While not an excuse, the normal alarms a pilot would have wouldn't be the same for him. Obviously he screwed up big time, but from a understanding what happened/objective approach, these items should be considered too.

I always thought the whole point of N-number placards on a dash is to remind a pilot what aircraft he/she is flying and to use their callsign whenever they talk to ATC. Maybe when he flies next time he should tape a large A4 sheet of paper to the dash reminding himself what aircraft he's flying eh?

Sure. Pilots even accidentally land on taxiways sometimes. But something seems really off with him that day, beginning with this departure. It's not only the words he says but how he stumbles through them. When you combine everything, something just doesn't feel right. Admittedly, I wasn't sitting next to him, so I don't know.

He just hadn't gotten over the loss of Carrie Fisher, poor guy.
