Great presentation. This the best I have seen on US guided venous access.
Your confidence scale reflects inefficient rollout. You shouldn't start on challenging patients, you should start on the extremely easy patients. The ones where you can clearly see the veins. Where they're superficial, straight and large caliber and gradually migrate to more challenging veins.

As RA pressure increases, there is flow reversal. The S wave becomes positive above the baseline as blood flows towards the probe. And this corresponds to more blood flowing away from the heart and making congestion in liver and abdomen worse?
