What about fluoride bombs? The occlusal surface breaks down late in the game. By the time you see a lucency the lesion is about 2mm deep radiographically and as you mentioned previously, it’s a lot worse when you drill it out.
Yes, until radiograph detects it can be an issue… but don’t forget that clinical lesions can also be seen as underlying shadows as per the ICDAS classification.

How is it not restorable ??
The caries was not only below the gingival level, but also very close to the level of the alveolar crest and below the CEJ. Tooth had also other issues such as mesial bone loss and Caries
Case one ,bone resorbtion and tooth six with a bad restoration, the four with distal caries

Hello, eventually you can use both hands to finish the knot towards outside the mouth, but to begin the procedure it is easier to keep the needle holder in the same position without pulling with the other hand (you can pull it towards outside as well, while the needle holder is not moving). Hello, sorry, not sure what you are talking about. Symptoms are not described here, but I believe you might be talking about the trabecular bone structure at the first molar site, right? The trabecular spaces vary from patient to patient, and it does not necessarily mean alteration ok? Take care

Hi doctor Just following up on case 3. I saw your other comment mentioning that we are only assessing here to see whether the tooth will need RCT or not and that was beautifully explained. If we are to look at overall prognosis of this tooth, would you rather go for EXT given extent of decay close to the bone? The same can be said for #30 as well however #31 prognosis is more unfavorable. Please share your thoughts when you get the chance. Thanks
Hello 👋, your concern is absolutely true… technically if the Caries didn’t pass the level of the alveolar bone crest, one could try to be conservative and attempt to raise the interproximal margins to restore, but in most of these cases, the lesions are way below the gingival tissue level and then if it is not restorable like that, it falls in the inclusion criteria of extraction as you said! 👍🏻

This is very helpful. In your Honest opinion, for someone who wants to get into doing and writing up his own case reports. I apologize as i'm going to need a bit of educating here. Anyone can do case reports right? The big achievement is whether or not they get picked up by a big well known group. I suppose case reports mean nothing to others unless they are published. I only ask because i'm becoming more interested in getting into prosthodontics for the purpose of getting a Masters and doing more research.
Hello, yes, anyone could do it, but what I meant is that you might need a team including at least one who has some knowledge and training in scientific writing to get published in a good journal. Take care

Very good question Veronique, there is the influence of cervical burnout for sure, but I agree that there is that narrow radiolucent horizontal image which is neither the usual presentation of Caries nor burnout. I will try to find some clinical info of that case to check ok

If the buccal nerve is anesthetized but not the Inferior alveolar nerve with conventional IANB, what am I doing wrong? Am I going too anterior or too low?
Maybe both of them, or even more lateral as well… take a look at my video of CBCT anatomy, look the position of the mandibular foramen in relation to the other structures ok? Take care

Yes I agree, but most likely from this radiograph, molars extracted, lesion removed with subsequent bone graft for future dental implants and endo of the 2nd molar should be among the main options…

Hello, I asked the team here and none of us actually have experience with VPT. Would you do it? 🙂
@@understandingdentistry I think it may be a good option, although it is one of those treatments that requires a real time decision (depth of decay, prolonged haemostasis etc). With CSCs you can get great results, but obviously RCT's are very safe and predictable option. So it's difficult to move away from a treatment that is so routine and practiced

Hello, so as you noticed the Caries are now approaching the root area. Although one can try to be conservative and restore, it will be challenging to keep this tooth after removing this root Caries. However this case was used to discuss whether or not this proximity to the pulp chamber would lead to root canal treatment, and it usually does, in cases like that…. Take care
