I know government medical colleges for my entire career was in those institutions as professor and at times as an administrator. I know a lot of private colleges too. I have talked to teachers in private medical colleges, some of whom were my colleagues who have taken up these posts after retirement and some of the teachers were my former students. I have also visited private colleges for inspection and as an examiner. There are two reasons why government colleges are better than private medical colleges. Quality of a medical college is determined by the quality of the students selected. The good students challenge the teachers to up the standard and the less than top fight students who are bound to be in any institution benefit in that process. The standard in the institution goes up. The quality of the students in government colleges is the best that is possible. In Government colleges in my state (Tamilnadu) in India and most certainly in the rest of the country selection is based on merit. Some concessions are available for students from communities that for millennia have been oppressed in the caste system but even here only the best in that group is selected. Students with merit choose government colleges because the fee is a pittance compared to private colleges where it costs a fortune. The cream join government colleges. Once while inspecting a private medical college I took a groups of students for a clinical demonstration. It did not take me long to realise that the students were not following the discussion. I had to retrace and spell out the details. Two of them who had missed selection into government colleges were up to the level but the rest of them were not suited for the rigours of the medical course. The second reason is about the quality of material available for teaching. Government medical colleges are attached to government hospitals which in India anyway are free hospitals. There is no dearth of clinical material for student demonstration as well as for hand-on experience for internees and postgraduates. Hands-on experience in private colleges is restricted for the patients who are paying patients and demand services from specific doctors. Two examples will illustrate my point about quality of material available for student demonstrations. When I was professor of surgery in the Thanjavur Medical College we were conducting MBBS surgery clinicals for the final years. The Surgical Registrar had to collect long and short cases for the examination that went on for ten days. On the first day of the examination the examiners, the two internal of which I was one and the two externals who came from colleges in and out of our state, had to choose the cases for that day from the group collected by the registrar. A crowd of over fifty patients were in the hall. The externals were astonished at the extravagant number and variety of clinical material. No case was ever repeated on those ten days. Each day we had to use six long and six short cases. My experience of private colleges is different. Once I went as external examiner for a private college in Bangalore. Three short cases were already selected and ready for allotment to the students. As the examination proceeded it was clear that the students were familiar with those cases. On questioning they promptly mentioned the diagnosis of the cases but were on a slippery slope when asked to explain how they arrived at that diagnosis. My marking did not bother my colleague, the internal examiner, but later the head of the department who was dealing with the long cases that day was very unhappy with the performance not of his students but of his co-examiners who did the short cases. The next day I was teamed with the professor for long cases. I had hardly started the questioning when the professor requested me to be lenient. He was open about it. He said that if the results are bad the management will terminate his services. On the third and last day once again, I was on the short cases side. The same three short cases were kept that day. I requested the registrar to get fresh cases. From the way he rolled his eyes I understood he did not have the authority to accede to my request. He told me that the day I was doing the long cases the same set of short cases was once again on display. That college of course is not the model for all private medical colleges in India, but neither is it an exception. Many are not very different. Private colleges like the non-profit CMC Vellore and a few others are as good as the best but they are only a handful from among the hundreds of private medical colleges. Most of those who join private medical colleges qualify sooner rather than later. They have paid a hefty fee for the course. I believe it in the region of 10 million (1 crore) rupees. They must recoup it from their future patients. Government colleges qualified doctors have paid a pittance. The government has subsidised their education in the hope that they will care for their fellow citizens by charging them at affordable rates. Two sets of colleges working to cross purposes. One trying to make it easy for the sick and the other making it expensive. Lately thanks to the example set by doctors who have qualified from private colleges, government qualified doctors have also raised their fees to matching levels. A lamentable situation. 19.3K views View 95 upvotes View 2 shares 1 of 3 answers Upvote 95 1 2 Add a comment... Vivek Sharma · Mar 9 well said sir👏

And not just me many of my batchmates too working with me in different specialities and few of my frnds also working in us as specialist there . I had also been helping one of my brother who is too mbbs (russia) passout he as well passed plab in his first attempt and if u would be curious about neet score thn his score was 220 to be precise.. question yourself do neet score really matters absolutely no if it does thn how an ST or SC students who gets govt medical seats less than 400 Marks can be a good doctor?? .... I personally have many of my friends who belong to reserved category are fabulous doctor.. it really depends on how much u r willing to work in medical college... And yes its university don't expect to be spoons fed u like u were in coching centre. U will get lots and lots of opportunities like for research and all u just need to be eager enough to reach out to the professor and one more point plz don't take addmission in shitty cheap universities try for above 20 lacs range one.. have a great day 😊
