At 17:09 when you apply clamp through the plate after introducing and locking the butress screw - doesn´t it cause tension in screw - plate junction? Also i would be concerned that it could pivot on the screw and as the proximal compression is achieved distal part of the plate would angle away from the bone. We usually fix the plate first with conventional screw i.e non locking so it compreses the plate to the bone wich also helps to narrow the fracture so tibial condyle is in line with femoral one. Or is the butress screw non locking?

Quel instructions parlez vous Je ette opérer l'année dernière ily à 1 ans avec la même fracture j'ai de métal l'opération ce déroule avec une fixation je passe 3 mois dans une hôpital pour rééducation même aujourd'hui j'arrive pas marcher correctement je marcher avec deux béquille difficilement je mal encore le métal ça me commence me gêne alors l'opération es fixé pour 3 juin pour sortir le métal Bonne guérison pour vous

how much time remove screw
Typically hardware removal is not necessary. However, if symptomatic 1 year from surgery then it is an option, but that is a minority of patients.

I have a FNSF right now I was suggested surgery, I do not have any family to take care of me here, they are going to put 3 screws into and ill be toe touch weight bearing. Should I go home to do this surgery so I have family to take care of me or should I do surgery here in Kansas where I don't have a car to go to a clinic and I have to ask for a ride or call a uber. Another thing if a surgery goes bad like with the man in the motorcycle accident, is this the patients fault or just the injury not healing?

This is one of the most informative presentation on femoral neck fracture on UA-cam. This and some other presentations indicate that the angle of the fracture is the determining factor of non-union. The degree of displacement is the determining factor of AVN. Surgery damage should not be overlooked. Quality of the imaging is very important to minimize surgery damage to blood supply to the femoral head and achieve anatomic reduction.

That was some incredible honesty. I wish my surgeon would have been more open and upfront. Telling your patient with a garden grade 3/4, pauwels 2 that they have 90%+ chance of success is comical. I’ve been unable to find that level of expected outcome in *any* literature.

Have the same fracture but did not opt for surgery, in a boot for 6 weeks and was given ok to weight bear after, 4 weeks on did xray and ortho coulnt see any healing, we did CT 2 days ago and it was confirmed it is non union, so now need to do the ORIF with autograph. Its like going back to square one. 😭

I was in a head on collision back in March of this year 2022 I have a pilon fracture / compand fractured broke my tibula and fibula had that apex thing on my leg from the 10 to the 18 when they did the surgery still can't put any weight on it . And it's slowly healing . What else can I do to get it to heal . I do not want an amputation and that's what they are considering now please help thank you

In my practice I have not seen any benefit to using a drain in this procedure. I generally only use them for cases involving a tenuous flap and risk for hematoma (e.g. extensile lateral for calcaneus), especially in the extremities where a circumferential dressing can be used. The literature has generally shown no benefit to drains except for increased need for blood transfusion. They are also uncomfortable for patients and frequently prolong the hospital stay un-necessarily.
