Hyperparathyroidism - causes, classification primary/secondary/tertiary, investigation, treatment
**Hyperparathyroidism** is a condition in which the parathyroid glands produce too much parathyroid hormone (PTH). This hormone regulates calcium levels in the blood. There are two main types of hyperparathyroidism: **primary** and **secondary**.
### 1. **Primary Hyperparathyroidism (PHPT)**:
- **Cause**: This occurs when one or more of the parathyroid glands become overactive, often due to a benign tumor (adenoma), hyperplasia, or rarely, parathyroid cancer.
- **Effect**: Excess PTH leads to high calcium levels in the blood (**hypercalcemia**) by increasing calcium release from bones, absorption from the intestines, and reabsorption from the kidneys.
- **Symptoms**:
- Kidney stones
- Bone pain or fractures (due to bone resorption)
- Fatigue, weakness
- Depression, confusion
- Gastrointestinal issues, such as constipation and ulcers.
- **Treatment**: Surgery to remove the overactive gland, medications to control calcium levels, or observation if mild.
### 2. **Secondary Hyperparathyroidism**:
- **Cause**: Occurs due to another condition that lowers calcium levels, causing the parathyroid glands to overproduce PTH to compensate. Common causes include chronic kidney disease (CKD) or vitamin D deficiency.
- **Effect**: The glands enlarge and secrete more PTH in response to low calcium, but the underlying issue (e.g., CKD) often prevents calcium normalization.
- **Symptoms**: Similar to primary but may also include specific complications related to kidney disease (like renal osteodystrophy).
- **Treatment**: Address the underlying cause, such as vitamin D supplementation or managing kidney disease, alongside medications to lower PTH.
### 3. **Tertiary Hyperparathyroidism**:
- Occurs when secondary hyperparathyroidism becomes autonomous and continues even after the original cause (e.g., kidney disease) has been treated, often requiring surgical intervention.

    If I can help one person resolve or manage this horrid condition I'll be delighted. No Doctor could help me, not one had ever seen or treated it before let alone help me treat it. My first Endocrinologist in New Zealand refused to believe I even had it until it became so severe she had to finally acknowledge the fact, and she had no answers for treatment. She simply told me I had to deal with it and I would likely have it for life. Not one for giving up I went on a research mission of my own, I was desperate. This was severe, much worse than the photos in this short video. It affected both legs to just above the mid calf area, it was ruining my life. Thankfully at the time I could still work as a construction worker, I wore grossly oversize boots without the laces done up. I had very severe Graves disease with severe Graves eye disease also, I was a wreck. Now seven years later after a total thyroidectomy and operations to both orbits to remove bone from my eye sockets to save my eyesight, the pretibial myxoedema is 99% resolved. How? Ok, so Im not a doctor of medicine, but I found what worked. I do not offer this as medical advice. Please talk to your doctor first before you follow my lead. Just be warned most doctors know little to nothing about this condition. Some doctors are hesitant to prescribe the ointment I used due to the risk of slow thinning of the skin from long term use. Change doctors like I did, find one who will support you. Be warned this is no quick fix it took me 6 years of real determination to remove this cursed condition and to restore my legs to look normal again. I am an ex bodybuilder, my legs were my finest attribute, I was ashamed of how they looked and it cause me major depression. Now my legs are back, I can wear shorts again and enjoy life. Method: Use a 0.05% - 0.1% max (Betamethasone ointment), not the cream it won't work as it is absorbed to easily by the skin. Coat and I mean coat the effected area with the ointment, put it on thickly, as thick as you can. I used one whole tube on both legs each night after I got home from work.The amount you use will depend on the area affected. But don't spare the stuff it's just a few dollars. Once coated, wash your hands and get some "cling film" I use Glad Wrap. Wrap your legs and feet to cover the ointment, then slip on a long sock or stocking to help keep everything in place overnight. Sure, you'll have stinky feet in the morning when you take it all apart, but do this just before you shower and spare your family or friends. I did this every night for 6 years and it worked.I hope you have the same success.

    Then we did another reshunting and now this time the tubing drained back in to my abdomen and now into part of my heart well after that it was a bunch of malfunctions or infections/ my body rejecting shunts from 16 to 18 I was in and out of the hospital at one point he changed my shunt to a programmable one so he took out the valve on the back right of my head and placed a programable one which goes on the top frontish of the head so since 16 my hairline is so far back smh but today at age 27 turning 28 October 1st I've had 193 brain surgeries in total not counting when they first placed it at ten months but from 14 to 25 193 brain alone 4 on my lung two on my lower intestine and a couple of heart surgeries I have so much scar tissue on my brain if u look at a x-ray picture of my brain it looks like there's spider webs all over my brain I have severe nerve damage and seizures I'm prescribed gabapentin for both and I'm prescribed Suboxone because I'm an addict now because I relied on the percs and morphine and valuims after surgeries and when my shunt malfunctioned before surgeries I almost died a bunch related to this disease I also am not aloud to lift 25 pounds or more because the tubing can be ripped out of my heart I was told i shouldn't be walking or talking that i actually shouldnt be alive but the surgeries and malfuntions caused alot of harm im disabled because of my limits and I have early onset dementia because of it oh i also have a lazy eye now as well cuz damage done to my optical nerve

