Hi David. I'm curious, how do you know that is the muscle of the client complaint? Have you seen this often in your practice? If so, can you mention what may cause an inflammation or other issue in that deep muscle? I ask because I suspected that is the muscle that has caused me so much pain since July 2024. The PTs say it is overuse and weak "glute muscles". I am improving and about 77% healed. I've been working out regularly (various muscular focused) since 1998 and was shocked that my glute muscles were considered "weak". I have, in the past 3 years moved form a mostly sitting job to a standing one which I was told is contributing. Thanks so much.
Hi Donna, I always hesitate to say that 1 muscle in particular is the "problem" for any issue and usually it's a whole group of things mis-firing or inefficiently balancing (or underbalancing) themselves in order to do any work or activity. I will say Quad Fem tends to get completely locked up in people who sit for long periods and stops being able to change tone, which then causes all the surrounding muscles to overwork. In terms of PTs saying glutes are "weak" that's also a bit vague, but the whole area tends to go "numb" from long periods of sitting requiring quads, hams, and other muscle groups to do more of the work than they are supposed to. When Quad Fem is "locked" (ie mostly scar tissue and unable to fully release and change tone), often doing some basic work on it helps it start moving again which then allows the brain to rebalance all of the musculate towards a more efficient and healthy balance of each other. Takes time and more PT (single leg balance exercises especially with some sort of asymmetrical load and twist can really help move things along) but without first getting Quad Fem to act more like a muscle again (and less like a tendon), all the PT in the world is mostly going to re-inforce inefficient muscle firings. So doing a release like in the video is a start to the process but not the end all be all.....(hope that helps clarify)

Thanks! It does require a bit more patience to be a male therapist although that’s improved greatly since I started 17 years ago. There can be a slight bias towards female therapists in various communities but it’s not that hard to overcome as long as you are trying to connect with clients

This opened my eyes to a contributing cause of a glute overuse injury I developed as the room I've been offering many sessions in is indeed tiny. I wondered why I completed my day feeling so exhausted when I would help out at other spas with rooms 50% larger, I would not feel as exhausted. I know the size limits my stroke/modality options (passive stretches have me banging up against the way) and it requires more of my energy to work within the room's physical boundaries but I did not seriously consider its contrbution to injury. Ugh! Thank you so much for this!
Thanks! And sorry that you, like many in the industry, are having to work in a room that is too small :/ I'm 6'1" so I made sure that all the rooms in both our studios are big enough to have plenty of room for me to stretch out in. Our smallest room is 8.5 x 14 ft but that's huge compared to many spas. Most of our rooms are 9 x 18. Not something easy to fix when you work for someone else unfortunately but if any of you go into your own private practice it's absolutely work spending the extra $ on a larger room. Your clients will also be impressed too :)

These are good tips as I am now, after 3 years of professional practice, am being body aware during my sessions, making adjustments in my positioning, working from my core, using my whole body and not just parts. I rarely use thumbs. Part of my self care includes epsom salts soaks for my arms/hands in a plastic basin on a table or on my lap. I am very surprised that a professional office such as yours allows LMTs to give sessions barefoot (I know some Thai practioners work this way).
Thanks! The epsom salt forearm bath is a fabulous self care practice...highly recommend and will totally do that every so often. If my tub were bigger and I weren't so tall I'd totally do a full tub salt soak once a week too! Personally I don't like to work barefoot for very long as my back acts up if I work a whole day that way. But some therapists ground better when they have full proprioception of the feeling of the feet on the floor so I don't like to force folks to wear shoes. I do recommend having something easy to slip on and off such as Crocs. We do try as a studio to present a "professional but not stuffy" vibe so we dress more on the casual side of things.

I have been doing massage 4-5 per day deep tissue and not one injury ever. Body mechanics and self care including strength training, posture corrective exercises and getting massage. People also have to want to be doing massage and make it a priority so that means putting in actual effort to change things up, learn new skills and do the self care work.

@@KT-sv6jx yes! The brain controls the muscle tone so if you want to rebalance that tone more equally across the musculature we need the brain to re-examine what it thinks “neutral” is :)

Might be a bit hard to do to yourself as the angles are all different but not at all impossible. Try crossing your ankle onto your other knee in a figure 4 and that should make it easier to get at the right muscles on yourself :)

What would a client complaint be requiring this technique? I think most of us do not breathe properly utilizing the diaphragm. I incorporate deep, slow breathing with my client causing movement at the belly and not at the shoulders. It is a wake-up call for many as they re-learn how to breathe.
Great question! Honestly this is work that I do for most client complaints, although it might show up sooner or later in a series depending on how intense the current issue is and how much I need to balance providing immediate relief vs long term support. I like to cue "360" breathing, ie the entire ribcage should expand and contract (yep, without extra lifting in the shoulders and neck) with a slight belly rise at the end of the inhale (most lift belly first instead of ribs/sternum). I think for most clients, the diaphragm gets mis-recruited for postural support which doesn't allow it to relax on the exhale....requiring using the ancillary breathing muscles more (scalenes, serratus, abs, pecs, etc). For the complaints that really need this work: any back pain for sure, as well as shoulder pain, neck pain. Basically anything attached to the ribs :) I like to say "there is no real 'right' way to breathe....but plenty of 'wrong ways'.....usually any that involve too much control over what should be run by your brainstem so that you cerebrum can do stuff like art and science!"
