I advice you to slow down your talk, and think before you talk next time! This ratio thing are just bullshit, humans are obsessed with numbers and ratios, there are huge sums of money involved fooling people with dangers . Arterie disease has nothing to do with numbers and ratios!

Doc - I am a LMHR with enormously high LDL, super low Triglycerides and, what one might say, very good levels of HDL. I have absolutely loved your presentation; thank you. In my being in the seventh decade of life, this without having one poor metabolic marker - (well, I do have high cholesterol) - I must hold my lane, trusting that my high fat low carb dietary choice over several decades have brought me the metabolic health that I enjoy. Your non-dogmatic, and balanced approach, is much appreciated.

Really insightful! I just did my blood panel (46yo f) and have elevated LDL and total cholesterol, but ApoB and all the ratios show excellent metabolic health, along with HgA1C. Been LC and physically active for years, so I feel better now, understanding what these ratios mean, thank you!😊

Does 2000mgs of omega 3 fishoil cause stroke
Unfortunately that is going to be impossible to answer. Overall the data doesn’t look like doses around 2g produce a ton of side effects, but it’s so variable for each individual. But overall 2g is not a super high amount

Overall recommendations are about 500mg of DHA and EPA and typically that is found in about 2g of fish oil. So your dose would be a normal dose but unfortunately I can’t tell you specifically if that’s right for you!

So basically, Anabolic window is bullshit. What about protein distribution? I read about maximizing protein synthesis if the protein consumed is divided within the day compare to taking one/two big portion. Whats your thought about this dr?
It all comes down to “optimization” vs “good enough.” I’m a “good enough” guy and so BY FAR the most important thing is total protein intake. If you wanted to squeeze out every possible little gain then there is some data suggesting that multiple protein rich meals can spike muscle protein synthesis and may lead to some improvements. Do worry about that specifically? No. As with most things, you can get super nuanced, but for the vast majority of people and their health needs, keeping it simple wins.

That’s a great question. I Assume you would want to do it around a workout to optimize muscle protein synthesis, but that would be an interesting experiment!
I have CECS in both of my lower legs and also both forearms. Has there been any upper body management techniques that you know of?
That’s a tough one! Usually activity modification is the big treatment plan, and I don’t know of any amazing treatment options for upper extremity cases. However, Botox can be used in the lower extremity, and theoretically could be used in the upper extremity as well, but that would be more experimental.
@@jrennickemd thanks for the reply! That’s what I thought, there is far less info for the upper body out there but it’s also much more easily managed- just do less pull ups and grip strength exercises basically 😂

The 3rd study is really interesting. I wonder how much of the life extension has to do with sports. I would speculate that the reason why people who do sports live longer than the people who do hard work is because who do sport have stong drive, optimal mental healrh, and optimal genes, which makes them be good at sport and motivate them to do it more.

When you are depressed it’s hard to even get out of bed but exerscing I think is key to feeling better
100%. It’s easy to say but hard to do. But worth it if you can!
Talong antindepressants for at least 10 years on and off, had severe depression 2 times . Has helped but side effects are low sex drive and confusion. But I’m still exersicing regularly, light weight training and jogging
That is one of the big problems with any medication… side effects. But glad you are routinely exercising now!
I had Botox for PAES, yet i'm getting the fasciotomy for CECS.
That is super interesting. Where did you get the Botox into for PAES?
@@jrennickemd Bristol southmead hospital in the UK. First time they'd injected it in the UK, i was on a clinical trial for military personnel only too see the effects vs decompression surgery. I walked up snowdon 2 days after the injection. I had the 6th type of PAES, which is where everything is where it's meant to be, but the gastro muscle is too big and crushed the artery. The way they told me it'd work, it paralyses the muscle, stops it for working at all, this causes muscle atrophy without cutting the muscle out. I'm very certian this botox for PAES and CECS was established by "The McGinley Clinic" This is where everyone in the states who has money goes to get the botox. For the botox it was Prof. Robert Hinchliffe And for the CECS its with Prof Paul lee of Harley street.

What are your thoughts on high cholesterol
Oh man that’s a loaded question! I generally think about it this way: elevated lipoproteins appear to be a risk factor for cardiovascular disease. It may not be the whole picture, but it is part of the picture. Therefore, I believe in trying to have a reasonable level just like I would want a reasonable blood pressure and insulin sensitivity to lower my overall risk.
What about krill oil supplements..
That’s a great question. This article did not specifically talk about krill oil so I can’t specifically speak to that. Definitely a great question though because if we could get the benefits of omega 3s but without the risk of AFib that would be ideal!
I took 4 grams of fish oil every day for 15 years. Started to get bouts of AFIB that lasted 10 to 15 seconds at a time. So I quit the fish oil and I no longer get these bouts of AFIB.

I remember the hulaballo (sp?) around 2013. A reporter called a few people who took fish oil. The reporter called again after a few weeks and when those people forgot about the first call the reporter concluded fish oil doesn't improve memory ergo it's bad for you. The news organization distributed the story apparently without reading the "study".

It’s almost like a medical degree is useless since you don’t investigate any disease you diagnose, but instead rely on assumption from medical journals published by corporate interests. Glad you spent 8 years being useless though.

