🥼 What is Degenerative Spinal Stenosis?
🩻 Similar to arthritis in the hips or knees, spinal stenosis is a form of arthritis that affects the joints of the back as part of the natural aging process, leading to disc degeneration and thickening of the ligamentum flavum (a ligament in the back). When these changes occur, they can cause narrowing where the nerves exit the spinal cord and may also narrow the central opening.
There are several treatment options available depending on the severity of the condition.

    I am in the same exact situation. In a few weeks, I am going in to have L3-S1 hardware removed. Since 2005 I’ve had six different surgeries in my lumbar area. I am actually scared that after the hardware is removed that my spine will be unstable. I’m only 41. S1 pedal screws are just floating on both sides. I sure hope my surgeon can relieve the sciatic and nerve pain in my leg. Thanks for the video.

    I've had these the last was a c t milligram sp? I can't move once in that position and going in the tube really freaks me out so I was medicated but I no longer have a dr. For any medications there's no way I could go into that tube. No matter the benefits of the outcome😞

    Thank you for the video I just had this exact same surgery a month ago, it's interesting to see exactly how it's all done. They had to remove my rods and screws because I had to have number 2 and 3 fused.

    My lower lumbar fusion broke. I unfortunately have noticed my belly button has mover I am sure it's to scar tissue. The main leg that goes numb is the right leg they both hurt 24 / 7 I hurt in my dreams. My shoulders are uneven sometime I cannot move my left arm at all and in the shoulder it felt like the " syringe at the dentists pushing the numbing needle in my jaw " but in my shoulder I haven't been able to lift my left arm all the way up to shave my armpit for over 3 years. My upper vertebrae were broken as well I opted out the 3rd surgery alter my fusion broke. The 2nd surgery took 17 hours at harborview and I was told it took that long because of scar tissue. In between surgeries and recouping and going in for extreme pain I was told I was surgically cut through I have before and after X-rays from each surgery but have no idea what or when it happened and wasn't told. I know there has to be am answer some little awesome thing that can be Done to fix this. I do not have a dr at this time I go into e r when I cannot walk. I only have Medicare and I know some where there's an answer that takes my Medical and can fix me. I'm tired of hurting

      A very similar procedure to an ALIF, but it is done from the side of the abdomen instead of through the belly button.

    I'm having a revised discectomy soon, 16 months after the first op. Protrusion of the same offending L5 S1 disc. The pain following last years surgery was easier to deal with than the nerve pain extending down my entire left leg with partial foot numbness, before the surgery. After 3 days of surgery my initial pain subsided, stiffness remained until my physio kicked in. This was and will be open surgery peformed by Orthopaedics, my insurance won't cover neuro surgery.

    If you have facet effusion but facet not super arthrithic can you still do adr lumbar

      Great question! It is possible that ADR could be used, but first the effusion needs to be treated with a facet injection for therapeutic and diagnostic reasons.

      Typically if there are not other posterior issues, the success can be around 85%.

      @@spinecenterorthoatlanta It’s called Nerofibramotosis, (NF1) in my left hip but it goes down to the bone and down my leg, affects my walking quite a bit lol, my mobility, I have almost no flexibility in my left ankle. Since it’s mostly on a patch of nerves it Also causes a bunch of pain if I bump against something or sometimes for no reason at all, I’m starting a medication, gabapentin to hopefully help. They’re doing both legs because I had a new doctor and she wanted to make sure it’s not in both, cuz my right ankles not flexible either lol

      @@spinecenterorthoatlanta dr Chappuis truly changed my life I’m thriving and have an amazing 3 year old daughter now I never imagined being able to go through another pregnancy I was in so much pain but I didn’t take any medicine other than at the hospital thanks to my surgery and the things I learned from my doctor pain medicine isn’t the life you have to live

    What about metal allergies? Germany surgeons published statistics regarding metal implanted allergies. It is my case. I am a mast cells activated syndrome patient, broke my L4 and had a L5 spondilolysthesis. I wasn’t in pain before the surgery. I mentioned my metal allergies to the surgeon, he agreed on using pure titanium. He never told me he didn’t use pure titanium but used Ti/Al/vanadium alloy. I wasn’t getting my post op report, I refused nerves blocks. My anaphylaxis was triggered much more often. As my PT wasn’t helping, I tried high doses of anti histamines and mast cells blockers, it decreased the pain drastically. I made a Melisa test and yes, I am allergic to many many metals. I therefore need a second surgery to replace the material. I find it so bad to talk about a failed spinal surgery syndrome…it is not even mentioned by surgeons. I made my own diagnosis, contacted other surgeons and Allergologist….and they validated my self diagnosis….

