Nugget is grooming and cleaning the pup. Lots of animals' maternal/paternal instincts are so strong that they will automatically go into "surrogate parent" mode if they encounter a young animal in a neutral setting, no matter any difference in species or size. There are a number of videos showing this online, with small animals raising large ones, cats or dogs raising chicks or baby rodents; even saw one with a big cat (can't remember what species) who tried to take in the foal of an antelope its pack had killed. This behavior is most often seen in carnivores and rodents (domestic or wild), as those are the most common types of communal animals humans will interact with. All of the actions you see Nugget doing are actions a mother (or father) squirrel would do to an infant squirrel to clean and groom him, including cleaning "down there", since most young mammals that are brought up in dens spend the first few days or weeks of their lives (usually corresponding to the period in which their eyes and ears are closed) unable to defecate without stimulus from the mother, to keep them from randomly dirtying up the den.

Squirrels tends to destroy things they're teeth can cut through a walnut and she's agitating that puppy while he's trying to rest and Squirrels play all the time and need special attention and affection if you're going to try and have one for a pet and they're very destructive. But that puppy is not going to play as much as that squirrel.
