    I had the surgery done, and probably should have waited as symptoms were mild, I was told the surgery went well and was lucky to have a top surgeon being that Im based in Canada and dont get to choose. That said I'm 2 months in and should have waited. I now have things going on that I didn't have. Lots of pain when peeing as well as rectal pain. Now it seems i have developed a hydrocele which was not there pre surgery. As for the climax scenario also a major let down its about 15% percent of what it was. So unless you really need this surgery HOLD OFF. The HOLEP sell sheet oversells and underdelivers. In addition, i now have to push pee out not so before the operation. SO end of day I ended up with problems that the surgery is supposed to fix. They also put me on Alfuzosin which is something u usually take to avoid or holding off getting the surgery.

      Don, I had mine done 5 days ago. So sorry you are having difficulties. I too have to strain to pee. Do they have any idea what is causing this??? No other issues except the straining.

    FYI ..the sell sheet that ejaculation doesn't change is far from accurate. Big difference and a huge letdown.

    My father is also 74 y/o had aquablation and the pathology shows 3+3 =6 Gleason … can this procedure spread the cancer cells ???

    At minute 18 you compare low volume vs. high volume OS based on CHAARTED and GETUG-AFU15. Please explain the utility or futility of this comparison in the age of PSMA PET. I'm willing to bet that those low volume patients were harboring multiple mets not visible by CT/bone scan. So, were they low volume? These trial results are outdated. Instead, new trials should be started that group men not only by PSMA PET results but also by well-known genomic markers e.g. PTEN, P53, RAS, BRCA, metabolic markers, etc. C'mon, guys, this is 2024.

    I had radiation treatment for prostate cancer. It appears the cancer is gone and my bladder is healed from the radiation treatment. During the healing from 12/15/23 to now (about 5 months) and hospitalized for severe bladder spasms, UTI and bacterial infection. I have had several UTI's, severe bladder spasms. I have an enlarged prostate blocking the urethera. I have had some form of Foley catheter, spanner stent, suprapubic catheter in and out since 12/23. Doctor may I ask a question. Is HOLEP a procedure for me knowing the side effects could be greater due to having the radiation treatments? Thank you if you can answer my question.

    #1 I went to Dollar General . #2 I purchased a lengthy hair tie along with some wooden beads sold in the craft section . #3 I went to Harbor Freight and purchased a 19mm socket . #4 I tied a knot at one end o've the lengthy hair ties . #5 With the aid o've paper clip , I straighten the clip leaving one side like a hook . Utilizing a total o've 3 beads and the hook section o've the clip the lengthy hair tie passes through the first bead . #6 I inserted the 19mm socket followed by 2 additional beads . The second o've the three holds the socket in place , while the third is utilized to tighten around penis near the pelvic area inter changing with the head area . During the initial stage (s) minutes later I experience a sudden and intense warm to hot pulling sensation on my right leg nerve . I remove the mandingo apparatus quickly allowing for the now stretched nerve to adjust , only to reattach it at a later time or day , for this method must be implemented in a conservative manner . Three month into the traditional method , the curve is still present just gain a couple o've centimeters length . While I did experience temporary emotional distress , the peyronies condition never affected my libido . Viewing programs through out , including political talk , everyone seems to grab the microphone directing it to their preferred setting , Hey Girl " YOU " can straighten and talk into the microphone anytime ! Thank you Jehovah God for listening to my prayers over this condition!

    You mention alternatives but you leave out PAE (prostatic artery embolization). You obviously value the MONEY you can make more than making men aware of other very effective treatments. What a disgrace. MONEY, MONEY, MONEY. Why would anyone trust you? All because urologists don’t do PAE.

    I just got my hoLEP done at the VA hospital in Las Vegas. I hope all military vets know the VA health care system does do the hoLEP and can send you out to a location. That does the hoLEP procedure.

