Hello, Information PGAD /SEGP : ( I have this Segp syndrome since 2021 , the cause of my Syndrome is of neurological source = Polyneuropathy peripheral at the level of certain nerves or because of the continuations of the former operation of a herniated disc L5/S1 ) I've come here to inform people who, like me, have persistent (permanent) genital arousal and who are looking for the reason for their PGAD / SEGP syndrome (apart from psychological causes) and who, as is often the case, find themselves in medical limbo. What is still regrettable in 2024, is this lack of knowledge on the part of many general practitioners and specialists: Gynaecologist, neurologist, urologist etc... So it takes a long time to find the cause and get treatment! So it's up to us to gently explain the PGAD syndrome, its various causes and the tests to be carried out! Of course, some doctors and their egos don't always appreciate this! But “it's not serious”, you just have to gently insist and they usually accept, and I know what I'm talking about! If possible, avoid treatment with certain antidepressants in the SSRI family (a list of these drugs can easily be found on the Internet), which can also trigger a Segp = hormonal imbalance during or after discontinuation of treatment! Here are the main causes of PGAG / SEGP : Often it's a Tarlov cyst or a disc protrusion (even asymptomatic) at the level of the sacro-lumbar vertebrae, which compresses or irritates one of the 4 nerve roots (nerves) of the great pudendal nerve, which originates at the level of these sacro-lumbar vertebrae and runs to the genital organs, including the dorsal nerve of the clitoris! Reference examination MRI of the dorsal spine. Often: a tarlov cyst(s) irritating or compressing part of the pudendal nerve (in the perineal/genital area). All it takes is for a nerve root to be irritated or compressed to cause permanent genital arousal! Reference examination: pelvic MRI. Other causes: Sometimes due to a neurological disease (S.E.P or perineal pudendal neuralgia) which causes peripheral neuropathy (nerve damage), especially when it affects the great pudendal nerve. Reference examination: EMG Electrophysiology Perineal Pudendal or EMG of the lower limbs, which I also had to do. Another very common cause of Segp: pelvic/genital varicose veins. Pelvic varicose veins mainly develop in women after childbirth. Occasionally, but more rarely, they also affect young people who have not had children. A basic examination is an abdomino-pelvic MRI or a more precise phleboscanner ! Other causes: Genital infection, clitoral mycosis, vulvodynia, clitoridynia or pelvic/pudendal neuralgia, which can sometimes also cause Segp! (Remember: the Pudendal nerve is the large nerve that runs from the sacro-lumbar vertebrae to the genital organs). Other causes: Sometimes it's due to inflammation of the large sacrospinous ligaments, which affects the pudendal nerve, or alcock's canal syndrome, where the pudendal nerve is sometimes irritated/ pinched. The ligaments are very close to the great pudendal nerve and can end up touching part of it, creating a PGS! Consult a good physiotherapist or osteopath first, and if there are no results, see a neurologist or neurosurgeon for possible infiltration of the pudendal nerve or surgery to free the pinched pudendal nerve! Basic examinations to be carried out first: an MRI of the dorsal spine , an Abdomino-pelvic MRI . the most complete EMG and Perineal Pudendal Electrophysiology to find the affected nerve(s) or, even simpler, an EMG of the lower limbs . Doctors to consult: Urologist, gynecologist, neurologist/neurosurgeon, phlebologist for pelvic varicose veins. Even if the medical course is sometimes long and painful morally, especially do not discourage you because there are solutions and also to cure! Sincerely 💚🌷
