      @@kathyfullarton8126 there’s no right answer to this! Ideally you would want to keep your patient within 20% of their normal blood pressure eg if starting systolic is 100 you might want to aim between 80-120. However, blood pressure change intraoperatively is also multifactorial eg. Depth of anaesthesia, haemorrhage, surgical stimulus, hypovolaemia, venous pooling. Younger and more fitter patients are also more tolerant to blood pressure changes compared to the elderly! And that’s the beauty of anaesthesia. Every patient is different, and as long as you can justify the need for administering medications to facilitate surgery/recuscitation of a critically unwell patient

    Nice overview and technique. When do you tend to use local? For cannulas above a certain gauge. for certain patients, etc?

      Hi Armuk! I tend to use local depending on the situation. If you’re planning to put a big one in eg a 16G most of the time local will be helpful. Also if I’m planning to use ultrasound for cannulation, putting local in before helps a lot as patients would tend to move less as they are less sore which gives you better patient cooperation. Do take into consideration that putting local in sometimes means you will be needling patient twice and putting local under skin would sometimes affect visualization of your vein

      @@DrYeoYo Thanks for the reply. Yeah that's my concern, that the local may impair my chances of successfully cannulating the vessel. Still, for 16G+ it seems reasonable to use

Heard from someone that they're not very similar, which made me question their usefulness for exam Looking forward to also watching your OSCE/viva revision videos after I hopefully pass the written hah

      @@armuk i used the BMJ question bank more of a revision! Or when you feel tired of reading and studying the books it provides another avenue for mental stimulation (: it’s not exactly similar to the exam but I found it really useful in identifying knowledge gaps!

    Hello Nicholas, I have heard that for IMG's they are holding off Primary FRCA exams until 2025 ? Do you have any information in this regard ?

      Hello! Sorry I’m not well aware of this. However I do know that recently IMGs have not been getting a spot in the exams due to overwhelming applicants. And the college prioritizes their own trainees as it’s a requirement to progress in training!

    Hi, thanks for the video! Just wondering whether you'd recommend starting with the e-La to get a broad overview of the basics in each of physics, pharmacology and physiology before diving into the core textbooks or whether you think it's best to just get started with the core textbooks as I imagine that will take quite some time.

      Hi! I've not fully done the e-LA modules so I can't really comment on that. However, what I would say is that you could probably use the e-La modules as just reading material on your free time eg. on the bus on the way home or something to supplement knowledge from the core textbooks! But ultimately I feel the core text books gives you good enough content for the exams by itself

      Hi! I’m not too entirely sure. Most of the OSCE courses are conducted regionally because it allows practical exposure and experience. I hope this helps! I’ve not came across an online FRCA OSCE course

      OFC! If you have an interest in dermatology and have portfolio evidence it think it would be a good point bringing up. It would show the examiners you have a career plan

      Hi Peshin, , I'm not the best to advice on this as I've did my medical school in the UK. However, the BMJ has guidelines on it www.bmj.com/careers/article/a-guide-to-img-applications-for-specialty-training-in-the-uk. You can post your question on www.reddit.com/r/JuniorDoctorsUK/. There are a lot of doctors there that can give you a better advice! I'm sorry I'm not the best to answer this question for you but I hope these 2 alternatives will give you what you need!

      Hi Namit! I’m sorry I don’t have the numbers for radiology. However, take note that radiology is more competitive than anesthesia so you would probably need to score better for entry

    yeah for me, my veins "see" the needle coming and dive then roll, I get stuck 2-4 times before they finally get one in and it hurts, I stress out to the point of triggering my own charlie horses, severe anxiety, and I start shaking, I might be having a upcoming surgery on my right foot again, and the Anesthesia Dr scares the living crap out of me, every time they've come in my room prior to surgery to talk to me, I literally jump and trigger charlie horses, and spend the next 15 min shaking

      Awk I’m sorry you had an unpleasant experience! Sometimes what we do is have one of us talk to the patient while the needle goes in to distract. But I get what you mean, I occasionally encounter veins that literally shrivel up the moment the needle touches it 😅
