No Middle Easter race Australian is allowed to work at top positions. Accountability is not required if a British mafia controls the system. Many Middle Eastern staff have been forced to leave their jobs by their British-Australian management in WA. Racism has never stopped in Australia since the beginning. No policy or law can even reduce racism as long as British mafia groups control every sector of government in this country.

Not sure I like the blunt end being “fixed,” it can move but when it moves, it makes the pointy end whiplash. Small movements at the handle make for large movements at the blade.
“I want to understand the whole system,” you can do that over time in complication, but in complexity, trying to understand the system is a Sisyphean task. You’re in the system therefore you cannot see the whole of the system while likely it is not merely complex but complex adaptive hence it is changing hence you cannot see it all even if you could “get above it.”

This can be done via ICAM and the use of PEEPO- Work as intended (what the procedure states you have to do). Work as normal (how the practice may have crept due to workers understanding that there is a better way of doing the job) and work as done (what the people actually did at the time of the incident being examined). Under People, what the directly involved persons did (work as done). Under Procedures (work as intended) and under Organisation (work as normal) examining potential workaround or habit practices that have developed.

That's just wrong. Neuroscience shows that it's mostly caused by the person's automatic response to a situation. Leaders can help, but even when they do, people are still inattentive. And even if they are caused by leaders, isn't it a good idea to have people be able to deal with it better?
