for Itachi again aside from his mother Izumi is the only other female he has a soft spot with, he started the massacre with Izumi & her mom using his Tsukoyomi fulfilling their dreams,surely he wouldn't go that far for someone he doesn't have deep feelings not to mentioned in Japanese culture there are lot of ways how men & women shows their love or confess. Japanese men & women are hard to understand including their courtship or confession culture. One moment you would be surprise that a girl can outright confess to you while a boy would use a scenery or Tanaka to confess. The author and the anime studio focuses on Naruto hence why they don't really give any clarification to other characters' story (open plot for them), there was even a discussion in Japanese anime community that the Uchiha clan and Itachi plot overshadowed Naruto hence why the studio decided to edit or cut some info/ideas that was supposed to be for them. Before the making of Boruto there were a lot of ideas from the Novel that was being considered for an anime but the studio is biased for Naruto and his offspring.

Даже Шисуи намеками говорил что у Итачи чувства к Изуми иные и Итачи покраснел и это естественно так как он не знал как реагировать и сам он понимал и знал то что Изуми просто друг «Итачи обрёл совсем немного друзей, и Изуми, несомненно, была одной из них. Другом... Не более.Но под натиском Шисуи, Итачи почувствовал, что его чувства иные. Однако это не обязательно значило, что он испытывал к ней что-то вроде любви.» То есть под таким давлением со стороны Шисуи Итачи мог бы себе что-то выдумать но он и сам понимал что она просто его друг. Итачи выбрал первую жертву Изуми так как знал что та слишком ранима и не смогла бы должно мыслить, она не должна быть сильно травмирована так как она прежде не выполняла сложных миссий. Он просто вырубил ее мать положил беззвучно на пол и с каменным лицом без всяких вопросов просто показал то что Хотела увидеть Изуми, Итачи показал все что она хотела видеть в этом цыкуцеми.
