Full time ND and HND have just finished their exams and have been advised to leave the school premises. You should be able to obtain the form currently.

No you can't. You can only combine WAEC and WAEC or NECO and NECO or WAEC and NECO (for some schools). No polytechnic or university would accept NABTEB and WAEC for O'level combination.
