Clarification: she was NOT on the stage. She was suspended by wire, which came apart and she fell below the stage. CdS is ALWAYS safety first. There are some specific performances in which the performer is not tethered (the Wheel of Death is one) so the risk is definitely there. The comment of "What a shame..." could also be said about those who drink and drive and kill someone with a 2+ ton vehicle.
