    Hello,i suffer from a abdominal bloating after each meal,early satiety, acid reflux, weight loss and 24/7 throat pain since 2012, when I had a surgery for my GERD nissen fundoplication. Gastric emptying test was T1/2 was 140min. I am looking for a solution to my debilitating health condition. I would be grateful for any information about a good specialist or the clinic in United Kingdom, where i would be diagnosed for my condition, maybe suitable candidate for G-poem.

    1-2 CM hiatal hernia's still cause VERY painful symptoms, the fact that you stated it shouldn't even be considered a hiatal hernia is dismissive to people who are suffering with symptoms.

    Awesome Video Dr Webster...After having Very difficult ERCp... Its good to understand what the problems could have been....I had a 10mm stone stuck in my CBD which by Luck came out after all 3 stents inserted all fell out into my bowel...It was very painful and I never want to see that amout of Bhile ever again....Disgusting Stuff ! But I now have a better undestanding of Stones than I ever had thanks to you . Fish and Chips anyone ?? (joking of course)

    After having a Very Difficult ERcP.....3 days later all 3! stents fell out into my stomach giving me Acute pain....Numerous Xrays and Ctc scans found them in my bowel...And nature had to take its course...Fortunately and by Luck the 10mm stone that was stuck in my CBD...got dislodged and it too came out naturally.! As soon as my Liiver F Tests appeared normal I was sent home after 8 days in Hospital with a review with the surgeon in 4 weeks....MRCP will advise whether there are any more stones and whether we need to do ERcp again !! And GBladder removal....I was extremely unlucky.....But do question if ERCP was so difficult (Due to the amount of Bhile and infection) why was the procedure not aborted...and done another day...

    I can only speak regarding my own experience. I am a very long-term PPI user (over 20 years!) Because of this fact my physician recently ran a complete analysis of my nutrient levels, did a bone density scan, basically everything he could do to see if there was any evidence of negative effects of my PPI use. Everything came out within normal limits. I do take a quality multivitamin including D3/magnesium/potassium, etc. All results perfectly normal. I am so relieved because now I have been diagnosed with Barrett’s Esophagus so I will remain on PPI’s indefinitely 😉

      but thats only reason your tests return normal, taking a multivitamin was probably counteracting it and stopping it depleting those things, its proven taking PPI's just for 3 months lowers magnesium levels.

    Great imaging skills ! Q: The gastric mucus is only ~1mm thick, so how can an 'atrophy' of it lead to those large folds? But if the smooth muscle layer is involved in the atrophy, then it makes sense.

    Thanks for sharing your case , but it seems you made the procedure more difficult by using long TTG knife and doing incomplete mucosal incision from the start

      Using standard extraction balloon. But don’t get greedy! Start distally and gradually move up duct taking fragments out. If you try to take them all in one go they’ll get impacted at the distal end and you’ll be stuck!

    Good news to everyone that I'm no longer suffering from gastritis 😊. All thanks to God and my sincere gratitude to Dr unugboje.     I have been on Pantoprazole for 15 years and stopped it last year, and I am really surprised how this "Doctor" just blows off the side effects. I had a constant b12 deficiency and D3. Now at 59 diagnosed with Ostropenia as a result of the PPi preventing the absorption of these vitamins. This doctor is so typical of how doctors think. Go on to Medscape and read the side effects.

      I have been taking PPI for several years, not sure how long , and also have B 12 deficiency. Also I think it has caused a chronic over-growth of yeast in intestines.

      eat more crucerficious vegetables like broccoli and cauliflower these have protective effects against H.pylori and heal gut lining but they are also Antifungal and PPI's feed candida look up the powerful effects of sulpherane on your health, I use to be on PPI's but were doing too much damage they also block calcium and cause magnesium deficiency as well.. should see some of the crappy additives/binders they use in PPI's, polysorbate 80 is one of the worst ones and that was in my Omeprazole Activas PPI.

    Is this only applied on stones in the duct or also on stones inside the gallbladder?

      Stones in the gallbladder are addressed with surgery (i.e. a cholecystectomy). Its rare to use endoscopy to address these. However, in patients who are not surgically fit for a cholecystectomy then drainage via EUS-guided lumen apposing metal stent (LAMS) is possible. Check out a video on our sit EUS-GB

    We use double pigtail regularly for hilar strictures and do not see the problem you mentioned. We use 8.5 Fr. though; could this be the reason?

