Hi, I am Dr. Srikanth Nithyanandam, a Primary Care Sports Medicine doctor practicing in the United States. I serve various roles as a clinician, such as a hospitalist, outpatient clinician, team physician, and medical director for Exercise Physiology Lab.
I completed my medical school from PSIMSR, Coimbatore, India. I did a Masters in exercise Physiology at the University of Kentucky. I then went to complete residency in Family Medicine, Fellowship in Academic Medicine and finally a Fellowship in Sports Medicine. I just recently completed an online Integrative Medicine Residency Program through University of Arizona.

Dr, I work on boats, I had a rough day on the water, boat bouncing a lot. At the end of the shift I couldn’t walk. I have severe pain in the front of the knee, and on both sides. Twisting it was Severe and made me pass out, on the sides is extremely tender to touch and a lot of pain and pain right on the knee cap. MRI showed no tears just patella maltrscking but I got worse after a week of resting and icing and I now completely can not walk. What could this be ?
Unlikely to be anything significant. But still your symptoms of not able to walk is concerning and I would recommend checking with a doctor to make sure all ok.
Hello Doctor... I had an MCL injury while I was playing volleyball. A boy leg hit on my knee and i fell down. The doctor told me I had an MCL injury. Its 20 days and I can see there is no pain but when i try to stretch the leg straight... I feel some tightness over there and also knee doesn't bent. Does these exercises will help me ?

Hello Doc. While jumping I had a innerward bend in my knee. From the pain and symptoms it seems it's MCL sprain grade 1. It's been a week, I can walk, I limp sometimes in between and have a problem in straightening my leg or bending at it's full. I'm having my Army interview next week. What am I supposed to do? I need to perform jumps and runs there.
I am concerned you could have a acl injury or could have subluxation of knee cap. I recommend seeing a physician for a thorough evaluation
I've been trying your exercises and it's really getting me to work. Also the medications and home remedies are working. From yesterday night I can straighten my knee without any pain. It's now matter of squatting.

I have stage 3 osteo arthritis in the left knee and stage 2 in the right and am considering stem cell therapy. My question Doctor, is if I get a hyaluronic acid shot, can I still try stem cells later on? if so, can it be done soon after the h/a shot? in other words, are they a compatible therapy and could be done together? Thanks
While I don't have extensive experience with stem cell therapy, I am familiar with PRP (platelet-rich plasma) treatment, which is somewhat analogous. I have no objections to performing PRP procedures following hyaluronic acid (HA) injections.

Thanks for your video Doctor. I met with car accident three months back and still taking bed rest. My both bones fractured below the knee. I consulted with the doctor and used knee brace for two weeks. I took mri 4 weeks after the accident. Mri shows fracture and acl tear. But I never have ACL symptoms. But after one month of accident I started to feel MCL symptoms. While walking with the walker inside the house i feel pain in mcl area. I have all symptoms of MCL..But MRI did not show MCL. .Now I sm walking with the walker without putting weight in one leg to avoid pain. How to recover from MCL pain doctor.
Given your history, a proper clinical exam is important and i would not go by MRI findings, Would recommend seeing a clinician to address this further.

Hello I strained my MCL 6 days ago but I don’t know how. I believe it’s a grade 1 since I am able to bear weight on the leg and can do slow full ROM squats with balance assistance. Initially, I felt a pulling sensation when I would bend my knee but it went away around the 5th day. Should I start at phase 1 and continue until the end of phase 2?

However, it's also very important to get a professional evaluation to ensure that you've correctly identified the issue and that everything is indeed okay. A doctor or a physical therapist can provide you with a precise diagnosis and guide you on the best course of action for your specific situation.
@ thank you for the reply. I tried phase 1 exercises yesterday and once I reached the 2nd exercise, around the 14th rep my knee began feeling off. The sensation of a pull in the inner part of my left knee returned. When I hyperextend (when standing) it doesn’t feel normal.
I just bought a bike. I can't wait for it to come in. I will start eating healthy, consuming less carbs and biking every morning before work! I weigh 225 lbs and running is difficult for me right now, so I am gonna bike until i'm confident enough to start running.

As far as I know , intraarticular cortisone shots are done when we have suprapatellar bursitis accompanies the osteoarthritis , just to make bursitis better. So is it also injected without having any bursitis ? Thanks

I notice many patients with knee osteoarthritis also come with suprapatellar bursitis with accumulation of synovial fluid . My question , When should we aspirate this synovial fluid accumulation ? What if we not treat this synovial fluid ?will that be absorbed by itself ? If not , how to treat this synovial fluid accumulation ?Is there any alternative to aspiration, something noninvasive ?
in my experience, these are just effusion inside the knee joint. most time they resolve on their own. in some case, i do consider aspiration followed by a steroid injection or prp injection on a case by case basis.
Hello, As far as I know , Heat is contraindicated when you have acute cases like acute pain in the joints due to osteoarthritis or acute bursitis , or acute tendonitis because heat increases the blood circulation to inflamed tissue and due to venous stasis you get more swelling and worsening of the case. I just heard this , please correct me and let me know what do you think about it.