    How long is it normal for pain to last after a spinal fusion? In my case L4-S1?

      My daughter mother had it at 15 and she’s now semi pain free at 30 But she also put on 59kg and was super heavily overweight Now that she had a stomach sleeve and lost 74kg she’s doing a lot better with her fusion (she has damaged it from 15 years of weight gain but it’s being managed and she’s recovering from the rectification surgery now and seems to be 80% pain free) But before she got depressed and gain weight 15-18 she was healing really well and had reduced pain He’s was from holes in her spine though not a back injury per say so abit different than most for the reason to have it

      Great question! The pain is relative to each person, but if you are experiencing new pain in any limb that was not present before or pain persisting beyond normal surgery healing (2-3 weeks) then it is best to reach out to your provider to discuss.       Yes, it is best to follow up with a CT scan to get confirmation if the fusion is solid. If the fusion is solid, but screws become loose or painful then they should be removed.

      @@avamolina289 If the fusion isn't solid, there are some options. Bone growth stimulator, an ACDF fusion from the front, or Posterior pedicle screws from the back. It is good to reach out to your provider to discuss all options that fit best for you. You can always set up an e-visit with us or give us a call to explore your options.

    So, if you didn’t have pain in your leg before the fusion and now you do, that’s something that should be addressed with the surgeon?

      Thank you so much for your comment! If there is leg pain that was not present before surgery, it should absolutely be addressed by the surgeon. You will potentially need new imaging and a new exam to locate what is causing the leg pain. Wishing you all the best!

    My lower lumbar fusion broke I can’t stand more than fivemin. My quality of life is affected. I’m a hermit getting out of bed. Every single day is a struggle. I can’t sleep more than three hours at a time. I can’t move when I wake up my other medical issues pretty much all are because of my back. I have no doctor at this time When it gets too much to handle, I go to the hospital. I know there has to be something that can be done there just has to.

      So sorry to hear you are going through that pain! It sounds like you may be experiencing Failed Back Surgery Syndrome. It would be best to see a doctor and get reevaluated with an MRI (or possible CT scan) to see the status of the fusion and create a treatment plan from there.       @@spinecenterorthoatlanta what are evisits? I’ve had the scans even a ctmilogram (sp?) I know there has to be a way to fix this. I’m hopeful there is. I just so wish it was fixed and I was recouping already.     Dr Chappuis I have only just seen this video and for the first time in my life the 30 + years of pain makes some sense. I am 63 have suffered with back pain from my late 20s. Had all the usual operations which culminated with a disc fusion L3, L4, L5 & S1 12/12/22. Brought to another ED 12/23/22 suffering with 40+ temperature and rigors (sent home same night on my own steam). Early hours of 12/26/22 pain became unbearable and only one hospital had the necessary medical specialists available. Operation re-worked as screws installed out of line initially. To add insult to injury, deep tissue samples revealed staphylococcus epidermidis and resulted in an additional three month stay in hospital on daily intravenous antibiotics. It is now 2024 and I still suffer severe pain daily and am waiting on a spinal cord stimulator. As the first specialist who has made sense to me can you explain if this is the correct route to take or should I be looking to have the fusion re-done for the third time.

After that a proper evaluation can be made to see if the issues can be treated surgically with a revision or if surgery is not an option with a spinal cord stimulator.     Ive had three failed back surgeries. The first they put the rods and screws in. A month later just getting out of bed i felt a sharp pain in my back running down my leg. Went for a follow up and one of the screws had broke. Had a second surgery, replaced the broken screw with the same size screw. Had a little pain then pain got worse. Went for another follow up and this time was told the screw was coming out of my back. Had a third surgery and they put in a bigger screw. Now i cant stand upright. I have to walk humped over and im in pain all day. I have to take pills at night just to get some sleep. I had an scs put in my back and that did work for maybe a month now it doesnt help. Ive been through physical thearopy and had the shots in my back and nothing has helped. Im going to a revision Dr. In a coulpe of weeks. Im going to try to get him to remove all of this shit in my back.

