Training as an Athlete and as a Surgeon | Guest: Dr. Michael McCarthy
A fun guest joins us on TheAthleteSpine for this most recent episode - Michael McCarthy, who crossed paths with Dr. Louie multiple times during their training days! Dr. McCarthy is a Partner and Orthopaedic Spine Surgeon at The Indiana Spine Group and also serves as an Associate Professor for the Department of Orthopaedic Surgery at Indiana University.
We dive into the similarities of training as an athlete versus training to become a surgeon, and the ongoing training that we as surgeons undergo.
- Actually set aside time to reflect and purposefully learn.
- Refine physical skills through improving mental engagement during a task. Practice focusing solely on the task at hand.
- Develop a program for deliberate practice in which you can undergo critical assessments to allow for graduated improvement.
- Find others to “train” and “Work” with intermittently for feedback
- Create and consistently review step-by-step notes on specific techniques to develop proficiency that can be translated directly to intraoperative opportunities.
Just as athletes refine their craft through dedication and purpose, surgeons too must adopt a mindset of continual improvement - it’s called the “practice” of medicine for a reason. In this episode, we explore how deliberate practice, mental engagement, and reflective learning form are critical for growth in both fields. Whether on the field (even the weekend warrior) or in the operating room, progress demands focus, feedback, and a commitment to simply “get better”.     Gents at about two minutes in toundisplau a SCUBA diver when discussing activities that may contribute to pars fractures. I've always understood diving as in platform, cliff and pool diving?

      Good question --- but a tough one to answer! With enough impact and an "extension force" in your low back --- diving/cliff jumping could be risk factors for pars fractures. There are also many people who perform these activities that do not develop pars fractures either!

    hi ive got 2 stress fractures l4 and l5 and ive been diagnosed and rested for 6 months and even though pain has decreased it is still there and its been a decent amount of time just wondering if im ever going to recover and be able to do the things i enjoy again

    Except I’ve had this for 2 years due to having slipping rib syndrome. It was causing so much irritation at the costovertebral joint because of the slipping happening on the anterior part of my rib cage. I’ve had 2 surgeries for it, and it helped all my SRS symptoms but my CV pain still persists…

    I have a question, i had to completely stop playing soccer because Everytime I practice after a few times striking the ball i get a pain and i get severe drop foot later that day and it lasts a few days until i can move my foot. I tried stopping for months then trying to play but I experienced the same issue, except now the pain doesnt go away, my leg is weaker, and i feel a tingling from my toes up to my lower spine. Is this a stress fracture or a pinched peroneal nerve?

      Great question! Yes -- this could be peroneal nerve related, but could also be nerve compression from the lumbar spine as well (ie. a disc herniation). Although a fracture may be present, that may be less likely, and the fracture itself may not be directly causing the symptoms.

      @@armxxn3048 With stress fractures --- its really just rest and trying to actually refraining from bending/twisting/lifting activities --- sometimes for 3 months ---- as the initial treatment

      This is a good question --- and a difficult one to answer broadly! Ultimately, it comes down to your specific anatomy and what you want to return to. You are able to return to a large number of activities after either a disc replacement or a cervical fusion.

    I underwent spinal fusion surgery by Dr. Baron Lonner (and his co-surgeon Dr. Wesley Bronson) in early March 2022. A week after being discharged, I complained that my back felt abnormally swollen and painful. My concerns were quickly dismissed. I had to suffer for another week until my first post-op visit. At that visit, the X-rays revealed that the surgical hardware had begun loosening and pulling away from my spine causing a collection of fluid and swelling on my back (the same swelling I had described a week earlier). As a result of the failed and incompetent surgical approach taken by Drs. Bronson and Lonner, I was forced to undergo a second surgery in late March 2022, costing me additional time away from work, school, my personal life, and negatively affecting my mental health. The second surgery also worsened my scarring, creating a very wide keloid scar. As an active and fit young adult, my chances for having a successful surgery should have been significantly higher. Instead, the services rendered by Drs. Bronson and Lonner and the Mount Sinai Spine Center were significantly below the standard of care and were negligent. Despite their own medical failures, my insurance was billed more than 1.3 million dollars for both the original and revision surgeries. It is unfathomable that Drs. Bronson and Lonner would feel entitled to be paid twice when they failed to do their jobs competently the first time. I am currently receiving post-op care at a different facility and with a different care team because I no longer trust Drs. Bronson and Lonner with my health and safety. I strongly encourage those reading this review to seek care elsewhere!