While it's important to approach each case individually and with caution, the idea that heat therapy exacerbates inflammation isn't as clear-cut as previously thought.

Jogging is a higher impact exercise than walking but must consider that jogging covers more distance with each stride than a walking step. The critical number is cumulative load. And the difference is not that significant. So bit misleading to say jogging is higher impact. If we have a 100 pound person walking 1 mile: 2000 steps X 250 lbs per step = ~500,000 lbs cumulative load. If we have a 100 pound person jogging 1 mile: 1600 strides X 420 lbs per stride = ~640,000 lbs cumulative load. Do you agree? 🤔

@@srikanthnithyanandammd Yes, my doctor told me to walk (diagnosed with knee OA and normal BMI). I asked him if I could jog. He said to listen to your body. So he did not restrict anything. In my experience, walking does not worsen symptoms. I typically walk two miles a day and rarely have joint pain the following day. I recently tried mixing in a 1/4 mile jog as part of my 2 mile walk and experienced moderate joint pain the next day. Expect that the joint pain will mostly resolve within 24 hours. Will continue to monitor. My goal is to jog 1 mile and walk one mile. I think it is possible with gradual introduction.

Recommend 2 times a day if you can be consistent. Consistency is more important to improve flexibility.

Hello sir , I had an accident 5 weeks back and got my MCL injured ( Grade 2 on MRI ) and complete MPFL tear but on clinical examination kneecap was not dislocated according to my orthopedic surgeon so surgery was not recommended for complete tear of MPFL . From last 5 weeks I’ve been using hinged knee brace continuously but had no exercise. I was on rest and now I can’t fully extend or flex my knee which is making it difficult for me to walk . Please make it clear for me from which exercises do I’ve to start and for how many days should I continue it ? Should I wear my knee brace during exercises or not ? And until when should I leave wearing a knee brace ?
Most of the time, the course of treatment depends on the clinical examination. If you've experienced a patellar dislocation, there's a chance you might also have a slight MCL sprain. However, the primary focus should be on healing the patellar dislocation. In cases involving an MPFL injury, I typically advise two weeks of immobilization to allow the ligament to heal properly. After this period, it's beneficial to begin slowly increasing the knee's range of motion. If it's been five weeks and you're still unable to completely extend or flex your knee, I highly recommend consulting a therapist. This situation calls for specialized, one-on-one rehabilitation to prevent the development of knee stiffness, which could lead to long-term issues. I hope this advice is beneficial.

Hello doctor My bp was high in jan 24 so doctor proscribed me nebevolol 2.5 mg daily my age is 31 now my bp is under control and my bp is realated to indigestion and hyper acidity so should i continue the drug or get ride off slowly.

It is unlikely you are injuring your mcl. Possible it could be pes anserine bursitis. Please discuss with a sports medicine doctor and sports physio to get a proper evaluation

Sir my knee is in flexed position while walking and standing.But i can do all exercise.struggling with it .some kind of numbness in foot ..I had taken mri but it only shows lateral root meniscus fraying . I m walking now but sometimes when i put my weight some kind of buckling causes . I had feeling of strectch pain over femoral ending of MCL which is quiet pain full Knee immobilization caused my knee muscles to weakness. I m fed up with these all . Hai Doctor I met with an injury on my left leg.on august 28 while playing football .the ball has hit directly to my outer part of ankle.At that time i doesnt had any pain .But 2 days after i felt pain in my medial side of left i doesnt cared abt it .Gradually the things get worsen. I really get appointment with the doctor and he assesed my knee bt not cleanly he didnt get it.i had pain on compression test and valgus stress test. Afterwards i had been on a stick.After that my cosin is an ortho dr has adviced me 2 wear knee immobilizer for 3 weeks . She said mcl and lcl injury. I had been immobilized for 3 weeks .after 3 weeks i tried to walk but its very difficult for me and i m struggling.But by the time i had taken an MRI .Subcutaneous edema,mild joint effusion,fraying of anterior root of lateral meniscus. After completing that month i had started gradual walking. But still i had limping,and knee in flexed postion during standing and walking . What has happened to me. I need my swelling to reduce .i want to return full level of activitym. When will ibget fully recovery please rply sir
