Complete a Consultation and Health Appraisal
Here's a full Consultation and Health Appraisal for your new client. So let's get started.
Step 4. Evaluate clients for coronary risk factors, diagnosed disease, and lifestyle.
This evaluation helps you stratify risk and refer clients to physicians as necessary. The charts and tables you'll need are in the description links.
The key areas to evaluate include positive risk factors associated with coronary artery disease (CAD), medical conditions and diagnosed disease, and current lifestyle.
Let's briefly discuss each one.
First, we want to evaluate Coronary Artery Disease Risk Factors.
It is necessary to evaluate positive risk factors associated with Coronary Artery Disease Risk Factors (also known as CAD) in order to identify individuals who may be at higher risk during exercise.
Next, we need to identify Medical Conditions and Diagnosed Diseases.
It's critical to identify signs and symptoms of various chronic cardiovascular, pulmonary, metabolic, and orthopedic diseases.
Finally, we should evaluate current lifestyles.
You should Identify your clients behavioral patterns such as: Dietary intake, physical activity, and stress management. This info helps you assess potential health risks associated with your clients current lifestyle.
After this step, you'll have a complete evaluation of your clients coronary risk factors, diagnosed disease, and lifestyle.
Important note: The charts and tables you'll need are in the description links. Once evaluation is completed you can move to step 5.
Assess and interpret results.
The next major step in the screening process is to identify individuals who may be at increased risk and to stratify that risk.
Use the par q form and the initial risk stratification method from the previous step to identify people who have a greater potential for risk and may require referral and clearance from a physician.
You can download the par q form and the table you'll need to complete this step by clicking the description links.
After you assess and interpret your clients results, you can move to the next step.
Step 6. Refer your client to a health professional when necessary.
This step is intended to help identify individuals who will need a referral to a health care professional for medical clearance prior to participating in activity.
Here’s the criterias to determine whether you should refer your client or not:
If your client is low risk,
It is not necessary to have a current medical examination and an exercise test prior to participation in moderate and vigorous exercise. It is also not necessary for a physician to supervise a submaximal or maximal exercise test.
If your client is moderate risk,
It is not necessary to have a current medical examination and exercise test for moderate exercise; however, these are recommended for vigorous exercise. It is not necessary for a physician to supervise a submaximal exercise test, but physician supervision is recommended for a maximal test.
If your client is high risk,
It is recommended that a current medical examination and exercise test be performed prior to moderate or vigorous exercise, and it is recommended that a physician supervise either a submaximal or a maximal exercise test.
Once you determine your clients risk, you can move to the final step.
Step 7. Obtain medical clearance and program recommendations if needed
If medical clearance is recommended, you should give the client a physician’s referral form in order to obtain the necessary information about health status, physical limitations, and restrictions that would be required to make future fitness program recommendations.
The physician recommendations provide you with guidance and directions regarding what specific concerns and needs your client has and which programs are appropriate.

After you finish this step, you would have completed a full Consultation and Health Appraisal for your new client.

      Her heart rate in calm state u take it after u wake up in the morning u go to toilet if you Need then u stay calm at your place and u count ur pulse in 6 seconds than multiply it to 10 u get 60

    Can i ask why you use 0.75? Some poeple use 0.7 some use 0.6 and i dnt understand why it differ

      The 0.75 is an arbitrary number for intensity of your workout. If you use 0.6 it will be an easy run, also 0.7 fits into that zone. 0.75 is pushing toward a lactate threshold intensity.

    130 is Heart Rate Reserve (HRR) not RHR (Resting Heart Rate) and 65 is supposed to be 60. lol.

    You're supposed to use a platform 16.5 in tall not 12 in. And you measure your heart rate immediately after for 15 seconds (x4) not the full minute.

    hello, may I ask you a question please? Where do you get maix intensity % and What is 65? form HRR intensity? 40-59% 60-89% 90%and above? if yes then where do you get the 75%? Sorry if it's a stupid question. 🙈

      The 65 is when you multiply the 130 x .75 = 65. The 75% is just an intensity level you pick.

      If you mean resting heart rate its 60 seconds or 1 minute. You can also get your pulse for 10 seconds and multiply by 6.