    This is an interesting scenario that brings to light one very important thing: if the PSA does not decrease after a resective procedure for BPH (TURP, laser, robotic water jet ablation (RWJA; Aquablation), this warrants further investigation. In the case discussed, the absence of a drop in PSA was an excellent indicator that biopsy needed, and indeed, found a cancer. There will be less of a drop in with procedures that aim to preserve ejaculation like RWJA, ejaculatory-hood preserving PVP, and MIST procedures like Rezum, Urolift, iTIND, and PAE; particularly compared to HoLEP and robotic simple prostatectomy (RASP) that when well-done can achieve PSA decreases to under 1 and with durable responses [but result in retrograde ejaculation, a non-harmful medical condition that many aging men may not be bothered]. Thank you for sharing.

    Had this procedure June 2023 by Dr K. at NMH Chicago. 100% satisfied. Bladder Botox too. 4:55 What a difference in quality of life and sleep. Absolutely no complications. Used pads for about a month, but as each week passed the number of pads used decreased. Botox wears off after about 5 months.

    Why does your Holep Case1 video start with you walking into a surgical room with a female nurse using her cell phone while patient is fully exposed to the room? How do you help prevent unauthorized cell phone pictures from being taken?

    So cancer in two separate lesions in separate areas of the prostate is a no go .

      That is not what I am told. I have two small lesions, we have been surveilling them for the last two years, but a recent biopsy showed that there were some traces of 3+4 in 11 out of 18 sectors in my prostate, not lesions, but positive results in the biopsy. I am scheduled to have Mridian SBRT 5 session photon radiation treatment in about a month from now. I am guessing that they will focus on each of the lesions while doing the treatment, but they will also treat the entire prostate to make sure they eliminate all the cancer.

    Thank you Dr. Krambeck. My husband urology oncologist said that he need to have Holep done before radiation. His PET showed prostate cancer spreading to 8 lymph nodes. Bone scan clear. The doctor claim that removing the prostate via Holep will help more to target lymph nodes during radiation. Do you agree with this doctor about Holep surgery on prostate cancer patient? We chose not to do surgery from the begin because of bad side affects. Quality of life!!! He is on Lupron and Xtandi for two months, PSA is 27. It was 80 before hormones therapy. The prostate is getting smaller. Please help what should we do? We are very confused with plan of treatments from this new doctor. We don’t want to have surgery if we don’t need it base on all your video talk about Holep that I watched.

    My PSA was rising for several years topping out at 6.35 at age 52, I went to several urologists who screamed at me there was no way to lower my PSA and that I needed a biopsy! I started fasting and KETO and I’d always exercised my entire life, FF 2 years my PSA dropped to 3.0! Now at age 57 it’s down to 2.9! I went and saw my GP and asked why my PSA had gone down and she ran out of the room!

      @@jeffmorgan5152 I’ve always exercised and have been in great health, I knew that my eating was atrocious and it was catching up with me plus as I got older I kept thinking, you know what I do not need this much food especially sugar, carbs, highly processed foods, eating/snacking all day was effecting me. I knew it! So started listening to the Dr Berg, Dr Schwartz, this fasting center in Costa Rica the doctors explained about chronic disease caused by excessive inflammation, the PSA is an inflammation marker so I changed my diet. I’m now day 12 OMAD/carnivore, while I had a ton of great results with KETO I got sucked into the KETO snacks and while some were healthy my weight had plateau’ed. Good health is dictated by your next plate of food.

    thanks; I have had chronic inflamation of my bowel, particularly colon, throughou my life and I developed prostatitis quite early (24 years old or so); eliminating dairy and sugar fixed at least 60% of it; eating very few or none green leafs was also beneficial; stress impacts it too (I´ve been high in anxiety all of my life). Not doing excersice is also bad for it. I will now try to eliminate any type of bread or flour. Finally, I will eliminate yerba mate, which has caffeine, at least for some time. Medical doctors don`t pay ANY attention to the most important variable in health: nutrition, food allergies, intolerances, etc. which is BAFFLING
