Dapat meron tayong CSO Application Form, BIR Registration, Business Permit, 3 yr Financial Statement audited by external auditor, good standing certification from Mayor, Good Standing Certification from other Govt Agencies, Omnibus Sworn Statement, for more information kindly contact your DA Regional Office CSO ACCREDITATION OFFICE.. or subscribe to this channel for more information...

Kung papalitan nyo ang pangalan ng inyong organization name, maaaring kayo ay mgbabago or mgkakaroon ng amendment sa bylaws at iba pang kaukulang dokumento para mkpag comply sa mga proceso. Ang validity ng CSO ay limang(5) taon kung at kailangan din etong i renew
