Posterior Bolton’s Discrepancy? New Analysis To Solve Old Problems 5 MINUTE SUMMARY
Join me for a summary looking at The Posterior Bolton Discrepancy, a new take on the classic Bolton discrepancy. Wayne Bolton’s analysis has been critically appraised and the outcome from Patrick Foley and his team has been the formation of the posterior Bolton analysis, a new perspective on an established tool in orthodontics which seeks to give better insight into the location of tooth size discrepancies. He has also explored through his research the effects of premolar extractions and the likely outcomes of compromised occlusal outcomes, and where we should expect to see it within the posterior segment.
Wayne Bolton established the Bolton’s ratio:
• Mesial distal widths of teeth
• Original study 55 well treated cases
• Anterior - ideal 77.2%
• Overall 91.3% - Anterior tooth size discrepancy maybe masked by a compensatory posterior discrepancy
What is the posterior Bolton’s ratio
• Not included in original study
• Formular sum of mandibular 4s, 5s, 6s,/ maxillary 4s, 5s, 6s x 100 = 105.27% - data from original Bolton’s study
Ratio confirmed by Mongillo 2021
• N=55 patients ideal outcomes
• Digital casts (from plaster)
• Posterior ratio 105.77% +/- 1.99% Vs Bolton’s data of 105.27%
The effect of 4 premolar extractions on the posterior Bolton ratio
Study: Mongillo 2021 (extraction of all 4s) Holton 2023 (extraction of upper 4s, lower 5s)
• Posterior Bolton increases 107% +/- 2.23% (or U4s and L5s 106.52 +/- 2.52%), ideal digital removal of teeth
• Observed Bolton’s was 110.48 % = 3.18% above Bolton’s ideal
• Space of 1.1mm - 1.28mm remains in mandible when ideal arch - only 1 patient did not have space
Clinical options
i. compromise occlusion
1. slightly class 3 molar and class 1 canine
2. class 1 molar and slightly class 2 canine
ii. IPR upper arch
iii. Bonding

47:17 Sanjivan's response to Farooq's inquiry about the impact of mouth breathing on the airway reveals a need for more knowledge and a reliance on personal attacks rather than scientific evidence. Instead of addressing the well-documented effects of mouth breathing on airway development and sleep-disordered breathing, Sanjivan deflected by attempting to discredit the author, framing her intentions rather than engaging with the facts. This ad hominem attack indicates that Sanjivan lacks a substantive defense and is more focused on personal vendettas than contributing constructively to the discussion. His recent publications, which appear to target a specific individual rather than addressing broader scientific issues, further suggest that his critiques are driven by a personal agenda. Expected higher scientific debate for his interview, but it wasn't very pleasant.
Dr. Audrey Yoon has been changing the world. She is super intelligent and warm-hearted; everyone around her loves her. This interview shows how much she cares and loves this field. The only thing about this interview is that the automatically generated subtitles have so many typos and show wrong words, distracting the interview. Maybe you can remove these subtitles? Thank you for the great interview. We are so proud of Dr. Audrey Yoon.
The opinion why Sanjivan doesn't want to do airway procedure such as MARPE and his critic to these articles are totally unreasonable and so bad.... Monkey's facial structure changes when monkey is mouth breathing, but it does not apply for human, because monkey is obligate nasal breather??? First of all, monkey is NOT obligate nasal breather, and monkey and human both are in same category for breathing. Monkey animal model is the best you can do among animal study for respiratory system. It is easy to criticize for those who never performed meaningful real research ( I see most of his publication are critics for others, not original studies, this UCSF Harvold monkey study is one of the landmark studies. All dental and medical procedures has possible risks especially if it is new, but you don't deny the procedure and critic the benefits because of possible rare complications. It is your choice you don't want to adopt it in your practice because you are afraid, but you cannot criticize the pioneering colleague who push the envelope and advance our field.
Dr. Yoon is so impressive. Her resilience, combined with her grace and positivity, truly sets her apart. This new paradigm shift she is advocating for is not only transformative for dentistry only but for overall health in general as well. Now is the era of Healthy Medicine 3.0 and 4.0, not 2.0, it it happening everywhere. I am curious to see how long the naysayers can hold out against this new global trend. Dr. Yoon embodies what it means to be a true leader, continually pushing the envelope and pioneering advancements at the forefront. Her vision and dedication are truly inspiring and it is clear she is shaping the future of this field. Thank you for great interview.

There are criteria for expansion, you mentioned several what about the Schwartz Korkhaus Analaysis, it all depends on your diagnosis, every patient is different I think you are over simplifying it. You are treating the individual.???

Devil’s advocate, IDBs have rare but severe risks, but most dentists are still doing them. Titanium allergy is rare too, but we aren’t testing every patient before inserting them. Most of us will develop serious health problems due to other lifestyle factors too. Risk vs benefit, informed consent.

Can MSE or MARPE be utilised more like a VIVOS DNA or HOMEOBLOCK? So wound up slower rather than rapidly? Would this reduce failure and asymmetry?

Clarifying or more confusing? Sad that so many site research that ortho tx has no effect on tmd. Yet many feel ortho can help tmd. Nice that we can have it both ways...
Can't rule out the placebo effect. Having ortho often improves self-esteem, which has a positive effect in the biopsychosocial model.
