Is it the same cast applied for 3rd metacarpal fracture?
@@n3a2i yes, you would just include the 3rd digit in the cast. So you would have the 5th, 4th and 3rd digits included in the cast.

What if, the long leg cast without 15/20 degree knee bend ??? Any problems ???
This person is careful with applying the cast in that he was gentle with adjusting the patient's foot position, whereas the hospital cast technician who did my cast started pushing hard the underside of my foot with both hands to force it into an extreme dorsiflex position. My foot was not even in a foot drop position. It caused me pain and he would not stop pushing. Obviously, my hospital's cast technician did not know what he was doing.
I had a cast like this for a fractured scaphoid, for some reason though, I only had it on for about 10 days. Before that I had a slapback cast for 10 days also. Not sure why it wasn’t for the full 6 weeks….i also have NF1 so my bones are softer and prone to breaking more easily. The consultant told me I didn’t need it for the full 6 weeks and never explained why. I’d have preferred it for the 6 weeks as now my wrist hurts every so often and I am hoping that it is healed as another downside of NF1 is that bones take longer to heal. I did tell all this to the consultant but they didn’t know much about NF1 so didn’t seem to be interested in what I was saying. My follow up CT scan also got cancelled because of the pandemic. This all happened in early 2020. I live in the UK/ England and the NHS is not as great as it’s made out to be. This is probably why my treatment was rushed. The guys in this video seem to take care and sound more professional than the doctors we have here too.

Doctor, I have a fracture in the neck of the fifth metatarsal of the hand, the ring finger, known as a boxer’s fracture, and I closed it with a splint for 5 weeks without restoring the bone. Does it have any effect or is it due to its function?
Perfect work. The stockinette you put on the fingers looks much thicker vs the arm. What is the name of that stockinette if you don’t mind me asking?

Some areas of concern. By folding the distal and proximal edging the fiberglass is not tethered to remain in place and will tend to migrate and may irritate patient. Also for actual boxers fracture injury the fracture will be reduced and put into Intrinsics plus positioning . Casting from this position is challenging but needed skill to be a proper Orthopedic Technologist.
is it okay to stretch the cast as much as the doctor is doing? I was taught to just roll the cast without stretching it or it will be too tight when hardens.
It is all about “feel”. You will quickly learn as you are applying casts how much you can stretch it without being too tight. You have to have it fairly snug so it is giving support/stability. It’s great to practice on coworkers or friends initially so that when you are applying a cast on an actual patient you have a pretty good feel for it.

This is dorsal splint. There is many easier techniques for this splint model. With this technique maseration between II and III fingers is certain.
Thanks for the reply. We use a piece of splint material to reinforce the cast so we don’t have to use as much cast material. If you watch @ 3:04 we talk about putting padding between the digits to help with maceration.
@@mcsmedcastspecialists1285 why not to use stockinettes on both fingers? That is much safer.

I need this type of cast following a tfcc wrist repair. What's the level of difficulty when using an ergonomic keyboard in this type of cast? Can you rotate your arm over to the point where fingers and effectively access the keyboard?
I hope your surgery is a success! The hardest part will be not having the ability to rotate your forearm (pronate and supinate). The cast should leave you with plenty of finger motion, but will restrict the wrist/forearm rotation.

How does the final layer the blue layer of tape or roll work? It looks simply like fabric without any plaster material so how does it Harden and become similar looking to plaster soaked blue material?
Each roll comes individually wrapped in an air-tight package. The roll is fiberglass with a compound that hardens once exposed to air. The fiberglass roll has a residue that will make a mess on anything it touched before it actually hardens. It takes less than 5 minutes for the chemical reaction to happen and the roll hardens up.

Can this type of cast help heal my damaged tendon in my wrist?
It depends on which tendon you damaged. This can be used for flexor tendon injuries in the fingers/hand. That would be totally dependent on the physician treating the injury.

@@adelaidebelinsky2124 this cast would not be ideal for an ECU tendon injury. A regular short arm cast or a wrist brace that would allow all of your fingers to be free would be ideal, but that would need to be determined by a physician.
@@mcsmedcastspecialists1285 that is the point he(aka dr. Hamilton) did a cast up past my Elbow and I’m really annoyed with the fact he didn’t even do a proper wrist cast on my wrist
This is not a Muenster cast. Muenster cast is to prevent the rotation of forearm. If the elbow extension goes over 60 degrees you can easily rotate you forearm!
Thank you for the comment. If you reference (14:40) we talk about only cutting out the anterior portion, but ultimately the physician will decide how much elbow motion they want the patient to have. I have found with the wings and a great mold, even giving them “almost” full rom in the elbow, it will still eliminate pronation and supination.

I want to know the broken fingers . ?
A typical Radial Gutter cast (like this one) will include the 2nd (index) and 3rd (middle) digits. This could mean a number of different injuries: Broken metacarpal, phalanx or even a flexor tendon injury. For this video - we could say that the patient has a distal 2nd metacarpal fracture :)
Would you ever put padding between the 4th and 5th digit to prevent rubbing?
Absolutely :) I believe we mentioned it in our commentary, but did not show it. I always put some kind of padding between the digit that will be included in the cast. It will prevent rubbing and the skin from getting macerated. You always want to be mindful of the injury you are treating and use the smallest amount of padding possible. 6inic of soft band will be better and faster
6” soft pad works great. It all comes down to personal preference. Sometimes rolling 6” you can get folds where it becomes loose. Practice with different sizes and use what you are comfortable with. Thanks for the comment and watching :)

Hello, most of our physicians will splint for 1-2 weeks to allow for swelling to go down and then cast for 3-4 weeks.

@@mcsmedcastspecialists1285 I work as an ER tech and we advise the patients not to get the splint wet because the skin will macerate underneath from the moisture. At our hospital we wet only one side which is enough to activate the fiberglass to harden and we put the dry side toward the patient skin that was wrapped in webril If you accidentally forget to close the foil from the fiberglass than the thing will harden without any water just from the moisture in the air.
@@anaborcean2000 awesome, thanks for your expertise! We also educate patients and let them know that they should never get their splints/casts wet. We lightly wet both sides, but ensure we dry it thoroughly as well. You are right though, it doesn’t take much water to activate, so just getting one side wet should be adequate.

What do I do if I accidentally get this wet?
Use a blow dryer. Don’t remove it unless your physician is okay with it. You may want to even consider going back into the office and have them re-wrap you, depending on how wet you got it.

Is it up to the doctor whether the distal IP joint of the thumb is immobilized?
Yes, typically the physicians I worked with would state whether or not to include the IP joint or leave it free.
I really like the double padding method pulling it back by folding twice in the palm very clean and nice. Plus the 2 inch thumb sleeve vs 1 inch. Nice Job!
I can’t move my toes with this cast on is this normal ?? Also it feel like my blood is in one spot still even though I’m letting my leg sit high
What kind of cast tape do you use? Yours lays down and conforms so much better than anything I’ve used. Looks great!
The blue is BSN Delta Lite Plus. It is phenomenal. We usually use Delta Lite Conformable for our base layer because of how well it conforms to body parts. It works really well on ulnar/radial gutter casts too!
Next do a bolster cast please.
