This fixed my board right up! Perfect! Thank you so much. What happens is, when people connect HP up to the side ports while the Board is operating at high output levels in the HP monitor section, it can take out the op amp. Be careful and turn HP volume down when connecting HP's.

That IC is an HC4066, Quad CMOS switch. Sometimes it shorts the internal supply.

I’m having the same problem with no output on any of the outputs. All the inputs work fine and the outputs on the stage box work fine. The power supply voltages all appear to be in spec. Did you have a problem with the AC power fail signal or the power good signal on the one that you fixed? Or was a voltage out of spec ? It’s interesting that Alan Heath has so many problems in this area and yet they do not publish the schematics on the boards, and provide no online help. It is wonderful that at least the international community is stepping up to help our fellow Audio technicians in need of some assistance. Thank you
