Can Restless Legs Syndrome (RLS) Be Painful?
Restless legs syndrome (RLS) is a condition defined by a discomfort and an urge to move the legs, but in some cases, can it be painful? @andyberkowskimd of ReLACS Health assesses the nature of pain in cases of RLS.
Among the topics addressed:
- The possibility of painful variant(s) of RLS that may throw off the usual diagnosis
- Differences between non-painful and painful RLS
- How the chemicals of the opioid system play a role in pain in RLS
- The usual treatment approaches to RLS including those with painful RLS
- Avoiding the use of dopamine agonists in RLS and the fact that some cases of augmentation can turn into painful RLS
- How opioid medications may be disproportionally more effective for those with painful RLS compared to first-line treatments.
Why dopamine agonists like pramipexole, ropinirole, and rotigotine should NOT be used in the treatment of restless legs syndrome:

I was diagnosed with type one narcolepsy back in 2015. I can not understand why the hell anyone would want to have it. It sucks ass, it is life changing, thinking about all the things you may want to do in a day and not being able to accomplish between ¾ to ⅔ of those planned tasks even with medication. I wish I didn't have it. I wish that I didn't have to lube with the narcolepsy, cataplexy, sleep paralysis and restless syndrome. Medusa definitely helps, but it is far from perfect.

I have painful RLS since a kid. My mom used to tell me it was growing pains, but I'm only 4'11" and 48 yrs old! I'm on gabapentin enacarbil and it helps, but not always. I also seem to have a caffeine sensitivity. I drink 1 cup of coffee, and hurt for 48 hrs. Energy drinks are torturous. I'm also severely low in iron. Right now haven't slept all night bc I'm in pain. The skin on my legs feel like it's on fire, and it feels like my leg bones are breaking. It's awful. Plus caffeine causes the excessive movements. Luckily I found a good neurologist who diagnosed my pseudotumor cerebri and helps me with the RLS.

Some of us can go straight down for 3-5minutes with very low bp, hr, and electrical activity. Opioids for rls, you should revaluate your treatment, Primum non nocere….. who funded these studies and who’s payroll were they on? Science so called is extremely muddy due to conflicting interests regulating themselves through kickbacks, favors, job promises to the ones that “regulate” their industry. Our life span has been reduced ever since a certain “safe and effective” medical intervention (gene therapy) was rolled out on mass. We now have an epidemic of young people passing of undiagnosed heart issues and rapidly progressing cancers.Buprenorphine also has an affinity for bone marrow, and causes physical dependence. It absolutely slows a patients heart rate and respiration…. Idk there’s got to be a safer option for antsy legs then a synthetic opioid that binds/blocks receptors in the brain and body with a ridiculous half life. How bout looking for the cause of the symptom rls, and addressing the cause rather than masking the symptom with narcotics

Husband dx with narcolepsy repeatedly, has no cataplexy but has definite sleep attacks. It's been a hallmark struggle through his life; so hear it dismissed as "not really existing" is just rude. Acknowledge poorly understood pathology and that it may need to be recategorized -- but dont just dismiss peoples lived realities as not real. Very unhelpful. Ironically i have had cataplexy all my life -- with no narcolepsy dx even after a sleep study.

I noticed you said NOT buprenorphine… my providers seem to have the idea that my primary central sleep apnea (176 out of 179 events were central on the first study, 162 out of 162 on the second) is because of my chronic suboxone use. Even though I’m on a very low dose, 2mg/day. Could you possibly provide any sources showing that buprenorphine does not cause central sleep apnea, or point me in the direction in which to look? Thank you for sharing this information. 😊

I have been on Suboxone for about 10 years now. I have always been OCD about mouth health. So when I had to be on the sublingual film I made sure I took extra care of my teeth I rinsed my mouth out after each time of taking my Suboxone and still my teeth are just crumbling. I have constant mouth infections my tongue underneath is red hot and sore My gums flare up. Pretty much every tooth I have is decayed with cavities. I've already had six teeth removed and I need about six root canals done and three fillings. My mouth is in constant pain. I live off of Advil and Tylenol which is messing up my stomach. I'm only 40 years old. I don't eat sweets but I sure as heck get cavities extremely simple. Suboxone although it saved my life and helped me get clean. Now that I am clean my health is awful. Because when our oral health isn't well nothing is well mind body and soul. I don't know what to do. I want to find a lawyer and I want compensation I want new teeth. I don't want stupid dentures I want teeth that I started taking Suboxone with teeth that were strong white and healthy. Now my teeth are tinged yellow and weak! All I can do is eat soft foods that are filled with carbs because I cannot eat healthy foods My teeth will break. It is like 85,000 to be able to get my teeth back to good. Who on earth has that kind of money to spend on teeth? Dentures are not good for young people. Our jawbone needs to be stimulated and when it isn't The bone on your jaw starts to deplete. By the time I'm 60 years old I don't want to have barely any jaw bone left. I feel disgusting I feel like I look disgusting. This isn't fair! I worked my butt off to get where I am today in order to have my health fall apart because of a medication that has destroyed my mouth health ! It's very disappointing

I had clonazepam prescribed a long time ago for I believe depression, I had some mental issues going on for a while. I took if for a week & had no problems until I was at my child’s softball game & had to be helped walking to the field, later that night my wife took me out to dinner & I was shaking so bad I couldn’t eat with a fork. So she took me to the emergency room & basically they said I was high from a week long build up of taking the drug , so I quit taking it & everything was fine. I’ve had sleep problems all my adult life I’m 66 now. Since I was 25 I’ve lived out my dreams hitting walls with my fist & my wife occasionally. Now I jump out of bed frequently it can be every night for a week or once a week. I was once diagnosed with Ondines Curse & am now on Oxygen 24 hours. I haven’t pushed for this rem disorder as I’m on a bi-pap now, but I guess I should as this jumping out of bed is quite annoying & even dangerous & I am scared about a Parkinson’s syndrome diagnoses in my future.

Is there a risk in using an ASV machine to manage complex sleep apnea, even if the central apnea might not be pathological? I seem to fall into that category, as I definitely have obstructive apnea with some central apnea, but there is no medical reason why I should have central apnea. The ASV machine definitely helps me get better sleep, but is using the ASV in this case a bad idea?

I have an active lawsuit going right now because of sublingual buprenorphine causing me to have all my teeth pulled. Buprenorphine literally caused all my teeth to start breaking apart and falling out. I had a few bad teeth before i started taking Suboxone, but after i started taking 2 a day is when i noticed my teeth literally falling out of my head. I just signed the lawyer agreement yesterday and i couldn't be more happy. I've been living without teeth for over a year now.