Cohort studies have so many confounding factors and it’s almost always impossible to determine causality. The study does not consider these important factors: 1) genetic, 2) granular physical activity (e.g. minutes/week of aerobic exercise, strength exercise, etc), 3) detailed dietary information, 4) sleep, 5) other medications and adherence to medication, 6) detailed information about fish oil supplements (e.g. epa/dha), 7) inflammation markers, etc.
Very well said. I agree that there are many confounders like you mentioned. But each article helps us put together the whole picture!
Ray Peat raised the alarm on fish oil 25 years ago. Basically, he thought it prevented inflammation by suppression of the bodies natural response to injury. If that injury is chronic and sustained, there will eventually be problems. Perhaps sudden events. So fish oil should be treated like Tylenol. Don't take too much or habitually for the long-term

Cholesterol used to be bad, now is good, Omega3 used to be good, now is bad, a few more years and they will be telling us that junk food is good, all his studies in the end are a feast for youtubers make money on videos.
I think that is just a testament to how challenging nutritional research is. Unfortunately we will always have some conflicting day. But the entire literature base does not say that omega 3s are bad… just that like most things we need to consider if we truly need supplements or if diet would suffice. Also I can assure you I have made exactly zero dollars on this!
Omega 3 is good while it is in food you eat. Being extracted and processed it become oxidized and rancid very fast. You can not store it for long time separate from fish. Fish is good, omega 3 pills are not.
Omega 3 is a cholesterol. But anyway meat and fat used to be bad. It was only more than 20 years that the new studies that blamed carbohydrates started appearing on the news wires.

Yup, I’ve been big on eggs, beef, 2 percent milk, and sharp cheddar cheese lately. My lab numbers and vitals are at their best now!

I started eating grass fed , mostly organic, and local free range pasteurized eggs about 25 years ago before it was in style. I knew something was wrong with the food supply even then.

@jrennickemd supplements are all about quality and dosing. Everyone in the industry knows this. 2-3 grams a day of high quality Omega-3s is generally the upper end of dosing. Combine this with a well balanced diet and voila.

Thank you. It was very informative. My outer right hip pain has been consent for 15 yrs or more. Is there a permanent fix? I'm grateful for lidocaine patches but need 3 a day. If I still can't get a doctor to help me, maybe I need to buy a sauna or a ice pool 🤔.
I am so sorry you are dealing with it! And it is possible to get long-term relief, but that involves fixing the underlying problem which is usually a problem with the tendon. I'm 35 years old athletic surfer, with radically solid sleep hygiene. Going on 10+ years of no processed food or added sugar - I only consume berries and sweet potatoes/veggies for carbs in the mornings, no seed oils, quality grass-fed/finished/pasture-raised meat, eggs, dairy, always organic veggies to try and reduce pesticides, etc, lift 6 days a week on a push/pull/legs split, and maintain abs year round. My total cholesterol has been fairly high by traditional standards - anywhere from 240-280 (the highest was at 320 but lowered my saturated fat consumption to try and appease my doctors) - but have consistently low triglycerides with an HDL to triglyceride ratio of 1.3. My LDL ranges from 187 to 252 - currently at 20, and I have an HS CRP of <0.3. My doctor has been trying to push statins constantly and is suggesting my cholesterol is so over the top high that it must be genetic and are considering multiple statins. I'm trying to request both an ultrasound and a coronary calcium scan while they are sending me to UCSF endocrinology and I just can't get on board with this idea that what I'm doing is wrong.
That’s a tough one for sure. By the guidelines you would qualify for a statin, and that is why your doctor is recommending them. This is where we use a shared decision making model. I think it’s super fair to get additional Imaging to help determine your true risk, as more data may be helpful. But also, sometimes genetics just can be a thorn in the side! If you told me the same exact story, but instead of cholesterol it was your Blood pressure, would it change your opinion on medication? All of these things are risk factors, but we all have to decide our comfort level with having “abnormal” levels of anything if that makes sense.
Also it may help to get apoB and Lp(a) to further stratify your risk

My LIPOPROTEIN (a) was <10 and my apoB was very high at 151 H (I can just imagine Peter Attia freaking out over a number this high!). However this was when my total cholesterol was 318 H and I managed to lower that to 240 by my second set of tests but didn't get the advanced lipid panel to recheck my apoB which I assume was lower. I'm very curious, if you had my results but ended up with perfect CAC and CT scores would you take a statin? It's all very confusing for me. I've been following the youtube channels High Intensity Health and Dr. Anthony Jay which have lead me to feel like I am probably alright. Hopefully I'll know more once I get an ultrasound and CCS etc. "If you told me the same exact story, but instead of cholesterol it was your Blood pressure, would it change your opinion on medication?" So full honesty, I've never had issues with blood pressure as it's always been considered within the range of a 'fit person' so I am entirely ignorant when it comes to this question as I just haven't spent the time learning about it. Multiple large trails published in top tier journals find hCRP more predictive of CVD than LDL-c. What we need are similar studies looking at ApoB, which is more predictive than LDL-c. Drug trials are underway to test lowering inflammation/hCRP to see if it impacts endpoints tested. Anyone who doubts the importance to controlling ApoB in reducing CVD is making a mistake they'll pay for some day. But, inflammation can't nor should be ignored and there's ways to do it now with both "natural" and others means now.
Excited to see apoB moving forward becoming more common place. And I like the way you think. It is a holistic view of risk factors, and then coming up with a plan to address those factors as the patient and physician feel fit. We will always have uncertainty, but using multiple inputs will become exceedingly more useful!
this idea bad vs good isn't how this should've ever been looked at when LDL is basically vital fr brain health there are too many factors one of them is good HDL numbers and dld numbers even if LDL is high. I run into research in a hospital taken over a year that patients who experienced a cardio event 78% not only were on statins but had acceptable numbers according to Medical allocations but what they did have in common was calcification. Feels at times like its pill pushing and not actual peer reviews or the latest research
Agreed. “Good” and “bad” cholesterol are too simplified for a complex topic like this.