    What can be done as a preventative measure to limit these types of injuries? My experience with these injuries in soccer is that they tend to occur when shooting and hyper extension of the back with a rotational component. Do you have any experience with certain types of physical therapy, pilates, sports/mobility training that helps to stabilize the spine and limit this type of injury? What do you specifically recommend?

      These injuries are hard to "prevent." Many of them occur in young pre-teen athletes that are already active and in some type of hyperextension activity! I wish I had a better answer than that ---- but this is the reality of pars fractures that occur in the younger population. These do not occur as commonly in adults.

    This is great content guys. Thank you! You listed various reasons for continued nerve pain after spine surgery. How often would you say the reason for continued nerve pain is due to the nerve simply not healing vs. nerve not being adequately decompressed or recurrent disc herniation? In other words, would you say it’s more common to find a reason the nerve isn’t healing or is it more common that the nerve simply doesn’t heal back?

      Thats a really good question, and I'm not sure I have a great answer to that. I think the situations are all so variable in this case. Yes -- there can certainly be ongoing nerve compression from incomplete decompression or recurrent stenosis from a recurrent herniated disc or even some scar tissue forming. From a nerve healing perspective, the severity and duration of compression can likely impact the ability to "heal" once the decompression has been performed. We ultimately do not understand nerve healing all that well. There is some evidence to show that these nerve roots often have the capacity to heal for about 12 months after surgery. So, where you are 12 months from surgery is likely where you will be as a "new normal." But, that is just an average. Hope that helps!

    Hi, I have a question about disc replacement vs fusion vs just rehab with physical therapy. I am not a high impact athlete, but I am firefighter in a busy city where we tend to go to a lot of fires and dangerous situations. I personally have a 5mm bulge in c5-c6 with weakness in my deltoid that is responding to physical therapy, but am worried down the road what I may be in for. If a disc replacement would be best, to strengthen the neck. Any advice would be great.

      Thats a great question --- with a complicated answer without seeing your imaging and your situation. The ultimate goal of a surgery in your situation is to take pressure off of the nerves; whether that is accomplished with a cervical disc replacement or fusion depends on a lot of variables. There may be aspects of your XR/MRI/CT that will lean us in one way or another. Our studies show, ultimately, that there are no major long term differences between a single-level disc replacement or fusion as long as we your nerves are adequately decompressed and the pressure is relieved. My bet, is that you would do well with either and hopefully return to work and your activities.

    I ride broncs for a living does anybody that has done this think it's a good idea if I want to keep going or should I wait. I'm 26 and have a good amount of atrophy in my riding shoulder and bicep. Has anybody tried decompression? I want to avoid surgery but I'm losing strength and muscle by the day

      I’m approaching 14 weeks post 2 level cervical disc replacement with Dr. Nemani, that happened October 26th, and doing well. I could be that 50+ woman he mentions! I’m so thankful that my insurance “forced” me to travel to Seattle for my surgery. Thank you Dr. Nemani!

    Why did they not perform a prothesis disc replacement instead a fusion ? I did have same . Surgeon Fusion on one level c5-c6

      Good question! I think there is still some hesitancy to perform disc replacement in high-level football athletes, largely due to the unpredictable forces to the neck. There are many that have undergone a cervical fusion in the NFL. And there recently have been 3 NHL players that underwent a cervical disc replacement. So, maybe in the near future?

    I have a question for you. When the stress fracture is full fracture and it is an acute injury with edema in present, is there any chance those fracture rejoined as boney healing?

      yes, there is a chance! It depends on several factors, but most people will give the fracture a chance to heal first.

    Radiologist here. No way could I get a needle there under fluoro. Do you guys think this is really what's going on? The calf strain last year was also very odd. Never seen someone miss that much time over that injury.

      Hi Will! We discussed this with a lot of friends and colleagues that perform many injections .... and many agree with you! That is a scary region to be injecting a needle, with high risk --- and no obvious certainty that they could confirm that location with flouroscopy.