      Because pigtail stents are narrow they're not designed to drain bile through them - but round them. Placing a few pigtail stents through a hilar stricture may therefore create enough space for the bile to drain through. However, a 10Fr straight stent is preferable (until a tissue diagnosis is confirmed)

    Nobody warned me about osteoporosis. I have been eating PPI's for 6 years and now I have low bone density. It's not osteoporosis yet, but close to it. I never trust doctors again. I should have read about it more, but all the medicine has huge lists of possible side effects, I didn't know this was one to be taken seriously, well now I learnt. At least I stopped using it before the situation got even worse, but I wish I would have never started it. So stupid of me to trust doctors.

    I've suffered for 10yrs+ a myraid of digestive ailments. My quality of life is terrible because of it. Everywhere online you read about how bad PPIs are and i've never taken them for more than a week or two because of it. They never really got rid of my symptoms either. Despite that I am going ot introduce them because of your video because this burning right now is too much! Fed up with this constant mucus in my mouth and bad breath on top of shortness of breath.

    Instead of this blind following and sole dependency on 'data' and 'studies'...a major problem with Western medicine and secular science in general...How about instead understand attributes and causality. PPIs dramatically cuts down stomach acid. Now understand that stomach acid is an important part of the digestive and immune system, so its absence will certainly cause problems... Acid is used to absorb nutrients...protein, fats, vitamins and minerals...and the hindrance or depletion of those causes all sorts of problems with tissues, processes, organs and function. A PPI will seemingly solve or reduce one problem/symptom but then create several others. H2 blockers being less dramatic in acid reduction are probably a better option in many cases as they can be altered more in dosage...but become less effective the more you depend on them, then you have to have more etc...And when stomach acid is stopped, when that stopping agent is removed...then the body starts producing more acid than normal and that then returns the symptoms. I had really bad internal pains stomach and also in back and below shoulders at back and I was put on PPI for a month and it removed the symptoms but I started feeling ill in other ways, and when the PPI stopped all those internal pains returned worse than ever. So it didn't cure anything. And before ranitidine was removed from the global market I got a load and used that instead...but I had problems for 18months...and it was only by stopping all pharmaceuticals and eating when I was in pain, and maybe other issues...my problems went away...but now after about 18mnths of being free, my problems have returned and have been in pain for weeks...and maybe that was because I stupidly started having too much coffee or maybe the cause was something else. But regardless, I intend to try and use PPIs and H2 blockers as little as possible in dealing with it, and PPI is always the last option, the nuclear option which stops the symptoms in an effective way but solves nothing. PPIs are also expensive, as compared to previous ranitidine which could be bought for £1 for 12 75mg tablets. And in UK we have not had an alternative H2 blocker made available over the counter. A lot of people are sick with a lot of conditions and we find a lot of these people are on long term PPI use without being aware of all their side effects and consequences, even though the Pharma manufacturer literature reveals a lot of these symptoms and side effects.

    Been taking 40 mg pantoprazole every day for the past few years. If I don’t take it I literally throw up stomach acid non stop. The only things I’ve noticed since taking it is weight loss. I literally can’t gain any weight no matter what I eat. If I don’t take it I notice just a few hours later and the throwing up starts.

      I just started using omeprazole (otc) a day ago after using the h2 blocker pepcid for a year (started out great eliminating reflux and other gerd things but it is a bit ineffective now). Just in one dose it's like I don't even suffer from this crap. Pretty pleased so far. What do you think caused your weight loss? Does it reduce your appetite? How long before you noticed you were losing weight?

      You should look at getting help from a functional medicine doctor. You obviously have a reason why you are in misery unless you take a ppi. Might as well get to the root cause.

      There is a rebound effect when stopping a ppi. More stomach acid is produced than normal. Have you tried tapering off them? Your situation sounds uncommon

      cause its stopping you from absorbing vitamins and other nutrients hence the weight loss, get off this crap and just eat more vegetables like Broccoli and Cauliflower 2 of the best for gut health and combating H.pylori which causes ulcers in first place, cut out coffee but if you cant then add almond milk to it to reduce the acidity.
