Chiari Check - diagnostic tool for Chiari-pain and syringomyelia
Chiari Check is questionnaire based diagnostic tool which gives a risk of Chiari-pain and syringomyelia. It is intended as a low cost tool to help caregivers and veterinary surgeons decide on the risk of disease before referral for MRI or when MRI is not available. It also also help monitoring progression and response to treatment. It works by asking a series of questions then generating a score. This short video explains the tool

    Hi Professor! Thanks for such an informative video. I was searching everywhere for some information regarding Pregabalin dosage for cats with FHS. We transitioned from Gabapentin at 100mg twice daily several months ago as my cat started refusing to take the meds in her food. We commenced Pregabalin at 25mg twice daily and in fact, her symptoms seemed improved. That was until the last week or so when she started tail biting/attacking again. I called my vet today who said to increase her dose to a total of 75mg daily (either 25mg x 3 or 3 capsules split into two doses). I’m concerned that this is too high a dose? She weighs around 6.3kg (gained weight since starting Pregabalin 🫣) I did give her a second 25mg dose today around 8hrs after the first with a plan to give her a third one before bed, however the second one really made her quite sedated/sleepy? Is this normal? She was never this quiet with the 2 x 25mg daily given roughly 12hrs apart. Thanks so much for your invaluable advice 😻

    Hi we have a 9 year old Chihuahua that was diagnosed by MRI having Chiari malformation. The doctor has advised against surgery and our dog is now on medication since 5 weeks. The medication is 1x 5 mg dermipred, 1x 5mg Losec-mups, and 2x 25mg Lyrica a day. In the beginning of the treatment we saw a big improvement in his condition when dermipred 5 mg was given. His condition was the he lost like 70% of his strength from one minute to the other and could barely stand on his feet, Falling regularly on his face while walking. Since dermipred i would say his strenght is now at 50% but he is not getting stronger. We also tried 3x gabapentine instead of the 2x 25mg Lyrica when we started the treatment but it seems that while his condition improves, it does not seem to be enough to get him trough a full day without pain. I am reaching out to everyone who has expertise on this terrible disease to get as much information on how to improve his life. All information is very welcome thank you.

    My dog has been diagnosed with CM/SM, and every time after she eats, she runs to my bed and energetically rubs her head, then burrows under the blankets or pillows. In the end, she sticks her head back out.Before the diagnosis, I always thought she was just having zoomies because she enjoyed her food. Now I constantly wonder if she's in pain after eating since she only does this after meals. How can I tell if eating is causing her pain? Is that possible, and if so, why?

    Thank you, Claire -- very nice summary. I will add that over the last 14 years we have not had a repeat surgery. I do exactly what you illustrated here (FMD, duroplasty, and SIS). I suture the SIS over the duroplasty defect and then place an autologous fat graft over the site. Like you, we don't have all the follow-up we would like, but in a coupe of cases, the repeat MRI at 3 years has shown the fat graft in place. Significant reduction in the syrinx, as you have said, is uncommon, but improvement is not uncommon -- at least on all of our follow-ups, we have always seen improvement -- usually with medications, sometimes not. I will say that we have seen a reduction in the syrinx in patients we perform this at less than 2 years of age. Many thanks! Andy

    Thank you so much for this information. My 6 year old Cavalier 11 kilos developed SM about a year ago, we don’t have MRI available in Tasmania and it would be very expensive and not worth doing unless you were having the surgery done. Willow has been on Gaberpentin 100 mg and ended up having 5 or so capsules a day for about 10 months. Another vet recently changed her to Pregabalin 25 mg capsules 2 twelve hours apart, as she had some breakthroughs we give 2 at night and 3 spread out through the day but Im wondering if they are better given two together maybe 3 time in 24 hours. She dosen’t yelp in pain but does head rubbing body twisting and some scratching. Still likes her walks etc and quite bright but yes has put on a lot of weight, doesn’t have heart trouble at the moment. Unfortunately my vet has not had a lot odd cases of SM so prefers if I do my own research. Thank you Liz.

      I am afraid that I cannot give advice on medication to individual patients that are not under my care although I get that you have been told to do own research. However your vet is able to contact me for (brief) advice though the usual channels (for vets). I will say that the dose of pregabalin is typically half that of the existing gabapentin dose although MAY be able to give less frequently ie 150mg gabapentin three times daily = 75mg pregabalin twice or three times daily.

    I want to thank you so much for this beautiful presentation... I am so grateful that one of you genius' has found a way to spell this out in laymen's terms..... I am currently fighting off any signs of suffering in my 13 year old Cav with gabapentine prednisone, prilosec, and CBD. As well as treating her heart murmur with cardiac meds.... I refuse to allow her or any of my sweet pups suffer but the comfort measures can only take us so far before we need to make a hard decision...

