Invaluable clinical experience sharing by Dr Muralidhar Sharma sir, the RISHI🙇‍♂ of Ayurvedic practice Guru ji our Guruji- Dr Ravishankar Pervaje sir APPROACH TO AYURVEDA -PART 1 INTRODUCTION DIE EMPTY by Todd Henry is one the books that I read recently .I recommend this book to all. The core concept of this book is that before one dies, before he goes to the grave, all achievements and experiences have to be returned to the society and this is a very impressive message. “DONT GO TO YOUR GRAVE WITH YOUR BEST WORK INSIDE OF YOU . CHOOSE TO DIE EMPTY“ When my retirement was nearing I had a plan of writing a book about what I gained in terms of clinical experience and Ayurveda clinical knowledge. But unfortunately for about a year after the retirement due to some unexpected commitments,I couldn't really start with that.I couldn't really focus on it . And I'm highly grateful to Doctor Ravishankar Pervaje who gave me the idea to speak about my experiences. Speaking is very easy as I have been in the teaching profession all throughout . So speaking out all the knowledge gained is a great opportunity . So it's my selfish idea that I chose this option of presenting these ideas in front of you. So I would expect your comments and your feedback so that I can improve . When I started my career as a teacher and then a practitioner there were some controversies arising. My specialization is surgery and I had to start with the surgery facility in the hospital and most importantly I had to maintain my identity as an Ayurvedic teacher and there was a need to show a feasible working model to the students. It's a commitment to show a model to the students and one of the possible models I selected was presenting a viable clinical practice model based☝🏼 only upon Ayurvedic principles and basics of Ayurveda. So I started my clinic in Udupi on Bharani nakshatra and it was run in such a way that the above model was put into practice . I placed a board and then without any publicity, it started and somehow ,I was lucky that it was a grand success. Studying Ayurveda for exams and learning to practice Ayurveda is very different . My learning of Ayurveda happened through teaching it to students . Once we conducted a medical camp in Eloor ,internees prepared Ayurvedic medicines by themselves and gave them to patients in the camp . I was just a motivating force among them all the work were done by my internees and that was one of the opportunities where I could get some different insight into the area. It was a game changer in my career. So I had to present a viable model of Ayurvedic clinical practice , sanctity has to be maintained too . I started my clinical practice in 1988 and till now have not given any Allopathic prescription nor dispensed any . So it is possible☝🏼 to do 100 %🏅 successful Ayurvedic practice . During the course of 30 plus years of practice , naturally we develop some extra subtle knowledge that is beyond☝🏼 books So all my efforts would be to convert that subtle whatever extra which I have gained into a presentable form. Something that is unique in my practice is that the number of drugs used in prescription is very limited. Out of the 47 drugs I use, 45 are classical medicines and there are 2 patent preparations ie Manasa and Diabecon. These are the 2 patent preparations which I have used in my practice. In BAMS curriculum you will be exposed to about 64000 yogas and naturally, there will be a confusion which of the yogas to be prescribed in which context and so on and everyone would have their own choice of prescription and many a times, patients also will be confused, that which medicine is better and so on and so forth. In addition to those 64000 classical yogas, we have innumerable patent preparations by different pharmacies. So one of the major drawback is that when you go into practice you do not have a clear cut guideline or limited choice of medicines. If you go on using different, large number of medicines, in larger number of patients maybe you may not get in-depth intuitive knowledge . Intuitive knowledge cannot ☝🏼be gained unless you use the same medicine or maybe a simpler number of medicines in larger number of people. So my number of drugs/medicines which I have used is limited based upon basic principle of using lesser number of drugs in larger number of people so that you will have some intuitive perception ☝🏼beyond what the textbook says and that idea of using limited number of medicines is not only my idea. It is a part of the national health policy, the essential drug list released by ministry in 2016 and 2017. It contains 274 medicines and again, out of 274, almost 99 percent are the classical medicines. Maybe there comes a day when we'll be forced to use only those medicines in the coming 10 - 15 years . We never know what is going to happen . Hence, we need to think about such possibilities of using limited drugs in practice. Of course, When I selected these drugs again, it was, not a random choice. The choice is based upon cost efficacy. Dear Sir ,your understanding,observation and presentation of Ayurveda especially Tridosha is incomplete and immature. It doesn’t mean the rest of the Ayurvedists understand well.All most all don’t have crystal clear understanding of Ayurveda,that’s why we are failing to educate the students properly,understanding the disease properly ,treating the disease properly and unable to manage the complications occurring during treatment. Tridosha theory holds good for Nija diseases.It doesn’t hold good for many diseases explained by Susruta in 7 kinds of diseases classification.Ayurvedists try to understand ayurveda on the lines of modern system,this is a big mistake and you will reach nowhere.understand ayurveda by Ayurvedic principles only but we failed in this angle.I agree with you in respect of Anatomy where it is incomplete to understand and practice.Try to understand Tridosha at physiological,pathological and treatment level either by prospective or retrospective methods.Research to understand disease by Tridosha and treatment as per Rasa permutation and combination as mentioned in Ayurveda,then you can arrive a logical conclusion other wise we will be in this confusion state forever.
