This glitch has been in the game for months. It happens when you perform a finisher and the enemy is killed before the animation ends. Resulting in you insta killing yourself. So if you attempt a Last Wish Petra or any flawless runs, you now have to add finishers to the list of things not to do.

There's a fairly rare glitch where if you finish an enemy that's already dead (presumably because they were shot before they became immune due to the finisher), the finisher will just kill you instead.