    Such a helpful and interesting video! Do you think that dogs with idiopathic epilepsy who used to have seizures no more frequently than once a month and who have now put on excessive weight due to Phenobarbital should be switched to Imepitoin? A lot of vets who put epileptic dogs on Phenobarbital don't ever bother monitoring their blood or checking their livers, so maybe it would be a safer option too in that case?

      Phenobarbital is more effective than imepitoin and there is a huge risk in switching a stable dog Having (sadly) seem a lot of cases where it went badly wrong I would not undertake such a switch lightly. However I would try a dog that was having seizures less frequently than every 2 weeks on Imepitoin as a first line therapy (ie before phenobarbital) I am so glad that you found the video helpful

    Hello doctor, thank you very much for this video. My dog, a 10 year 7 month old golden retriever, was treated with radiation therapy treatment (for brain meningioma) 4 months ago, and she has been on a mild dose of pregabalin 100 mg 2x 1 tablet daily (3 mg /kg twice daily) for the last 4 months. Recently, we observed a strange head twitching for half an hour; during this time, she was completely active and responsive. Could you please advise if it could be a side effect of pregabalin that showed as a myoclonus-type seizure? If Pregabalin is tapered down and eventually stopped, will it help her? The other anti-epileptic drug my dog takes is Levetiracetam 500 mg 3x 2 tablets daily. Please suggest. Thank you very much.

      I am afraid that our Royal College of Veterinary Surgeons ethical guidelines (quite rightly) prevent me from making comments on medical management of patients that are not under my care so I can only refer you to your veterinary neurologist to discuss this. This because i cannot safely comment without knowing more about the dogs medical history. Yes pregabalin can very rarely be associated with movement disorders but there are many many other explanation including a benign idiopathic head tremor so speak to your vet. I wish you and her all the best

      I have not had any problems giving specific canine and feline joint collagen nutraceuticals (available in the UK )with pregabalin. obviously the ones that I advise for my clients in in the UK are ones that have a higher level of science to back them up (rather than ones intended for humans etc)

    Thank you for the informative video! My doxie was recently diagnosed with lvl 2 IVDD. Took MRIs and decided to go the conservative route since her pain was manageable with medication and the hernia was relatively small. I’m already seeing a huge improvement in her walking after 5 days. Will be starting physiotherapy and hydrotherapy tomorrow 😊 Biggest difficulty this far has been keeping her resting, as she’s full of energy now that we’ve found effective pain management!

      She’s on robenacoxib and gabapentin. This combo seems to work really well and I haven’t observed any side effects beyond a bit of loopiness the first day on gabapentin.

    Thanks for this. 8 years ago I had a cavalier with chiari malformation and SM, and gabapentin seemed to keep her mostly comfortable for a few years and then she got more painful episodes so I changed to pregabalin. The painful episodes seemed less frequent but were still intense. They occurred out of the blue but were worse with excitement or if she was in the hot sun. One bad one occurred after someone left salty crisps lying about which she ate. I am convinced this induced a very painful episode for her. At these times she would scratch intensely at the back of her head and then collapse on the floor scratching and then he body seemed to go into a spasm as if in pain, but she was still aware and awake. They tended to last a minute or so and then she got up as if she was tired. I tried many other drugs for her as well over the years but the pregabalin seemed most effective.

    Hello, i just found yourbpage today, April 29, 2024. Is there anyway you could contact me?? I am in dire straights. I need advice and help. I took my Cavalier to a Neurologist and she put him on Prednisone along with other drugs. He has been on them for a while now at least 2 yrs. He just turned 6yrs and i am so upset because I dont want him on this Steriod anymore, and I am being told that is all that will help him. PLEASE HELP ME... Thank you

    My dog was just prescribed pregablin for seizures or possible dyskinesia. He was on generic keppra, but his anxiety has greatly increased since he started that drug. Do you think I should be concerned in using pregablin as an alternative treatment for his seizures?

      Really a conversation to be had with your vet - I am afraid that I cannot comment on the management of individual animals although I am always happy to contact me via usual channels to ask advice on challenging cases.

