We are lower limb vascular clinicians who have posted these demonstration videos to help other clinicians develop their peripheral vascular skills and techniques.
A 3P's brief arterial assessment for a limb of concern.. in under 3 minutes.
A 3Ps vascular assessment (pulses, phases, pressures) can be done in any lower limb clinic or patient consultation, on a limb of concern, in under 3 minutes..
One of the main reasons cited repeatably by lower limb clinicians for not performing ABPI or TBPI, in people presenting with leg or foot wounds, limb oedema or pain that may be vascular, is ‘lack of time’.
This video demonstrates how any clinician who develops pulse palpation, Doppler handling and manual sphyg use skills, can provide a brief, minimum vascular assessment, that can help confirm or exclude peripheral arterial disease or chronic limb threatening ischaemia.
Try it out with your colleagues, if time or lack of toe pressure kit are your main barriers to initial arterial assessment.

Hello. It is only possible to do with a good audible digital artery pulse sound. For people with difficult to hear digital Doppler sounds (most people with suspected PAD) it is easier and more reliable to use a PPG sensor, in my experience.
You were the only video i could find on a TBI using a Doppler and where to place for the big toe. Thank you!
what the heck is brachial? Do you mean arm? If it is arm, why don't you just say it, if not then what the heck is it?
Should the pressure be roughly the same as brachial systolic?
Yes, if the patient is lying down and rested for a few minutes.

Doppler signal is not the same as pulse
Does anyone know? This procedure is NOT covered via Medicare, but I need it done. Best place to have it done on a cash basis?
According to Elsevier- Evolve, that is incorrect. The best way to palpate the popliteal pulse is with the patient prone and the knee flexed. Pulsations of the popliteal pulse can be palpated deep in the popliteal fossa lateral to the midline. The shown content does not allow for deep palparían of the popliteal pulse.
Thanks for your thoughts. We use this technique daily as part of a full lower limb vascular assessment (foot to femoral pulse checks - palpating & Doppler insonation). As originally taught by our vascular surgeons a few years ago. No need to move the patient to a prone position, as we are resting them too at this point, prior to doing a resting ABPI / TBPI. The weight of the relaxed, flexed knee on the fingers, helps to establish if the pulse is easily palpable or not. It's not an easy pulse to palpate, for most clinicians, including GPs & experienced vascular podiatrists, nurses & surgeons. Doppler insonation is a good next step, if unsure. See our other demo video. Would be happy to see an uploaded alternative video suggestion on how best to palpate popliteal pulses, if you want to produce one. The brachial SBP is normally higher than the ankle as I understand it. How does one determine the vascular implications - what difference are we looking for ?

Is this test alone valid enough in detecting peripheral arterial disease or I should use the ABI test?
AM Assaf use ABPI to help clinically diagnose PAD. This is an example of a brief assessment to check pulses & ankle systolic pressures. It can help identify CLI along with history of rest pain and / or non healing ulcer or necrosis.
