Yeah, Malkavian player characters in the TTRPG can be completely unhinged on another level. There are certain states of dementation that they can arrive at through their madness, and most of them end up being in states referred to as “Maniac” or “Fool” for their entire vampire existence; which essentially means that they are permanently in that early stage of spasms, strokes, and frequent inability to control their thoughts and actions all while being entirely disconnected from reality- or perhaps more connected than everyone else. They’re completely unpredictable when played right; you could be seeing events that haven’t happened yet and hearing whispers that might reveal secret intentions, but you just can’t for the life of you understand what a fork is and that stupid newscaster on the television won’t shut up about his favorite way to cook a…oh…the television isn’t on…well, good for him, I could see the blood pushing through his veins… Anyway, what where we talking about? Oh, yeah! The Tremere! They’re pretty great…were pretty great.

From Clan Book Malkavian Revised " He could eat the name of a person, and that person would falter and die, and everything that lived in that person he would digest and incorporate into himself" He still existed after consuming his name, I guess if you digest yourself you just become yourself again. for your other question, the way it's written he eats the concept and the "power" of the word out of reality and into himself. You're right, a lot of the time ghouls are depicted as some level of drug addict. Their is additional fluff about a few of the clans vitae (like Setites being particularly addictive for example). I think the allusion to PCP is to paint the image of an urban gang banger that shrugs off taser shots.

a Malkavians stage of development isn't tied to how long they have been alive, for example most get stuck as a "fool' or a 'maniac". Which basically means they are batshit crazy their whole second life. this comes from old word of darkness lore ( 1997 Clanbook Malkavian). thanks for the comment
