    Amazing skills…is this performed by ECMO RNs or is this skill limited to Physicians and APPs such as yourself?

    I love the locking needles. That’s what I use for my port but I don’t know what brand mine is. I’d have to look. But, I also go straight in. Someone told me a different way and the way they did it HURT, even with numbing cream I put on. I go straight in and it just hurts like a pin prick. I don’t always use numbing cream. Thank you that I’ve done it right where there is very little pain.

    Thank you for the demo, and for your time and efforts. The silicone model is nice. Congrats. With some kind of background material of a different color (e.g. blue), the whole procedure would have been easier to follow.

      There are different types/brands of Port needles and some are safety needles. You anchor/ hold port under skin while you de-access needle by physicallly pulling needle straight out away from chest/Port.

      "The beveled tip of a Huber needle will not remove a core of silicone from your port-this prevents a chunk of silicone or skin from lodging in your catheter line (an all-too-common complication) and makes your port last longer"

    Why use the mid and thumb finger for holding the catheter , why not the index and thumb

    • @michaelpickettaprnmsncpnp4435
      @michaelpickettaprnmsncpnp4435 2 роки тому

      Hi...the video shows both techniques. One way is to use mid and thumb finger to hold cath, then with same hand use index finger to thread catheter sheath off the needle. If you use your thumb and index finger to hold cath you can use the other hand to thread catheter off needle. It's really up to you what you like.

    Shouldn't you have lifted the lines and cleaned under them? I know you were not using a securement device, but it should still be cleaned.

    A few things I noticed done incorrectly. You applied sterile gloves at the beginning of the procedure, prior to removing the old dressing. You basically contaminated your sterile gloves by removing the old dressing with your sterile gloves, and then going to your sterile field to get everything for cleaning. I usually use clean, non sterile gloves when removing the old dressing. After removing the old dressing, I remove the clean gloves, sanitize my hands, and then don my sterile gloves for cleaning site, so that I don’t contaminate my sterile field. Secondly, you did not apply a securement device to the site which adds the risk of migration. Per INS guidelines, the dressing you applied isn’t a form of a securement device such as a stat lock or picc cvc securement device, even though the manufacturer states so. INS guidelines don’t recommend this practice. Also a quick tip, to reduce the risk of migration when removing a CHG dressing, squirt a couple drops of sterile ns from a flush, and the gel comes right off eliminating the risk of line migration. All this could technically be done one person but two person is better if there is availability.

      Hi Melissa- thank you so much for watching the CVC dressing change video and for taking the time to provide us with feedback. At the institution this video was filmed at they require sterile gloves be applied, all packages on the sterile field be opened and prepared (so there is no delay while the line is open to air), then they require removal of the dressing with those sterile gloves. Once the dressing is removed- the nurse removes his sterile gloves at 5:45 min in this video, performs hand hygiene and dons a new pair of sterile gloves. We don't demonstrate the donning of the second pair of gloves in the video in the interest of time. You are correct- the nurse in this demonstration did not apply a securement device. In this video we were demonstrating a dressing change on a line that did not require a securement device such as a stat lock (this institution uses other methods of securement on some lines). I really appreciate you sharing the tip on the CHG dressing with me and for taking the time to watch this demonstration and provide feedback. I will be sure to pass all of this advice on to the specialized central line team at this organization. Thanks again!

    🙌🏼🙌🏼🙌🏼🙌🏼 I’ve been having trouble with IV insertion since everything is “seen” or “done” online. I work as a PCT and inserting an IV catheter or needles is a delicate process.

      Hi Nicole...some of the other videos in our PIV playlist ua-cam.com/play/PLvWZmgauEkKKDoEUquReYuzhDRsAFzVHN.html should help as well. PIV insertion basics is 9 min long and filmed during a training session, there are others on PIV securement/pressure ulcer prevention and using IV Cath to draw blood in tiny veins. Best regards, Micheal

    I keep getting a flash but the catheter doesn’t advance. Why is this?

    • @michaelpickettaprnmsncpnp4435
      @michaelpickettaprnmsncpnp4435 3 роки тому

      A few thoughts, when you take the catheter out of package, loosen catheter off of needle, then reset catheter before starting IV.....could be the IV catheter itself and how you are using it when threading, or could be catheter is no fully in vessel as shown on last part of video. ***Recommend using a IV trainer to practice, especially when have new IV catheters. Here is a link to the IV trainer we use www.sawbones.com/catalogsearch/result/?product_list_order=sku&product_list_dir=asc&q=redistik+wearable Please reference another video in our playlist titled PIV insertion basics(TCH/Global HOPE). We filmed during a training session. It is longer (9min) but has good teaching points that should help. ua-cam.com/play/PLvWZmgauEkKKDoEUquReYuzhDRsAFzVHN.html Best regards Michael
