Mastering Biomechanics in Orthodontics: Dr. Gustavo's Insights
It was an absolute pleasure to connect with the London Dental Institute Group and discuss one of my greatest passions: biomechanics! The warm welcome from Dr. James and Dr. John made this an incredibly rewarding exchange of ideas. We explored how biomechanics serves as the "secret ingredient" for mastering orthodontic control and understanding both the desired and unintended effects of treatment mechanics.

    If I have to do buccal ectopic canine extrusion, will I open the angles 1,3,4 and give palatal root torque and a slight lingual activation to avoid buccal flaring of the crown? Is this correct?

      Yes. But the lingual root torque and the extrusive force will already avoid buccal flaring of the crown. The slight lingual activation is not needed.

    Great, very useful in hybrid treatments with aligners, where I first close the space with sectional and then align with plastic! I usually use an indirect anchorage, I connect the tad to the second premolar and first use an alpha beta spring in geometry VI then I mesialize with a PC from lingual and buccal.

      The archwire in the opposite quadrant (anchorage unit) is to minimize or avoid the movement in this side.

      It depends on the intensity of the inclination. I'd say that most cases are solved between 3 to 4 months, no more than that. Some times is even faster.

      the tear-drop loop is to reduce the load-deflection rate and the omega loop is an auxiliary that can be used to activate the appliance. Instead of activating from the distal (as in the video), you can tie a ligature wire from the molar to the omega as a second way of activation.

    Dear doctor, I have ordered and read your book. I’m confused about 6 geometries. Can rectangular loop and root spring create geometries I,II,III ? or just IV,V,VI?

      It's not impossible....but if we focus on ONE unit (the active unit that we want to move), and you draw the desired forces for this unit, you'll see that the forces needed to achieve equilibrium will always result in a geometry V, V or VI (considering the diagram of force system of our two-tooth segment.

    If the molars have different tip inclinations, does the activation have to be different to avoid anterior cant? Could it be useful to make two separate cantilevers?

      In this case you should try asymmetric bends ("tipbacks") until the anterior part of the arch is relatively symmetric in the anterior region.

      Not a good idea with R-loops! Because this torque also adds buccal force in this case (association of forces and moments in this plane of space with this tool).

    How can we know if only moments are acting in trial activation?

      In the trial activation, we apply gentle couples (as shown in the video)....and then you check if the parallelism between the appliance and the tube occur exactly at the tube level - in this case, there will be only moments acting. If during trial activation you observe that the parallelism occur when the appliance is below the tube, there will be an intrusive force on this unit. On the other hand, if the parallelism occur when the appliance is above the tube, there will be an extrusive force on this unit.

      Sorry, now I got your question. The answer is the same - yes, 200 g is enough even for the entire distalization of upper arch. It may seems strange that we can use this same magnitude for different purposes, but it's true. Literature has shown that around 200gm is enough even for distalization of the entire amndibular dentition - Roberts WE, Viecilli RF, Chang C, Katona TR, Paydar NH. Biology of biomechanics: Finite element analysis of a statically determinate system to rotate the occlusal plane for correction of a skeletal Class III open-bite malocclusion. Am J Orthod Dentofacial Orthop. 2015 Dec;148(6):943-55.

      by the application of a lingual force, in combination with the intrusive force. Therefore, note that I'm applying TWO forces simultaneously - the intrusive force (2nd order preactivation) + the lingual force (1st order preactivation).

      The aim was to produce a translational movement. Some tipping occured because I exaggerated the reactivation in this simulation. Clinically, I prefer to already add bends (gable effect) to minimize the final molar angulation problem. If necessary, it's also possible to make the bends and cinch the teeth for later correction.

      Hola Dra. Las fuerzas y momentos percibidos por el mini-implante pueden ser soportados sin problemas, siempre y cuando en este caso (lado inferior izquierdo, por ejemplo) utilices un tornillo de rosca izquierda. Esto se debe a que, al aplicar algunas dobleces gable (no demostradas en el video), el tornillo recibirá un momento antihorario, mientras que el molar recibirá un momento horario para minimizar la inclinación hacia mesial. Gracias por la excelente pregunta y la oportunidad de aclarar este punto.

      Yes, if you do not use the appropriate TAD. In this case, a left handed screw should be used.
