The "Concentrate" trait mostly interacts with abilities with the "Rage" trait. "Sustain" is more similar to "Concentration" in 5e. With an ability/spell with "sustained" in its Duration you have to spend 1 Action each round to maintain the spell or it's lost. For example, summoning spells where you have to spend 1 Action each round to give your summon 2 Actions. This means you can have as much Sustained spells you are willing to sacrifice Actions for. However, anything in which you give your Action/s to give something else Actions can only be once per round.

Seeing as you're not used to these types of games, I wouldn't take getting stumped too hard. Good news is that Lara usually gives more hints later on when you use survival instincts.
