Industrial Safety and Engineering Company has two main departments:
1-Incident investigation: ISEC is an independent office investigates about incidents in the oil and gas industries and determines the root causes and conditions which led to the events. All the investigative staffs include electrical, chemical and mechanical engineers and safety experts who have worked in industrial plants and have years of experience. After completing the investigation and final report, their safety videos are produced by industrial animation production department of ISEC.

Rated load capacity label is installed next to the master control panel.
At the condition of water tower operation 1000-1500 GPM water stream, for the elevation angle from -10 degrees to 45 degrees no one is allowed to stand at the tip but for the elevation angle from 46 to 85 degrees, one people is allowed to stand at the tip. The maximum weight considered for each person is 250 LBS.

What was the reasons for the incident?
Might have been a corroded pipe seeing as it was a common occurrence, or maybe an emergency pressure valve not properly directed to a flare tower or safe place to vent…who knows?

I had no idea ammonia was such an explosion hazard. I live in farm country and see the tanks of anhydrous ammonia being drug around every spring. They used to marked with a placard that said Non Flammable Gas. Now they get a green diamond marked 1005. Still nothing to indicate that the contents can cause other materials to explode.
That's right. In fact this explosion occurred by Hydrogen gas. Hydrogen is the process gas in this units.

Ammonia is rated JUST below highly explosive. It takes about 16-25% by volume in air and an ignition source of 1,200°F to ignite. Not impossible! But not usually likely, so long as proper safety measure are implemented. I used to be an ammonia refrigeration tech, and at first, it scared me a lot… You get used to it 😬

Propane is never stored in tanks as shown. It is stored in bullets at high pressure to keep it from becoming a gas.