      Yes you are right - I apologise - I got the sides mixed up (DUH!) - well spotted. the correct text is hypermetric gait on the left side (especially left fore and localised as paradoxical (right) vestibular syndrome involving left cerebellum

    Dear Professor Rusbridge, thank you so much for this video, it was very informative. I have two questions, please (unrelated to Pregabalin, though). What is your opinion on frunevetmab and bedinvetmab, please? I have been working with pets, and unfortunately it has been observed that many (some overall healthy, others with osteoarthritis) pets’ health deteriorated either shortly after the injection, or after few months of treatment. Some go blind, others get paralyzed or develop tonic/clonic seizures, ulcers, kidney issues, to name a few; or die. It has also been noticed that many vets in UK are prescribing those drugs either without a proper OA diagnosis, basing on presumptions/as a “just in case” solution; or for other health issues (for example, constipation, kidney issues…, toothache etc.). I believe many people, myself included, would greatly appreciate if you could please share your opinion on those medications. Second question relates to pet strokes and idiopathic epilepsy. Feline patient, age 13, healthy, suddenly developed occasional tonic seizures (only), diagnosed with idiopathic epilepsy and treated with Levetiracetam. Due to misdiagnosed stroke mistaken for an epileptic attack few months later, patient was prescribed Phenobarbital on top of Levetiracetam. Developed excessive scratching of ears and face, which continues months after, despite stopping Phenobarbital after 10 days. No underlying cause can be found (vet & neurologist). Would that be a likely result of the stroke? I would appreciate if you could please discuss feline stroke in one of the videos as well. Thank you in advance. Respectfully, J.       Thank you for your kind comments I have not see the problems that you describe with the nerve root growth factor inhibitors however I would say that many older dogs/cats that are "slowing up" may be initially assumed that their signs are due to osteoarthritis and given those drugs - when in fact something else is going on. Drug reactions are not typically months after giving a drug they are usually within days. It would be unusual for a stroke to cause a pain syndrome unless in the thalamus and in that instance the scratching would be one side only. So to which region of the brain was the stroke? Strokes are rare in cats in the UK - there typically has to be a predisposing cause of embolism (something blocking a block vessel) and most reported cases have involved the cerebellum . I don't think I have ever seen a stoke be a cause of acquired epilepsy in a cat. Idiopathic epilepsy is when the structure of the brain was normal. If I was to give a lecture on strokes in the cat then it would be quite short and mostly the cases in this paper and a couple more pubmed.ncbi.nlm.nih.gov/17317258/ . Some people confuse "strokes" with vestibular disease (balance problem) - some vets say "stroke" rather than vestibular disease because they think it is easier for caregivers to understand but generally that just causes confusion!

      @@clare-neurovet thank you so much for reply and information. They couldn’t find anything as underlying cause of the stroke, his MRI was clear. It happened right after he stopped eating, his bowl was directly on the floor; he started walking really slowly and in circles, crossing his front paws like he didn’t know how to walk, was very confused, dilated pupils. After few seconds he sat down. I took him to the vet straight away, and by the time we got there he was fine. Prescribed phenobarb on top of levetiracetam, and after few days the scratching (both sides) has started. I then insisted on referring him back to Queens Vet Hospital to see neurologist. They did all the tests and everything was ok, but saw the video of his stroke and disagreed with his vet’s epileptic attack diagnosis. Stopped phenobarb, left levetiracetam only, as it was too much for him with both meds. The scratching continues, although it’s a bit better with steroids. Since that event, I have raised his bowls 8 cm, and it never happened again. As for his epilepsy, it started last July, completely out of the blue. He started shivering for about 2 days, then started having paw, ear or whiskers twitches; these are his only symptoms. First, we suspected FARS, but again-neurologist wasn’t sure, his tests were normal (there were some changes in the brain, but minor and apparently normal at his age), so his diagnosis is FARS or idiopathic epilepsy. His twitches happen only when he lies down, either asleep or awake. No FARS triggering sounds are present.

    Thank you very much for the lectures and books about Syringomyelia! Prof. Rusbridge, could you please tell me, is it possible to get your online consultation? Is there an online appointment? We live in another country, my Cavalier King Charles Spaniel has Chiari and Syringomyelia (According to MRI, extension of the central channel in the area C1-Th1, up to a maximum of 7.7x6.5 mm). The dog is 11 months old; a veterinary neurologist recommends not to take gabapentin, but to do surgery (foramen magnum decompression). We have some doubts. There is an MRI disk and a video of symptoms (phantom scratching), if a consultation is possible, please let me know. Thanks a lot!

      generally quoted to be at least 80% successful but depends on the severity grade with less for grade 5 dogs and also depends on breed eg French Bulldogs do more poorly because of there other problems related to being flat faced (when anxious / painful their breathing difficulties can be life threatening or even fatal).

    My girl was diagnosed with Chiara but not SM at least yet. She also has allergies so it's been hard to figure out what's what. The Neuro implied she shouldn't have pain if she doesn't have SM but I strongly suspect at least some of her issues are Neuro pain.