    I was born with hydrocephulus had my first shunt placed at 10 months a but I was born with brain damage I have severe dyslexia but all was good till I was about 12ish or 13ish I woke up one day and the tubing of the shunt on my neck was bulging then I had extremely severe pain in my abdomen so bad that my mom rushed me to Hershey medical center where my brain surgeon was what happend was the shunt malfunctioned and instead of it draining into my abdomen it was forming pockets of csf on my lower intestine and that caused a cyst to form and burst hence why i was in extreme pain so i got a new shunt and my shunt drained into the lining of my right lung well all was well till a month later im in school and I got a very painful feeling in my right lung that got worse very fast then I just couldn't breath got rushed to Hershey and they pushed my mom out the room numbed where my rib cage is cut dug his finger into my ribs then lung grabbed the end of the tubing and stuck a small one in beside it attached to a syringe and by the time he got all the spinal fluid out my lung there was almost two liters

      I can’t say it can cause it but from experience as a care taker of hospice and patients with that ailment… their stools were NOT greasy, or oily… it was more consistent with new born baby poop but way more foul and darker in cooler than the usual mustard color of baby poop

      Hey, unless the 16yr old is undergoing chemotherapy, this is not normal, and the underlying cause should be investigated so that serious causes are ruled out. -MD in training

    8 months out from a hysterectomy for cancer, as he’s taking a Pap smear, the doctor says, “Whoops, hold still, don’t move, don’t sit up.” The next day, after emergency surgery he shows up in my room and tells me I had a dehiscence and then he says, you be in this business long enough and you’ll see everything.

    It would be great to see treatment protocols for NAFLD, NAFLD with Alpha-1 antitrypsin deficiency, and NAFLD with Alpha-1 AND Hemechromatosis. Many Alpha-1s have NAFLD (even when skinny), but some have NAFLD, Alpha-1, AND Hemechromatosis! How each are treated, how to balance nutrients (especially when the thyroid is affected and chronic pain syndromes, such as CRPS & signs of myleon sheath degradation, similar to MS, is an issue as well. I auspect there are many, many more people out there who have these issues, yet sadly, they aren't treated properly by the medical community, and many die. I myself have seen several. Let's fix that! Certainly, Alpha-1s are the "zebras" in the medical patronage, but the standard accusations of drinking/alcoholism, drug use, etc. and abuse by some medical personnel (such as threatening to run a drug test (blood), then waiting so that the patient has to be drawn again by another phlebotomist, when they've already been drawn over 20 times in the hospitl and had many IVs because of liver failure. This needs to stop! It is understandable to do dur diligence, as sadly, there are people who don't tell the truth, but there are those who do tell the truth and who are often abused by medical personnel. What should they do? Run the tests, but do so with the others that need to be done, be respectful of the patient (regardless), and go by the lab work. Do NOT threaten patients, then have them drawn again just for a drug screen! Do your due diligence and run that test with tylenol, a metabolic panel, liver panel, electrolytes, copper, iron, ferrous, and hereditary Hemochromatosis DNA Analysis, Wilson's disease DNA testing and if not done already, Alpha-1 Antitrypsin deficiency genetic testing, and if positive on any of those, typing. When a patient is in acute liver failure, you and they, need to know why! When you find a single reason, do NOT stop! There may be more than a single cause! Tamiflu has been a culprit in a number of liver failure cases, and if it was prescribed in the weeks before the liver failure, it needs to be reported. In rats, it causes liver failure, too. Order the tests all at once when you first get a patient to minimize excessive venipunctures that can, in those with CRPS, lead to additional problems, such as chronic pain issues. These are paticularly problematic because a person with high ferrous/iron levels shouldn't be taking vitamin C, which usually can prevent CRPS from forming on susceptible individuals. Keep an eye on the iron/ferritin & TIBC levels, and should you need to take a liter off via therapeutic phlebotomy, do so via an existing hep lock instead of sticking the patient again. Get your initial labs drawn and out of the way in your first orders, and order tubes for the secondary tests so you don't have excessive venipunctures. And no matter what you think of a patient... be respectful at all times! Ask anyone who's an Alpha-1, and most, if not all, will tell you that they've had a doctor accuse them of drinking, and usually excessively, and most don't drink at all, as it makes them sick. Our medical community needs to recognize that although there is that saying in med school that says, "Think horses, not zebras", Occam's Razor and all... BUT!!! (And there's a huge, massive but!!!) You are dealing with human beings, and there are many who's health goes totally against Occam's razor, who ARE the "Zebras" of your patients. Ask yourself this... If you were a "zebra" and went to many doctors already who were insensitive, abusive, who didn’t listen to you, who even prescribed you medications that were essentially poisoning you, and you looked for a physician who you wouldn't classify as a "Quack", would you want to be seen by you? How would you want someone to treat you? Sadly, many in the medical profession haven't stopped to think about that, but they should - with each and every patient who either comes to you or who is brought to you. Be that professional who listens, who has compassion, who treats every patient as an individual. Be that professional who treats every single patient as you yourself would want to be treated.