      @@spinecenterorthoatlanta well it looks like my revision surgery isn't going to happen. I got a call from nevro yesterday to shut it off so I could get an MRI. I did that . but when I went in today for the MRI I was not told to bring the remote. Since I didn't bring it they refused to do the MRI. After three years of this pain and not getting anywhere ive decided to go ahead and take my life. I know it sounds selfish of me but I can't stand the pain anymore. I can't think straight , I can barely walk , and I can't sleep at night without taking pills. Doctors will not give me anything for the pain. Its been going on for almost four years with no end in sight. So my only option is to stop this pain the only way I know how. I have to wait for my wife to leave the house so I can get to the sleeping pills. Otherwise she will try and stop me. Sorry to everyone....update. My son called me and talked me down. He told me that I have to be here for my grandson and he pointed out how great my wife and I raised him. When he was young he used to get mad at me for working all the time. But now that he's a father he understands. He even got me to laugh a couple of times. Something I havent done in a long time. So I will try again. I can't say what will happen if they throw more roadblocks in my way.

      @@bigbadjohnpesek9894don’t take your life The damage you will cause to your family is irreversible I was in the same place as you with my back 28 unable to walk in agony 24/7 not being able to play with my kids or raise them how I wanted and it hurt so much mentally emotionally and physically It’s a hard road but now I can run hit the gym lost 67kg, I run a company for the nicest boss I’ve ever had who understands my injury and tailored my role around it I earn a great 6 figure wage and spend sooo much time with my kids and reconnected with my ex fiancé and we’re back together Life is hard and it’s never easy but staying around is worth it Those mile stones of your children and grandchildren is something you can never explain It’s the great thing about happiness it doesn’t have to be yours for you to enjoy it There is a saying that helped me through it from my phycologist Society and family grows when men plant trees the shade of which they know they will never sit in She told me of the troubles her patients who had family or a parent take there life have and how even if things are difficult it’s so much better to be there for them even if all you can give them is a loving ear to listen and a cup of tea and company when they need you You have family and you are loved so please don’t harm yourself or your family they will never recover from that pain, and if you need someone to talk to there are always people there for you and who will listen and try to help

    I have Medicaid I would love to find out exact steps to take and I live in another state so seeing what’s available in transportation consultation procedure and transportation home is my goal. I’ve sent info and am excitedly and anxious to get this is motion God bless all of you!

      Great question! Technically, pedicle screws are meant for temporary fixation while a fusion is healing. However, they can become painful or unnecessary and can be removed.       Thank you so much for the comment! Our ORs are used for both spinal and extremity surgeries. A majority of our spinal surgeries are done anterior (or through the front) so a Jackson Table is not required.

    Well done Sir. Thank you for this very informative video. Have always had pain in the lower back after 3 fusions. The surgery was helpful and I'm grateful for the relief but there is still persistent pain. I'm thinking the screws, I have had a recent Cat scan was told the fusions are solid. Going for the top two lumbar fusions later this year,hopefully we can get a better look at the screws.

    My mom had a surgery on her l4 l5 prior to which she had constant severe pain on her left leg .. after this surgery in 2 weeks she has severe pain on her right leg and she could hardly stand up . She's bedridden . Could it be this?

      Failed back surgery syndrome could be a possibility for your mom. Severe pain lasting that long does need to be looked at by a doctor and new imaging of her back might help assess her condition. We do offer free MRI reviews if you have a recent MRI post-op.       Screw move and wiggle, hurt where nerve live. Take screw away and pain no hurty. Got to give it a little bzzzz to make sure first. If bzzzz says no, then we no go. I think.

    How long should residual nerve pain last after lumbar microdiscectomy? Is it normal to still have residual nerve pain 3-6 months after the microdiscectomy surgery?

      Hi David, thank you so much for your questions! It is unusual for nerve pain to last 3-6 months after a microdiscectomy. This indicates that a post-op contrast MRI should be conducted to see if residual compression is present. We do MRIs on site at our Atlanta and Savannah locations, but we can also do e-visits if you send us your imaging. Wishing you all the best on your healing journey!

    Thank you for sharing a great video. Very worried after very bad fall. Very bad button pain groin, throughs can’t walk far doing my head in. Waiting now. Had leminectomy surgery some 50 ago. Fractures of T section from fall also need to lay down most of the time. I have faith in a Nuro I have seen some 4 times now just need to think positive but hard at 72 not young
