The different strengths are for people with different weights and are in blister packs of 6 tablets or 12,18 or 24. Kindly go back to your doctor and let him/her determine the dose for you.       Thank you for visiting my channel. We must rule out all the contradictions before attempting lumbar puncture for csf. Then, we can go ahead with the procedure to make the diagnosis and whenever there’s a relapse. Some may choose to repeat lumbar puncture with the condition in remission after ruling out all contradictions and to be sure that the patient is well. In all,it depends on what is obtainable in your jurisdiction,absence of contradictions,patients’ willingness and financial commitment.

I don’t advise anyone on precise manner of medication administration because my channel is opened to all and I have to prevent self medication that could be harmful. Please,discuss this with your physician or pharmacist. Personally,since this medication is not in Category A when it comes to pregnancy, I would rather choose other medication (s) from the safe zone of category A. Someone might be at a tight corner of choosing to save the life of the baby or the mother or both.In remote places without any way to save the situation,you can place calls to the pharmacist,other doctors and discuss with the patient that there’s is no other option. I would rather go for the agents under category A. Follow the guidelines in your jurisdiction,please and remember that self medication is harmful to you and your unborn baby.

    I'm extremely worried about the risk of retinopathy. I'm becoming very reluctant to take this treatment. I'm a triple negative ET patient.

      You're absolutely correct, retinopathy is one of the concerns and you could see that at 8:40-56 of this presentation under warnings. Kindly discuss your concern with your doctor, pharmacist or ophthalmologist. They will be able to take appropriate step after accurate history, thorough examination and some necessary investigations as they deem it fit.     I know this is four years old, but I have to say that I have still not found anything that works for the absolute life disrupting and debilitating symptoms of menopause. I just try to never eat or move or go anywhere. I get dizzy, and feel like I am going to pass out. I can't drive anymore. I live in fear. Now my husband, who is on a medication that causes hot flashes, is also suffering. It's too much. Someone has to come up with a real treatment for this. I can't work like this.

    Doc I'm feeling lightheaded and drowsy, and my urine is colorless. People are telling me that I'm overhydrated, but I can't stop drinking water because I feel thirsty all the time. I'm not sure if this is just a psychological issue or if there's something wrong with my brain.

It's imperative that I respond quickly to your question thus: Medicine is not practised by 'PROXY', therefore I wouldn't be able to categorically tell you that you're fine or not. I can tell you here that you have a condition known as 'POLYDIPSIA'. For me to be precise as per the exact cause, I will need to see you physically, take a full history, have a thorough physical examination, then decide on certain laboratory investigations like Urinalysis ,Random Blood Sugar or Fasting Blood Sugar ,assay for the full electrolytes and be sure that you're not in trouble with Hyponatremia already. Depending on the finding(s) with the history and examinations ,psychogenic conditions could also be ruled out and if your doctor deems it fit, he or she might ask for the imaging of your brain. So, it may be nothing, something simple or a serious condition or psychogenic. Since medicine is not practised by proxy and my desire is to see you healthy ,then, I'll plead with you to walk to your doctor today and let him or her have the entire history and physical examination followed by the appropriate investigations.       @@drtodavica From what I understand, hyponatremia occurs when the brain swells due to an imbalance of too much water and insufficient electrolytes in the body. Is it possible for someone to have low levels of both water and electrolytes simultaneously? Can one experience both hyponatremia and dehydration at the same time?

      As a responsible parent seeking the solution here, I salute your courage. If you can check the presentation again, I have briefly mentioned what we can do as doctors to help your son. However, since it's a daily occurrence and as my own principle, I can't state definitively here what you must do but here are some suggestions: (1) Please, seek medical advice form your family physician who had known the history of your son as a SS patient. (2) Take all measures that one could take in case of secondary enuresis like --Reducing the fluid intake at evening time -Making sure that he urinates as last thing before going to bed -No caffeinated or carbonated drinks at night time -A bedwetting alarm -Alarm to be waking up at a certain time to pee and return to bed -Avoid visitation to places where bedwetting might be embarrassing to you and him -Use adult diaper -Cover the bed with water-proof coverings -Reward his efforts to stay dry at night Certain medications could help, but, not to cure ( we have about 5 to 7 of them) but I don't write prescription for anyone because medicine is not practised by "Proxy" Finally, please, contact a doctor who can take further history, do physical examination and carry out certain tests. That might help rule out other issues and then narrow the thinking to only SS or assist you as it's appropriate if other thing(s) is/are found.

