I am a GP in Hertfordshire, a trainer for over 10 years, and a TPD for the East of England Deanery supporting international medical graduates. I have a lot of experience coaching trainees through the GP RCA/CSA examinations.
These are videos taking you through consultation skills from start to finish. I am confident that if you implement these skills you will pass the exam and feel more confident in all consultations.
Transparent consulting is any tool you can use to enable the patient to understand what is happening in the consultation. Why are you asking those questions, and what is happening at each stage of the consultation? It is patient centred care.
The article published in the BJGP looks specifically at changing the closed questions so they are more focused to the differential diagnosis list by engaging the patient as a partner in the process. The patient will have greater understanding of how you are reaching your decisions.

Thanks for the comment. When I get time I’ll have a look at this. Better time management comes with lots of the skills in the videos: truly understand the presenting complaint, understand the ICE, use acknowledgment/signposting, structure the management stage.

I use this in shortness of breath cases - I tell pt - lets think about your lung ..... heart ..... blood clot....... anemia ..... . do you think this is a good idea .

OMG - i got these random answers about bowel each time I ask about cauda equina ! I truly can relate to that . In my last videos I started to explain it just because I felt I need to - now with TC - we have more structure , thanks a lot Martin !
Martin this is great but in reality this can take some time. Can you record some example consultation / role play of TC that can be done ( with examination ) in 12 minutes ?
Several examples already recorded. I promise this leads to shorter and more accurate consultations. Patients will answer more efficiently when they know why the question has been asked, the diagnosis discussion will make more sense and they will understand the safety net.
I will try to do this in next week recording and let you know . Already trying to speak out loud of how to do it . Wish me luck

Martin , quick question , do you have any vedios on ( acute cases ) as the structure is a little bit different and the manegment part is mostly breaking bad news , I struggle to make it easier for the patinet . ? any ideas how to tackle that please ?
I am going to do a video on negotiating, so I will build in one or two examples using acute presentations.

can you teach us how to deal with uncertinity - like SOB cases and TATT . when we can not figure out the real cause and may be there will not be any !!!!

I am watching all your vedios about 2nd part of consultation. I do a good ice , affect effect and differential diagnosis. My struggle is explaining my diagnosis and putting a shared plan. Thanks alot for helping us . I found those really helpful. I wish i saw these before my previous unsuccessful attempt 😔.

HI Martin , I just failed my RCA - they said pt should not seen in recent past for same problem . do you think I should not record at all if they be seen at all for their problem ?
To maximise marks you need to be data gathering, assessing that data, making a new diagnosis, presenting a management plan that includes/adapts to the patients ICE/circumstances.
The case could be ok if you found new information (medical and psychosocial), there was a new diagnosis considered, and a new management plan. It is possible but less likely than a new presentation.
Thanks for your answer.. I here you do some RCA courses for EOE NORFOLK SCHEME. How can I join? Shall I ask the tpds? I need to improve my second part of consultation/ explination and CM. Also i was marked down mainly for my cases being seen before . So I am not sure now about case selections. ? Can you make a vedio about cases selection please .