    Well documented and informative narrative on Lacunar Infarcts. My father just suffered from this over the Christmas weekend. Clinical presentations were subtle initially and progressed over the course of several hours. I'm an RN by profession, so I was unfamiliar with the specifics of this type of infarct, rather, the typical presentations of a stroke. It began with very mild, left sided facial drop, subtle pronator drift; which progressed to gait and speech disturbances. I had my father to the ER upon recognizing the facial drooping and drift. The rest followed at the ER. CT and MRI did not show anything on its imaging, and ultimately, the attending initially stated this was age related secondary to white matter nodules, which prompted my frustration as I knew better (my father is a robust and extremely active man). My understanding of white matter illnesses tends to have gradual onset (my mother suffers from white matter illness, and her decline has taken many years). I insisted on an admission, which was granted. The following morning, it was confirmed as a Lacunar Infarct, and treatment was iniated (Plavix, ASA, statins, and various rehab methods). We are on day 4, and I've already seen improvement in my father. My only concern or question is when would a presentation of dementia begin to show if there was thalamus damage. At this time, it doesn't seem to be the impression, but it makes me concerned nonetheless.

It's my prayer that each day brings him closer to the expected or desired relief. He's fortunate to have you. As per your concern/question, the answer is already in this presentation. However, I don’t want you to feel insulted; I'll still repeat the words on "Thalamic Dementia" as you could find it from 8:00-9:00 of the presentation thus: (J) Thalamic dementia Lesions of the thalamus with neuropsychological disturbances like Memory/Speech/Visuospatial dysfunction Develops over hours to a few days No stupor, No coma ,No LOC, No seizures In conclusion, you're aware ( because you are a nurse ) that the fact that it’s been stated as above doesn't make it a RIGID rule that it must occur like that in ALL patients.       I can't practise medicine by 'PROXY',kindly let your doctor know that it's not yet over. What I can do will be the following: ( 1) If you've been given empirically, then ,it's time to have a definitive agent. ( 2) Microscopy Culture and Sensitivity test would be done and switch over to the agent that the organism is most sensitive to. ( 3 ) Continuous use of antibiotics for 10-14 days or longer is possible in some cases but it is not advisable to use one agent and repeat the same agent all within 2 to 4 weeks if not so indicated. Indiscriminate use of antibiotics can tilt the user to the possibility of resistance being built and more side effects.     All antidepressants are neurotoxins. Antidepressants do not cure anything. They all have hundreds of side effects and the side effects can pop up at any time. Some of the side effects can be PERMANENT and last long after the antidepressant is stopped like Tardive Dyskinesia, tinnitus, akathisia and sexual side effects. Antidepressants actually increase the risk of SUICIDE in people of all ages (not just young adults), they are linked to worst long term outcomes compared to those who do not take antidepressants for their anxiety and depression. The chemical imbalance theory is not true but rather a marketing ploy by big pharma. Antidepressants also all need to be slowly tapered and can cause debilitating withdrawal symptoms that can last for years in some cases if someone doesn't taper safely. The issue would be to try to find the root cause of your symptoms. If not caused by trauma then anxiety and depression are thought to be symptoms of something physically going on in the body. It could be nutritional deficiencies or allergies, hormonal imbalances such as Thyroid disease or vitamin D or other vitamin deficiencies, heavy metal or chemical toxins, Lyme disease, genetic defects such as the MTHFR gene variations, histamine intolerance, previous medication toxicity, bad gut health, brain trauma, systemic inflammation and oxidative stress, unbalanced blood sugar levels and more. If you can find a functional or integrative doctor then they can do the testing to find the root cause of your symptoms. Antidepressants only mask symptoms and damage a person's health while they do not realize it. Any part of the brain and body can be affected so many doctors do not tie health problems with the antidepressant. Most Psychiatrists, Doctors and Pharmacists are clueless about tapering off and follow old tapering guidelines by Big Pharma. They also lack informed consent about debilitating withdrawals, health conditions caused by SSRIs. Their research is based on short term, 6-8 week trials. No long term studies. If coming off antidepressants, tapering no more than 2-5% every 2-4+ weeks is not recognized by them and deny withdrawals with their tapering and blame it on reoccurring conditions or something else. You're then a prisoner of psychotropic drugs.
